Graves 병에서 예후 인자로서의 혈청 TSH치의 의의

To evaluate the significance of TSH(IRMA) in diagnosing the remission of Graves' disease with antithyroid drug therapy and to compare it with TRH stimulation test and TBII activities, serum TSH concentrations were measured by immunoradiometric assay(IRMA) in 18 patients with untreated Graves' disease, 23 patients with remission, and 11 normal controls. The results were as follows: 1) Mean TSH value in untreated Graves' disease (0.06±0.1 U/ml) was significantly lower than those of normal controls (1.45±0.16 U/ml) and Graves' disease with remission (1.03±0.13 U/ml). 2) All the TSH values in untreated Graves' disease were below the normal range. Among the 16 patients in Graves' disease with remission, 15 patients (93.8%) were within the normal range of basal TSH value. 3) Mean TBII activity in untreated Graves' disease (37.83±5.90%) was increased compared with normal controls (1.82±0.40%), also with Graves' disease with remission (7.84±2.96%), significantly. 4) Sensitivity and specificity by TSH mesurement A were 93.8% and 100% while those were 87.5% and 57.1% by TBII activities. With the above results, it could be concluded that measuring TSH(IRMA) is a good and useful index in diagnosis of Graves' disease and estimating the prognosis. (J. Kor Soc Endocrinol 4:218~224, 1989)
