        Long Term Result of Pediculectomy in Symptomatic Isthmic Spondylolisthesis with Severe Osteoporosis

        Objective : Patients with symptomatic root compression in isthmic spondylolisthesis are commonly treated by bilateral wide posterior decompression and concomitant fusion with transpedicular instrumentation. But, in cases associated with severe osteoporosis or poor general conditions for surgery, high fusion failure rate or high operation risk is expected. The authors report the results of partial pediculectomy for root decompression without fusion procedure in 7 cases. Followed up period was over 3 years. Methods : From December 1993 to May 1998, we performed operation in 87 patients with isthmic spondylolesthesis. Posterior lumbar interbody fusion was performed in 80 cases. Unilateral or bilateral removal of about 3mm length of infero-medial part of pedicle was performed in six cases who were associated with severe osteoporosis (T < -3.0 , BMD) and in 1 case with high risk cardiac disease. Results : Radiating pain disappeared immediately after the surgery in all cases. There were no increasing instability after surgery. In long-term follow up, aggravation of symptoms were observed in 3 cases. In one of these 3 patients (high risk cardiac disease patient) bilateral wide decompression and bone fusion with transpedicular screwing was performed. Conclusion : In the surgical treatment of osteoporotic isthmic spondylolisthesis with foraminal stenosis in which transpedicular screwing and fusion is not indicated, partial pediculectomy without stabilization may be a effective method for relieving radicular pain, but in part of these patients symptoms may recur in long-term follow up.

        ADI의 미세조직과 기계적 특성에 미치는 특수열처리 및 합금원소의 영향

        The effects of heat treatments and alloying elements on the microstructure and mechanical characteristics of specially austem-pared ADIs containing alloying elements such as Cu, Mo and Ni were investigated. To compare with the effect of conventional and normal(CN) austempering treatment, two kinds of special austempering treatments which pre-quenching and pre-heating were conducted. The hardness and uniaxial fatigue tests were carried out to evaluate the mechanical characteristics. The hardness of ADI treated by CN heat cycles was higher than those of other ADIs. Cu added ADI(Cu-ADI) tempered at 400 after austenitizing exhibited the highest fatigue life cycles. While austempering after prequenching makes the austenite with high carbon stable, which resulted in transformation to highly strengthened bainites from the carbon enriched austenite phases during tempering. The high carbon enriched banites is considered to improve the fatigue strength.(Received January 5, 2002)

      • 족배부 복합 피부-건 유리피판을 이용한 Achilles건의 일단계 재건술

        The soft tissue defects including the Achilles tendon are complex and very difficult to reconstruct. Recently, several free composite flaps including the tendon have been used to reconstruct large defects in this area in an one-stage effort. Our case presents a patient reconstructed with free composite dorsalis pedis flap along with the extensor digitorum longus and superficial peroneal nerve for extensive defects of the Achilles tendon and surrounding soft tissue. A 36-year-old-man sustained an open injury to the Achilles tendon. He was referred to our department with gross infection of the wound and complete rupture of the tendon associated with loss of skin following reduction of distal tibial bone fracture. After extensive debridement, $6{\times}8cm$ of skin loss and 8cm of tendon defect was noted. Corresponding to the size of the defect, the composite dorsalis pedis flap was raised as a neurosensory unit including the extensor digitorum longus to provide tendon repair and sensate skin for an one-stage reconstruction. One tendon slip was sutured to the soleus musculotendinous portion, the other two were sutured to the gastrocnemius musculotendinous portion with 2-0 Prolene. The superficial peroneal nerve was then coaptated to the medial sural cutaneous nerve. The anterior tibial artery and vein were anastomosed to the posterior tibial artery and accompanying vein in an end to end fashion. After 12 months of follow-up, 5 degrees of dorsiflexion due to the checkrein deformity and 58 degrees of plantar flexion was achieved. The patient was able to walk without crutches. Twopoint discrimination and moving two-point discrimination were more than 1mm at the transferred flap site. The donor site healed uneventfully. Of the various free composite flaps for the Achilles tendon reconstruction when skin coverage is also needed, we recommand the composite dorsalis pedis flap. The advantages such as to control infection, adequate restoration of ankle contour for normal foot wear, transfer of the long tendinous portion, and protective sensation makes this flap our first choice for reconstruction of soft tissue defect including the Achilles tendon.

      • 복합 내측 족저부 및 내측 족부 피판을 이용한 광범위 족저부 결손의 재건

        The reconstruction of soft tissue defects of the sole requires to stand the force of weight bearing, provide sensation and adequacy for normal foot-wear. Although certain local flaps have been described and used for resurfacing the foot, extensive injury requires distant or free flaps for coverage. There is no doubt that the ideal tissue for resurfacing the sole is the plantar tissue itself. The specialized dermal-epidermal histology and fibrous septa of the subcutaneous layer gives its unique property to stand the pressure and to absorb the shock upon gait. This paper presents a case of reconstructing the sole that involves about 70% of the weight bearing portion. The combined medial plantar and dorsalis pedis chimeric free flap based on the medial plantar artery and medial plantar nerve adds another dimension in resurfacing the weight bearing sole of moderate to large sized defects.

        혁신적 인사시스템이 인적자원역량 및 조직효과성에 미치는 영향에 관한 연구: 중소기업과 중견기업의 비교를 중심으로

        [Purpose]The purpose of this study is to demonstrate the effect of the innovative HR system on HR competence and organizational effectiveness for SMEs and MSEs, and to provide implications for the development of HR systems that achieve high performance according to the size of the company. [Methodology]Based on previous studies on strategic HRM systems, this study has thoroughly discussed the mechanisms between innovative HR systems and organizational effectiveness. This study analyzed the response data of 9,090 workers from 378 companies in the 7th HCCP data. The hypothesis was verified by the structural equation modeling analysis. [Findings]For both SMEs and MSEs, the training bundle does not affect HR competence and organizational effectiveness. Motivation bundles do not affect organizational effectiveness in SMEs, but have a positive effect on job satisfaction and organizational performance in MSEs. Participation opportunities have a greater effect on organizational effectiveness in SMEs than MSEs, and motivation bundles have a greater effect on MSEs than SMEs. [Implications]This study demonstrates that the positive effects of the innovative HR system are different depending on the type of company. So, it is necessary to develop a HR system suitable for SMEs and MSEs. [연구목적]본 연구에서는 중소기업과 중견기업을 대상으로 혁신적 인사시스템이 인적자원역량 및 조직효과성에 미치는 영향력을 실증하여, 기업 규모에 따라 고성과를 추구하는 인사시스템 개발에 대한 시사점을 제공하고자 하였다. [연구방법]본 연구는 전략적 인적자원관리 시스템에 관한 선행연구들을 바탕으로, 혁신적 인사시스템과 조직효과성 사이의 메커니즘에 대한 논의를 충분히 검토하여, 가설을 도출하였다. 한국직업능력개발원의 인적자본기업패널 7차 자료의 378개 기업, 9,090명의 근로자 응답 데이터로 분석하였으며, 구조방정식 모형으로 가설을 검증하였다. [연구결과]중소기업과 중견기업 모두 교육훈련 묶음이 인적자원역량 및 조직효과성에 영향을 미치지 않는다. 중소기업에서는 동기부여 묶음이 조직효과성에 영향을 미치지 않으나, 중견기업의 경우는 직무만족, 조직성과에 긍정적 영향을 미치고 있다. 참여기회 묶음은 중견기업보다 중소기업에서 조직효과성에 더 큰 영향을 미치고, 동기부여 묶음은 중견기업에서 중소기업에서 보다 더 큰 영향을 미친다. 중소기업과 중견기업 모두 인적자원역량이 조직효과성에 유의한 영향을 미치는 것으로 나타났다. [연구의 시사점]기업 유형에 따라 혁신적 인사시스템의 제도가 미치는 긍정적 효과가 다름을 실증하였다. 즉, 대기업 위주로 만들어진 기존의 인사시스템이 아니라 중소기업과 중견기업에 적합한 인사시스템의 개발이 필요하다는 시사점을 제시한다.

        Development of a Predictive Model for the Reduction of Escherichia coli on the Surface of Materials as a Function of Concentration and Treatment Time with Ethanol

        The purpose of this study was to develop a model that could predict the reduction of Escherichia coli on different surfaces treated with ethanol as a function of concentration and treatment time. The reduction in E. coli on the surfaces of stainless steel, plastic, wood, rubber,glass, and ceramic at various ethanol concentrations (0-70%) after 0-5 min treatment times were evaluated. The contamination levels of E. coli were reduced to 6.73, 5.49,4.68, 6.38, 5.40, and 6.30 log CFU/cm^2 on the surface of stainless steel, plastic, wood, rubber, glass, and ceramic,respectively, when treated with a maximum concentration (70%) of ethanol for 5 min. Each predictive model was significant (p<0.05) and the fitness was defined by the lack of fit and probability of normal residuals. Therefore, these models could be used to determine the minimum required ethanol concentrations and treatment times to control E. coli on the surface of various materials.

        6차산업 인증 프로세스를 검토하고 그 의의를 파악하여 농업융복합 사업을 위해 필요한 준비사항을 점검하고 실행할 수 있도록 프로세스를 정립하고자 한다. 6차산업과 관련된 선행연구와 농식품부 최근 정책방향, 6차산업 추진현황을 검토하고 인증관련 세부 프로세스와 내용을 체계화 하고자 하였다. 연구결과는 인증세부 절차 및 평가기준을 명확화하여 기존 농업 경영체 및 창업기업이 최적의 사업 준비를 할 수 있도록 지원하고자 제안점을 제시하였다. 6차산업인증 프로세스와 인증 시 필요한 제반 실무를 시행착오 없이 신속히 수행할 수 있는 방안 제시하였다.

        Cytomegaloviurs (CMV) usually causes severe infection only in immunocompromised patients. However, in rare cases, gastrointestinal infection can occur in patients with normal immunity. Though most such patients recover spontaneously, major complications such as massive hemorrhage have also been reported. We report a case of massive hemorrhage in an immunocompetent elderly patient which was successfully treated with angiographic embolization.

        겨우살이 종자 발아 및 화아 배양에 의한 캘러스 형성

        Effects of growth regulators and culture conditions on seed germination, haustorium development, and callus formation of Korean mistletoe (Viscum album var. coloratum (Kom.) Ohwi) were described. Histological examination showed that seed of V. album contained one or two zygotic embryos with rod shape, and actively dividing cells were mainly distributed in radicle region rather than cotyledon of zygotic embryo. The most significant factor for seed germination and haustorium development of V. album was the requirement of the light. Various growth regulators examined in this study failed to substitute the effect of the light on seed germination. The frequency of callus formation was highest at 27.3% when flower buds were cultured onto B5 medium containing 0.1 mgl-1 IAA. Explants from other organs were recalcitrant in forming calluses. Culture conditions described in this study could be applied for production of useful metabolites and multiplication of V. album in future.
