To verify the effect of immunosuppressants on the endotoxin-induced increase in iNOS activity, the action of immunosuppressants, dexamethasone (1.5 mg/kg), azathioprine (5 mg/kg/day) and cyclosporine (10 mg/kg), were evaluated in mice pretreated with LPS. The intraperitoneal injection of lipopolysaccharide (10 mg/kg) increased the nitric oxide synthase (NOS) activity in the brain and liver to maximum at 1 and 3 hours, respectively. The increase in NOS activity was blocked by the treatment with NOS inhibitor, LNAME(300 mg/kg) and aminoguanidine(100 mg/kg); a protein inhibitor, cycloheximide (10 mg/kg); and a transcription inhibitor of inducible NOS(iNOS), dexamethasone(1.5 mg/kg). Immunosuppressants, azathioprine (5 mg/kg) and cyclosporine (10 mg/kg), effectively blocked the increase in NOS activity. These results suggest that iNOS expression plays an important role in LPS-induced the increase in NOS activity and that immunosuppressants can be used as candidate for therapeutic agents in endotoxemia.

사람과 컴퓨터와의 정보 전달 수준이 사람과 사람 사이의 의사 소통과 같이 자연스럽게 이루어 지게 하기 위한 도구 중에서 제스쳐에 의한 정보 전달 메커니즘이 중요한 분야로서 자리 잡게 되었다. 이를 위해 제스쳐 인식에 관한 많은 연구가 진행되어 왔으나 영상처리 및 인식 부분의 어려움을 극복하기 위해 데이타 글로브나 구별이 쉬운 장갑 등의 외부 장비를 사용하거나, 연속적인 제스쳐 상에서 의미 있는 부분을 임의로 추출하여 인식하는 연구가 수행되어 왔다. 본 연구에서는 제스쳐의 인식을 순수히 시각정보에 의해 수행되도록 고려하였으며, 의미 있는 부분인 키-제스쳐를 구분해 내기 위하여 하나의 제스쳐가 세 가지 단계로 이루어 지고 각 단계 사이에는 속도의 변화가 있다는 조건을 강화하여 이를 스팟팅의 기본 알고리즘으로 활용하였다. 제스쳐의 궤적은 손의 움직임에 의한 중심점 토큰들의 집합으로 나타나며 이들은 극좌표 상에 투영된다. 극 좌표 상에서 제스쳐를 구성하는 토큰들은 LBG 알고리즘에 의해 군집화되며, 각 군집이 하나의 심볼로서 표현된다. 이러한 제스쳐를 구성하는 각 토큰들의 심볼들은 HMM(Hidden Markov Models) 상에서 학습되어 인식에 활용된다. 본 연구에서 대상으로 하는 제스쳐의 어휘는 기본적인 도형요소의 편집에 활용하기 위한 22개의 동적 제스쳐 이다. 각 제스쳐에 대해 100 개의 데이타를 수집하여 그 중 50개는 학습에 활용하고 나머지 50개는 인식 실험에 활용하였으며, 인식결과 95% 수준의 인식률을 얻었다. 또한, 개발된 시스템은 실시간으로 기본 도형요소의 편집이 가능하도록 개발되어 사람과 컴퓨터, 또는 사람에 의한 기기의 제어 등에서 활용될 수 있음을 확인하였다. Researches on the gesture recognition have become a very interesting topic in the computer vision area. Gesture recognition from visual images has a number of potential applications such as HCI (Human Computer Interaction), VR(Virtual Reality), machine vision. To overcome the technical barriers in visual processing, conventional approaches have employed cumbersome devices such as datagloves or color marked gloves. In this research, we capture gesture images without using external devices and generate a gesture trajectory composed of point-tokens. The trajectory is spotted using phase-based velocity constraints and recognized using the discrete left-right HMM. Input vectors to the HMM are obtained by using the LBG clustering algorithm on a polar-coordinate space where point-tokens on the Cartesian space are converted. A gesture vocabulary is composed of twenty-two dynamic ham) gestures for editing drawing elements. In our experiment, one hundred data per gesture are collected from twenty persons. Fifty data are used for training and another fifty data for recognition experiment. The recognition result shows about 95% recognition rate and also the possibility that these results can be applied to several potential systems operated by gestures. The developed system is running in real time for editing basic graphic primitives in the hardware environments of a Pentium-pro (200 MHz), a Matrox Meteor graphic board and a CCD camera, and a Window95 and Visual C++ software environment.

This retrospective study was performed to determine the rate of collapse in asymptomatic hips in patients with non-traumatic osteonecrosis. 32 hips in 32 patients with asymptomatic osteonecrosis were followed up for at least five years. The necrotic lesions of all hips were confirmed by MR imag-ing. The evaluation was made primarily based on radiographic features and MR findings according to the location and size of the necrotic lesion. Twelve(37.5%) of the femoral heads had collapsed in the average of fifty-two months. Gender, age, initial radiographic staging, and etiology did not affect the collapse. The collapse took place predominantly in cases involving at least one-third of the diameter of the head and the major weight bearing areas. These results indicate that symptomatic osteonecro-sis is at signification risk of collapse, especially when the necrotic lesion is extensive and involves a weight bearing area.

Arterial injuries after total hip replacement are very rate rare but serious complications. The authors report a patient who developed late ischemia and gengrenous change of the ipsilateral foot following total hip replacement. The patient was smoker and had a hypertensive heart disease. The ischemia after total hip replacement was probably the result of interruption of collateral circulation about the hip and overextension of atherosclerotic artery, intimal fracture with subsequent thrombus formation, Careful preoperative evaluation, early recognition of ischemic signs and prompt institution of appropriate management are essential to prevent this complication and to treat it adequately once it occurs.

Failure of fixation is a major complication of the treatment of proximal femoral fractures including trochanteric fracture. In a retrospective study we assessed the causes of failure in a series of 136 proximal femoral fractures which had been internally fixed with a compression hip screw. All patients were followed from 1 year to 6.6 years(average 2.5 years). The overall rate of fixation failure was 13.2%; excessive sliding of the lag screw(7.4%), cutting-out of lag screw before union(3.7%), varus malunion(1.5%), and pulling-out of the side plate(0.7%). Fixation failure was commoner in unstable fractures (p=0.0001) and osteoporotic patients(p=0.031). Fixation failure was also associated with superior placement of the lag screw within head(p<0.0001) and anteromedial displacement of the distal fragment(p<0.0001). These results indicate that these fractures should be reduced as accurate as possible and care must be taken to avoid the placement of the lag screw in the superior half of the head. Dynamic hip screw with a laterally mounted trochanteric stabilizing plate(DHS/TSP) or Gamma nail may be an aid in the treatment of these difficult fractures because the problem with the medialization or anterior displacement of the distal fragment frequently associated with the compression hip screw is prevented.
