Gastroscopy and colonoscopy are widely used for the early diagnosis of stomach and colorectal cancer. The present revision integrates recent data regarding previous quality indicators and novel indicators suggested for gastroscopy and colonoscopy procedures for the National Cancer Screening Program in Korea. The new indicators, developed by the Quality Improvement Committee of the Korean Society for Gastrointestinal Endoscopy, vary in the level of supporting evidence, and most are based solely on expert opinion. Updated indicators validated by clinical research were prioritized, but were chosen by expert consensus when such studies were absent. The resultant quality indicators were graded according to the levels of consensus and recommendations. The updated indicators will provide a relevant guideline for high-quality endoscopy. The future direction of quality indicator development should include relevant outcome measures and an evidence-based approach to support proposed performance targets. The acoustic characteristics of a direct radiator type loudspeaker has been studied in this paper. The natural modes of the speaker cone vibration analyzed numerically by the finite element method have been verified by comparing them with experimental results. The so-ap-proved finite-element model has been used to calculate the vibration response of the cone excited by the voice coil. The vibration displacement of the speaker cone paper has been converted into the vibration velocity and used as a boundary condition for the acoustic analysis. The frequency characteristics, directivity, and sound pressure distribution of the loudspeaker have been calculated by the boundary element method. The numerical results have been verified by the experiments carried out in an anechoic chamber. The variations of the acoustic characteristics due to the changes of some design parameter values can be examined using the numerical model.
위식도역류질환이 의심되는 환자에서 다채널 강내 임피던스-pH 검사의 유용성

목적: 위식도역류질환의 진단에 다채널 강내 임피던스-pH 검사의 유용성을 알아보고자 하였다. 첫째, 위식도역류질환에 대한 다채널 강내 임피던스-pH 검사의 유용성을 알아보고자 하였다. 둘째, 증상지수 양성인 환자를 대상으로 산역류 또는 비산역류와의 증상과의 관련성을 알아보고자 하였다. 대상 및 방법: 위식도역류질환이 의심되는 총 74명의 환자를 대상으로 다채널 강내 임피던스-pH 검사를 시행하고 결과를 분석하였다. 결과: 1) 보행성 24시간 식도 pH 검사상 위식도역류질환 양성인 환자 수는 20예였으며 산에 대한 증상지수 양성 (SI>50%) 환자 수는 15예였다. 나머지 39예 중 다채널 강내 임피던스 pH 검사로 측정한 비산 역류에 의한 증상지수 양성은 9예, 식괴 역류 총 %시간이 증가된 경우 8예로 총 17예가 위식도역류질환으로 추가 진단되었다. 2) 다채널 강내 임피던스 pH 검사로 측정한 증상지수 양성 29명 중에 산성 역류 시에는 산 역류 증상을 더 많이 호소하였고 비산성 역류 시에는 흉부작열감과 비심인성 흉통을 더 많이 호소하였다. 결론: 다채널 강내 임피던스-pH 검사는 위식도역류질환을 진단하는데 예민한 검사법으로 생각할 수 있고 역류물과 증상과의 상호관계를 알 수 있었다. Background/Aims: To investigate the usefulness of multichannel intraluminal impedance-pH metry (MII-pH) as a diagnostic tool for gastroesophageal reflux disease (GERD): 1) the usefulness of MII-pH to increase the diagnostic sensitivity of GERD; and 2) to analyze whether various GERD symptoms were correlated with acid and nonacid reflux in patients with a positive symptom index (SI). Methods: We retrospectively compared the results of pH-metry and the results of MII-pH metry from the data of MII-pH in 74 patients with suspected GERD. Results: 1) Twenty out of 74 patients were diagnosed with GERD by pH monitoring, and an additional 15 cases had a positive SI (> 50%). Out of the remaining 39 patients, a positive SI for nonacid reflux was present in nine patients. A total of 17 cases were added to the GERD diagnosis including eight patients who showed an increase in total bolus reflux time percentage by MII-pH. 2) Regurgitation was more commonly observed with acid reflux in 29 patients with positive SI, but heartburn and noncardiogenic chest pain were more commonly correlated to nonacid reflux. Conclusions: Multichannel intraluminal impedance-pH metry is a sensitive method for the diagnosis of GERD and informs the correlation between symptoms and reflux. N/A Backgroud/Aims: To evaluate the factors which are related to the transition from achalasia to diffuse esophageal spasm (DES) or nutcracker esophagus (NE) after botulinum toxin inj ection to lower esophageal sphincter (LES). Methods: This study included the 23 patients with achalasia who received an intrasphincteric injection of botulinum toxin. Stational esophageal manometry, 24-hour ambulatory esophageal manometry with Ph monitoring, barium esophagogram and endoscopic ultrasonography were performed before and after treatment. We analyzed the parameters from these studies between the cases that transformed to DES or NE within a week and the cases that do not transit. Result: Five patients (21.7%) transformed to DES (1) or NE (4) within a week. There were significant differences in contraction amplitude of esophageal body (median, 31mmHg vs 23 mmHg, p<0.05) and maximal diameter of esophageal body (median, 2.6 cm vs 4.4 cm, p<0.05) between these five patients and the remaining patients. There were no significant differences in sex, LES pressure and thickness of muscle layer between two groups. Conclusion: Factors involved in transition to NE or DES after botulinum toxin injection to LES of achalasia appears as high amplitude contractions in body of esophagus and less dilation of esophageal body. 도재브라켓 접착면의 처리방식이 전단결합강도에 미치는 영향

The purpose of this study was to evaluate the effects of different bases of ceramic brackets on shear bond strength and to observe failure patterns of bracket bondings. Lower bicuspid brackets whose bases designed for the macromechanical and silane treated chemical bonding, those for silane treated chemical bonding, those for micromechanical bonding, and those for macromechanical bonding were tested as experimental groups, and foil mesh-backed metal brackets as a control group. All the brackets were bonded with Mono-Lok2( ™)on the labial surface of extracted human lower bicuspids after etching the enamel with 38% phosphoric acid solution for 60 seconds. The shear bond strengths were measured on the universal test machine after 24 hours passed in the 37℃ water bath. The gathered data were evaluated and tested by ANOVA and Duncan's multiple range test, and those results were as follows. The shear bond strengths of brackets for macromechanical and chemical bonding, those for chemical bonding, and those for micromechnical bonding were not different (p>0.05), but showed statistically higher than those of metal bracket and those of ceramic bracket for macromechanical bonding(p<0.05). The shear bond strengths of ceramic bracket for macromechanical bonding showed statistically lower than those of metal bracket(p<0.05). The enamel fractures and/or ceramic bracket fractures were observed in the cases of higher bond strength than that of metal bracket. These results supported that silane treated base of ceramic bracket show higher bond strength than of metal bracket, and suggested that micromechanical form of ceramic bracket bases show higher shear bond strength than that of macromechanical form.
