Henoch-Schonlein purpura (HSP) is an immunologically mediated systemic vasculitis of small blood vessels that primarily involves the skin, gastrointestinal tract, joints, and kidneys. HSP is a relatively common vasculitic syndrome in children, which usually resolves within several weeks, and requires no treatment. The pulmonary parenchymal involvement of HSP is a rare complication that predominantly occurs in adolescents and adults. Mortality following HSP pulmonary presentation is extremely high. Here, we report a case of HSP combined with pulmonary hemorrhage as a presentation of the vasculitic involvement of the lung. Recently, we experienced a case of a 49-year-old male patient with HSP who presented with palpable purpura, enteritis, and glomerulonephritis. Following the diagnosis of HSP, the patient developed sudden pulmonary hemorrhage due to the pulmonary involvement of vasculitis, and recovered following glucocorticoid therapy. Dry sliding wear behavior of electro-pressure sintered Fe-Ni and Co-Fe-Ni compacts was investigated. Pin-on-disk wear tests were performed on the sintered Fe-Ni, Co-Fe-Ni disk specimens against alumina $(Al_2O_3)$ and silica $(SiO_2)$ ball counterparts at various loads ranging from 3N to 12N. A constant sliding speed of 0.1m/sec was employed. Wear rate was calculated by dividing the weight loss measured after the test by specific gravity and sliding distance. Worn surfaces and cross sections of them were examined by a scanning electron microscopy, and wear mechanism of the compacts was investigated. Wear characteristics of the compacts were discussed as a function of composition of the compacts. Relationship between the wear rate and mechancial properties of the compact was explored, and effects of the oxide layer that was formed on wearing surface of the compacts on the wear were also studied.

인터넷 기술의 발전과 웹 사용자의 증가로 웹에는 매우 많은 정보들이 축적되고 있으며, 여기에는 또한 매우 많은 지역정보가 존재한다. 일반적으로 웹 정보라고 하는 것은 웹의 컨텐츠, 사용이력, 구조를 중심으로 한 정보이며, 이러한 정보를 하나의 중요한 공간 정보원으로 취급하여 웹으로부터 특정 지역에 관한 컨텐츠를 수집하고 이를 분석하여 사용자의 공간 정보원으로서 사용할 수 있다. 또한 인터넷, 무선통신, 지리정보시스템(GIS), 위치기반서비스(LBS) 등의 기술들이 모바일기기에 복합적으로 접목되면서 모바일기기에서의 새로운 형태의 정보에 대한 응용이 요구되고 있다. 본 논문에서는 이러한 웹 환경을 활용하고, 모바일 환경을 고려하여 GIS와 Web을 통합한 모바일 지역정보 검색 시스템을 제안한다.
의료의 질 향상 및 환자안전을 위한 제도 도입에 관한 고찰

The landmark report To Err is Human, by the Institute of Medicine in 1999, cited studies that found that at least 44,000 people and potentially as many as 98,000 people die in United States hospitals each year as a result of preventable medical errors. So, medical errors were a big issue in U.S. And U.S. Congress make the legal system for preventing medical errors. The medical mistakes can be prevented by designing and utilizing the medical errors database system through reporting adverse events. But medical providers are reluctant to report their adverse events and participate in quality review activities for fear of liability or injury to their reputations. This problem can cause to prevent the accumulation of a sufficient number of adverse events. But if medical providers have the privilege of exemption from liability when they report their adverse events on the Patient Safety and Quality Improvement Act, report system for the medical errors database system may operate properly. Through a comparative study on the Patient Safety and Quality Improvement Act of foreign countries, we can draw implications for enacting the Patient Safety and Quality Improvement Act in Korea.
일측 서혜탈장 환아에서 반대측 서혜탈장 발생에 대한 임상적 연구

Contralateral exploration in children with unilateral inguinal hernia is controversial. This study was done to identify risk factors for the development of contralateral inguinal hernia in patients with unilateral inguinal hernia. The clinical experience of 4,206 inguinal hernias repaired by one pediatric surgeon on 3,358 children at HanYang University Hospital from September 1979 to December 2002 was analyzed. 1,868 (55.6%) hernias occurred on right side, 1,190 (35.4%) on left side, and 300 (8.9%) were bilateral. 2,702 children were boys and 656 were girls (M:F = 4.1:1). 170 children of 3,058 children with unilateral hernias (5.6%) developed contralateral inguinal hernia at 1 day to 95 months after herniotomy. 146 children were boys and 24 were girls (M:F = 6.1:1). The patients who had had herniotomy before 1 year of age developed contralateral hernia in 17.4%, compared with 5.6% overall average. The earlier the first herniotomy was performed, the more frequently contralateral hernia developed. The occurrence of contralateral inguinal hernia was more frequent in boys (146 of 2,460, 5.9%) than girls (24 of 598, 4.0%) and more frequent after left herniotomy (80 of 1,190, 6.7%) than after right herniotomy (90 of 1,868, 4.8%), but statistically not significant. 52.9% of contralateral inguinal hernia developed within 1 year after hernia repair, and 87.6% developed within 3 years. Routine exploration on contralateral side in children is not necessary.
