체계적 분석, 진단, 치료 계획을 통한 전악 보철 재건 증례

성공적인 보철 치료를 위해서는 정확한 진단이 근거가 되어야 한다. 즉, 처음 환자가 내원하였을 때, 환자의 주소를 바탕으로 한 문제점을 찾고, 그 원인을 분석하여 이에 입각한 치료 계획을 세웠을 때, 만족스러운 결과를 기대할 수 있을 것이다. 본 증례의 환자는 77세 여성으로, 저작 시 상 하악 좌측 소구치 부위의 통증 및 기존 보철물의 비심미성을 주소로 내원하였다. 임상적, 방사선학적 검사 및 모형 분석을 통해 문제점을 파악하였고, 여러 문헌에 근거하여 지대치 평가, 보철물의 여러 가지 디자인에 따른 예후 및 최종 보철물의 장착 시 유지 관리의 용이성 등을 고려하여 치료 계획을 수립하기로 하였다. 심미적 문제, 보철을 위한 악간 공간의 부족을 해소하고자 최소한의 수직 고경 증가를 동반한 전악 수복을 시행하기로 결정하였다. 기능, 심미성 및 교합의 안정을 통한 환자의 적응도를 평가하여 치료 계획의 적절성 여부를 판단할 수 있었다. 따라서, 최종적인 고정성 국소 의치를 제작하고 장착하여 만족스러운 결과를 얻었기에 이를 보고하는 바이다. Data collection including checking the problem list and analyzing the etiology cannot be overemphasized in full mouth rehabilitation cases. It is well reported that favorable prognosis of prosthodontic treatment depends on accurate diagnosis. Seventy seven year old female patient presented with chief complaints of chewing pain on maxillary and mandibular left premolars and unesthetic existing prosthesis. Clinical evaluation, radiographic examination and cast analysis were performed to gather comprehensive data and problem lists. Treatment planning was based on collected data and related articles. Abutment evaluation, prognosis depending on various types of prosthesis, and maintenance were considered as well. Occlusal vertical dimension was increased to solve esthetic dissatisfaction and insufficient interocclusal space. Patient adaptation was verified with provisional restorations followed by diagnostic wax up. Function, esthetics and occlusal stability were verified during 8 week follow-up period. Prosthodontic reconstruction based on systemic analysis, diagnosis, and treatment plan led to satisfactory result after delivery of definitive prosthesis.

여자 청소년들의 휴대전화 사용행태와 컴퓨터단말기증후군 증상경험

본 연구는 여자 청소년들의 휴대전화 사용행태와 컴퓨터단말기증후군 증상경험을 파악하기 위한 서술적 조사연구이다. 자료 수집은 2013년 5월 6일부터 6월 3일까지 중학교 및 고둥학교에 재학 중인 여학생들을 대상으로 VDT 증후군 자각증상 조사표를 이용하였다. 수집된 자료는 SPSS 21.0 프로그램을 이용하여 기술통계 및 Kruskal-Wallis test, Mann-Whitney U test 등으로 분석하였다. 연구결과 대상자들의 96.3%가 휴대전화를 사용하고 있었으며 컴퓨터단말기증후군 증상을 하나 이상 경험한 비율은 87.7%였으며, 증상경험 점수는 .39±.41점이었다. 눈 관련 증상을 가장 많이 경험하였으며, 근골격계 증상의 정도가 가장 심하였다. 컴퓨터단말기증후군 증상경험은 휴대전화 사용자세, 사용시간, 사용 중 휴식여부에 따라 차이가 있었다. 따라서 휴대전화 사용 시 바른 자세로 사용하고, 사용 중에는 휴식 시간을 갖는 것이 컴퓨터단말기증후군 증상경험 완화에 도움이 될 것이다. 또한 여자 청소년들이 자신의 휴대전화 이용 상태를 객관적으로 점검할 수 있도록 휴대전화 이용 상태 진단 도구의 개발과 주기적인 점검 기회를 제공할 필요가 있으며, 휴대전화 중독 위험집단에게는 중재 프로그램 또는 예방교육이 필요한 것으로 보인다. The purpose of this study was to identify the relation between mobile phone use and visual display terminal syndrome symptom experience in female students from middle school and high school. The convenience sample consisted of 349 female students in four schools. The data were collected from May to June in 2013 using VDTS Subjective Symptom Questionnaire. Descriptive statistics, Kruskal-Wallis test, and Mann-Whitney U test were used to analyze the data using SPSS 21.0. Almost all (96.3%) female students use mobile phone. Among them, 87.7% of the subjects experienced more than one VDTS symptom and the average score was .39 ±.41 in VDTS symptom experience. Eye related symptom was most experienced and musculoskeletal symptom was the highest in severity among five subdomain of VDTS symptoms. There were significant differences in VDTS symptom scores in accordance with posture, hours of use, and break type during mobile phone use. These findings suggest that the education program for adolescents would be necessary, and the development of checklist would be useful for adolescents to check themselves.
