        Purpose S표 immobilization of the patients is essential for diagnostic CT angiography. Our objective was to develop and evaluate a non-invasive device for rigid immobilization and surface disease non-contact of the 표 in the upper and lower extremity during CT angiography. Materials and Methods Patients undergoing CT examination during 8 months January to September 2004. 655 patients were evaluated with CT angiography. Upper limbs(n=41), Lower limbs(n=614) patients. Image quality in the infrapopliteal(lower limb), brachial region(upper limb). The patients upper limb device consists of a polyethylene resin which need to be immobilize is wrapped with up around the elbow joint area based vecro tape. Lower limb device stabilizing elements made of polyethylene resin soft materials, and pelvis parts foam pad is used for noncontact surface. In a prospective study the upper and lower extremity device was used in patients who underwent a CT angiography of the upper and lower limb. Results Immobilization with our device was well tolerated by all patients. The rigid immobilization resulted in a complete absence of motion artifacts. The device is well tolerated easy and quick to apply, and is now routinely used in limb angiography. During the examination the patients were explicitly asked about pain or any other discomfort, which all denied. The improvement in image quality was especially obvious in the patients who had undergone a previous CT examination without adequate immobilization. Conclusion The device immobilzation allows for comfor표, effective extremity immobilization during CT angiography resulting in complete absence of motion artifacts.

        MDCT 검사에서 환자 체중을 이용한 선량 평가

        환자가 두부, 흉부, 복부 MDCT 검사에서 피폭되는 선량을 평가하였다. 선량의 지표는 CTDIvol, DLP, 유효선량을 평가하여 환자의 체중과 선량과의 상관관계를 분석하였다. CT 검사에서 검사 조건을 동일하게 하여 연속적으로 두부(520명), 흉부(531명), 복부(724명) 환자를 대상으로 하였다. CTDIvol과 DLP 평균선량은 두부검사에서 CTDIvol (48.6 mGy), DLP (1,604 $mGy{\cdot}cm$), 흉부검사에서 CTDIvol (6.9 mGy), DLP (250 $mGy{\cdot}cm$), 복부는 CTDIvol (10.5 mGy), DLP (575 $mGy{\cdot}cm$), 유효선량은 두부, 흉부, 복부에서 3.6, 4.2, 8.6 mSv로 분석되었다. 환자의 체중과 선량과의 상관분석에서 두부, 흉부, 복부에서, 두부는 상관관계가가 보이지 않았고, 흉부는 CTDIvol ($r^2$=0.62), DLP ($r^2$=0.694), 복부는 CTDIvol ($r^2$=0.691), DLP ($r^2$=0.741)와 양의 상관관계를 나타내냈다. 검사에 따른 체중과의 상관관계를 이해하고 검사 하면 방사선 피폭에 의한 선량을 평가하고 관리하는데 기여 할 수 있을 것이다. The purpose of this study provides measurements of radiation dose from MDCT of head, chest, abdomen and pelvic examinations. A series of dose quantities that are measured of patient weight to compare the dose received during MDCT examinations. Data collected included: weight together with CT dose descriptors, volume CT dose index (CTDIvol) and dose length product (DLP). The effective dose was also estimated and served as collective dose estimation data. Data from 1,774 adult patients attending for a CT examination of the head (n=520) or chest (n=531) or abdomen (n=724) was obtained from spiral CT units using a same CT protocol. Mean values of CTDIvol was a range of 48.6 mGy for head and 6.9, 10.5 mGy for chest, abdomen examinations, respectively. And mean values of DLP was range of 1,604 $mGy{\cdot}cm$ for head, 250 $mGy{\cdot}cm$ for chest, 575 $mGy{\cdot}cm$ for abdomen examinations, respectively. Mean effective dose values for head, chest, abdominal CT were 3.6, 4.2, and 8.6 mSv, respectively. The degree of CTDIvol and DLP was a positive correlation with weight. And there was a positive correlation for weight versus CTDIvol ($r^2$=0.62), DLP ($r^2$=0.694) in chest. And head was also positive correlation with weight versus CTDIvol ($r^2$=0.691), DLP ($r^2$=0.741). We conclude that CTDIvol and DLP is an important determinant of weight within the CT examinations. The results for this study suggest that CT protocol should be tailored according to patient weight.

        CT 검사에서 유리선량계를 이용한 수정체의 비스무트 차폐 효과

        CT 검사에서 수정체 차폐용 비스무트(bismuth)를 사용하여 수정체 선량의 감소효과를 유리선량계로 측정하여 비스무트 차폐효과 및 영상을 평가하였다. 안구 및 두부 CT 검사에서 차폐용 비스무트를 사용하여 수정체 선량의 감소와 차폐 효과를 평가하기 위해 인체모형 팬텀으로 유리선량계를 이용하여 수정체 선량을 평가하였다. 유리선량계를 이용하여 비스무트를 사용하기 전의 평균 선량의 21.54 mGy이었고, 사용 후의 선량은 10.46mGy로 51.3%의 선량 감소효과가 있다. 차폐용 비스무트를 사용한 안구 64 MDCT 촬영에서 선량감소 효과가 있어 수정체를 포함한 안구 CT 스캔에서는 비스무트를 사용하여 검사하도록 권고한다. The purpose of our study was to determine the eyeradiation dose when performing routine multi-detector computed tomography (MDCT). We also evaluated dose reduction and the effect on image quality of using a bismuth eye shield when performing head MDCT. Examinations were performed with a 64MDCT scanner. To compare the shielded/unshielded lens dose. the examination was performed with abd without bismuth shielding in anthropomorphic phantom. To determine the average lens radiation dose, we imaged an anthropomorphic phantom into which calibrated photoluminescence glass dosimeter(PLD) were placed to measure the dose to lens. The phantom was imaged using the same protocol. Radiation dosed to the lens with and without the lensshielding were measured and compared using the Student t test. In the qualitative evaluation of the MDCT scana, all were considered to be of diagnostic quality. We did not see any differences in quality between the shielded and unshielded brain. The mean radiation doses to the eyewith the shield were 21.54 versus 10.46 mGy, respectively. The lens shield enabled a 51.3% decrease in rediation dose to the lens. Bismuth in-plane shielding for routine eye and head MDCT decreased radiation dose to the lenswithout qualitative changes in image quality. The other radiosensitive superficial organs specifically must be protected with shielding.

        방사선사의 개인적 요인이 직무스트레스원에 영향을 주는 관련성 분석

        This study is aimed at analyzing the relationship between personal factors of radiological technologists and their job stressors. For this aim, a survey was conducted by means of 890 questionnaires from the middle of July to the end of August 2003 to the subjects of radiological technologists who are working for 44 general hospitals in 16 cities and provinces across the country. The results of the survey could be summed up as follows: 1. The biggest stressor that affects a radiological technologist personally under the working situation turned out to be position, job satisfaction and physical symptom(p<0.001), while job satisfaction and physical symptom also played an important role in personal relationship(p<0.001). 2. In terms of job conflicts, colleagues, immediate seniors, job satisfaction and physical symptoms appeared to exercise great impact(p<0.001), As for job autonomy, age, position and job satisfaction were known to be heavily influential(p<0.001). 3. With regard to job load, job satisfaction and behavioral symptom turned out to have great influence while, in job stability, position and job satisfaction seemed to be immensely influential (p<0.001). The present study has a limit in that it covers only radiological technologists who are working at the 3rd reference hospitals but excludes those who are working at the first and second reference hospitals. The findings, however, are surely believed to be able to serve as basic data to improve the medical service quality as they will help reduce the stressors of and enhance mental and physical health for radiological technologists who play important roles as teammates with expertise in the medical field. These outcomes could also be referred to in future studies in this area. 전국 16개 시도의 44개 3차 의료기관에 근무하는 방사선사의 직무환경에서 발생되는 스트레스원과 개인적인 관련요인을 분석한 결과는 다음과 같다. 근무환경에서 스트레스원에 영향을 주는 개인적인 관련요인은 직위, 직무만족도, 신체화 증상이고, 역할관련에서는 직무만족도와, 신체적 증상이며, 직무안정성에 있어서는 직위와 직무만족도가 많은 영향을 주는 요인으로 나타났다. 직무자율성에서 영향을 주는 요인은 연령, 직위, 직무만족도이고, 직무부담에서는 직무만족도, 행동적 증상이며, 직무갈등에서는 직장동료, 직속상관, 직무만족도, 신체적 증상이 영향을 많이 주고 있는 것으로 나타났다.
