        협진병원 근무 의사들과 종합병원 근무 의사들의 양.한방 협진에 대한 인식도

        Objectives : This study was performed to compare consciousness of doctors on cooperative practices of western medicine and traditional Korean medicine, and to provide policy implication for development of cooperative practices. Methods : The structured questionnaires were mailed to 132 doctors working in non-cooperative practicing university hospital and 77 doctors working in cooperative practicing hospitals in Busan metropolitan city. The response rate was 40.2% and 40.3% respectively. This survey was performed from 10 Oct. 2008 to 31 Oct. 2008. Results : The doctors working in general hospital had comparatively negative consciousness on basic concept, value and necessity for cooperative practices and traditional Korean medicine. In regards with disease treatment's effectiveness of cooperative practices, both groups evaluated musculoskeletal and immune disease were more effective than others. There were positive relationships between perception for cost-effectiveness and consciousness on intention to participate cooperative practices(p<0.05). Also doctors who experienced traditional medicine treatment had positive consciousness on cooperative practices(p=0.05). Conclusions : To activate cooperative practices of western medicine and traditional Korean medicine, some efforts should be carried out. These include promoting cooperative education programs in medical schools and traditional Korean medical schools, doing research on cost-effectiveness of cooperative practices, and trying to minimize legal and systemic restrictions for cooperative practices.

      • 평행봉 Healy 버티기 동작의 3차원 운동학적 분석

        The purpose of this study is to examine the kinematical mechanism on Healy rwist to reward swing in support motion, and to grope for the better swing technique on the parallel bar. The subjects were four national ranked gymnasts who participated in the 1996 olympic game and four collegiate gymnasts. 2 S-VHS vedio caneras were used for the three-dimentional cinemerographical analysis. Also a rectangular shape control point containing markers of known coordinates, which encompassed the space of movement, was constructed and filmed to permit caliblation of the movement, After the filming of the control point it was removed and the Healy twist to reward swing in support motion techniques of each subject were filmed. And for the synchronization of three cameras, it was used the moment of braking out the ballon as the syn-event. The Direct Linear Transformation(DLT) technique was employed to obtain 3-D position coordinates and a cubic spline function was used for smoothing and differentiation. And the selected kinematical variables were the leaning arms angles at release, the leaning arms gngles at regrasp, the shoulder and hip angle, the velocity change of the center of the mass and the horizontal and vertical displacement of the center of the mass, T-test was used to test for differences between the skilled and the less-skilled gymnasts in the selected kinematical variables using SPSS-PC statistical program. Based on the results of this study, the following conclusions are drawn; 1) Angle Variable There are statistically no significant difference between two group in the all of the angle variables such as leaning arm of at release & at regrasp, shoulder & hip angle of at regrasp. but The mean values between two group are all the difference. The skilled group all the larger angles than the less-skilled group. 2) Displacement Variable(center of mass) There are statistically no significant difference between two group in the all of the displacement variables at each phase, but the mean values between two group are all the difference. The skilled group showed the longer displacement than the less-skilled group in all the horizontal displacement at each phase and vertical displacement at regrasp, vertical position. The less-skilled group showed the longer vertical displacement than the skilled group at release and horizontal position. 3) Velocity Variable(center of mass) There are statistically no significant difference between two group in all of the velocity variables at all phase. but there are all the difference between the mean value of groups. The skilled group showed the higher resultant velocity at 1st and 2nd phase and the less-skilled group showed the higher resultant velocity at 3rd phase. The skilled group showed the higher horizontal velocity at 1st phase and the lower horizontal velocity at 2nd and 3rd phase than the less-skilled group. The skilled group showed the lower vertical velocity except 2nd phase than the less-skilled group. In summary, the skilled group performs the Healy twist to reward swing in support motion with the larger angle variables. This leads to increase the leaning arm angle and the height of regrasp position. The increased leaning arm, shoulder and hip angles lead to increase the velocity of the Healy motion. Therefore, the body position without crouch at all phase and the higher center of the mass at regrasp make easy to perform more than skills after the Healy swing motion.

        달리기 시 일정한 속도에서 보폭 차이가 하지 관절의 3 차원 모멘트에 미치는 영향

        The purpose of this study was to determined the force and moment of the ankle and the knee joint at different step length relative to the length of the lower extremity during a steady running. Six digital cameras(Qualisis) and a forceplatform(A.M.T.I) were used to obtain the kinematic data of the segments and kinetic data on the running at speed of 5.18m/s. The force and moment measured from six subjects participated in this study were limited to the support phase and their values were averaged at the moment of heel strike, mid stance, and toe off of a running for making a comparison between the condition 1(relative step length 1.1) and the condition 2(relative step length 1.4). It was concluded that internal forces except mediolateral force of the condition 2 were greater in the ankle and the knee joint than those of the condition 1, but all moments of condition 2 were greater from the descriptive statistic point of view. For the future study, it was needed to consider a number of subjects, a various running speed, and a individual step preference for applying generally results to the running strategy.

        Relationship between Center of Pressure and Local Stability of the Lower Joints during Walking in the Elderly Wome

        Objective: The purpose of this study was to determine the relationship between center of pressure (CoP) and local stability of the lower joints, which was calculated based on approximate entropy (ApEn) during walking in elderly women. Method: Eighteen elderly women were recruited (age: 66.4±1.2 yrs; mass: 55.4±8.3 kg; height: 1.56±0.04 m) for this study. Before collecting data, reflective marker triads composed of 3 non-collinear spheres were attached to the lateral surface of the thigh and shank near the mid-segment to measure motion of the thigh and shank segments. To measure foot motion, reflective markers were placed on the shoe at the heel, head of the fifth metatarsal, and lateral malleolus, and were also placed on the right anterior-superior iliac spine, left anterior-superior iliac spine, and sacrum to observe pelvic motion. During treadmill walking, kinematic data were recorded using 6 infrared cameras (Oqus 300, Qualisys, Sweden) with a 100 Hz sampling frequency and kinetic data were collected from a treadmill (Instrumented Treadmill, Bertec, USA) for 20 strides. From kinematic data, 3D angles of the lower extremity's joint were calculated using Cardan technique and then ApEn were computed for their angles to evaluate local stability. Range of CoP was determined from the kinetic data. Pearson product-moment and Spearman rank correlation coefficient were applied to find relationship between CoP and ApEn. The level of significance was determined at p<.05. Results: There was a negative linear correlation between CoP and ApEn of hip joint adduction-abduction motion (p<.05), but ApEn of other joint motion did not affect the CoP. Conclusion: It was conjectured that ApEn, local stability index, for adduction/abduction of the hip joint during walking could be useful as a fall predictor.

        달리기 시 신체 충격 크기와 흡수의 성차

        J. S. RYU. Gender differences in the impact magnitude and its attenuation during running. Korean Journal of Sport Biomechanics, Vol. 15, No. 1, pp. 91-109, 2005. The goal of this research was to determine whether gender differences exist in impact force and impact shock variables at stance phase during a preferred running. Ten male and ten female subjects volunteered to participate in this study. Impact force was quantified by using a surface-mounted force plate. In addition, Axial accelerations of the tibias and mouth were measured using low-mass accelerometers. Comparison of parameters relating to impact force and impact shock which attained from time domain, and impact shock parameters which were analyzed in frequency domain were made between genders. The conclusions based on results were as follows; 1. There were no significantly differences in impact force, mouth and tibia acceleration peak in time domain between two genders. 2. The male group was greater in impact shock peak of PSD(power spectral density) at the tibia than female group(p<05), but no differences in active impact of FSD at the tibia and the mouth between two genders. 3. Female subjects exhibited that a peak of impact shock attenuation analyzed in frequency domain moved toward a high frequency, but no difference in time domain between two genders.

      • 축구 인스텝 킥(instep kick)시 하지 분절의 속도 요인과 볼 속도와의 관계

        The purposes of this study were to (1) analyze the kinematic factors at 3 dimension by phase and (2) analyze the relationship between ball speed and swinging lower segments when 8 H-university soccer players conducting instep kicking movements. The results were as follows; (1) In the velocity, the thigh segment showed large-score by 1-phase and maximized. at the 4-phase, shank+foot segment maximized at 5-phase when ball was impacted. Generally, the velocity for the shank+foot segment showed high relationship with ball velocity during all phases and showed the hieghest correlations at 5-phase when the ball was impacted. (2) In the acceteration factor, the thigh segment increased at 1 phase and showed the greatest change at 4-phase. The acceleration for the shank+foot segment in creased at 1-phase very slowly and showed the greatest change at 5-phase. The relationship between acceleration of the shank+foot segment and the ball velocity showed low correlations for all phases. (3) In angular-velocity factor, the thigh segment showed large-score at 4-phase, the shank+foot segment showed large-score at 5-phase. The relationship between angular-velocity of the shank+foot segment and the ball velocity showed very low correlations for all phases. (4) In angular acceleration factor, the thigh segment showed linear in creasement until 3 phase, but showed negative phenomenon at 5-phase. The angular acceleration for the shank+foot segment showed negative phenomenon at phase, but increased very sharply at 5-phase. The relationship between the angular-acceleration of the thigh segment and the ball velocity showed high correlations at 5-phase, but showed low correlations with the angular-acceleration of shank+foot segment for all phases.

        노인 보행 시 하지 근 활동 양상과 관절의 안정성이 낙상에 미치는 영향 -전향적 연구(Prospective Study)-

        Objective: The aim of this study was to determine the peak torques of the knee and ankle joint and local stability of the lower extremity's joints, and muscle activation patterns of the lower extremity's muscles between fallers and non-fallers in the elderly women during walking. Method: Four elderly women (age: 74.5±5.2 yrs.; height: 152.1±5.6 cm; mass: 55.3±5.4 kg; preference walking speed: 1.19±0.06 m/s) who experienced falls within six months since experiment had been conducted (falls group) and thirty-six subjects (74.2±3.09 yrs.; height: 153.6±4.9 cm; mass: 56.7±6.4 kg; preference walking speed: 1.24±0.10 m/s) who had no experience in falls (non-falls group) within this periods participated in this study. They were measured torque peaks of the knee and ankle joint using a Human Norm and while they were walking on a treadmill at their natural pace, kinematic variables and EMG signals were collected with using a 3-D motion capture system and a wireless EMG system, respectively. Lyapunov Exponent (LyE) was determined to observe the dynamic local stability of the lower extremity's joints, and muscles activation and their co-contraction index were also analysed from EMG signals. Hypotheses between falls and non-falls group were tested using paired t-test and Mann-Whitey. Level of significance was set at p<.05. Results: Local dynamic stability in the adduction-abduction movement of the knee joint was significantly lower in falling group than non-falling group (p<.05). Conclusion: In conclusion, muscles which act on the abduction-adduction movement of the knee joint need to be strengthened to prevent from potential falls during walking. However, a small number of samples for fallers make it difficult to generalize the results of this study.
