Minimal Deviation Endometrioid Adenocarcinoma of the Uterine Cervix - A Case Report -

We describe here a case of minimal deviation endometrioid adenocarcinoma (MDEA) of the uterine cervix that was initially suspected according to the abnormal cytologic findings in a 39-year-old woman. The Papanicolaou (pap) smear showed many neoplastic glandular cells in monolayered sheets, rosettes, and clusters with palisading and feathering borders. The tumor cells had oval, hyperchromatic nuclei, with chromatin clumping and small nucleoli. Histologic examination disclosed endometrial-type glands with a bland, isolated, mainly rounded appearance and these glands were widely scattered deep into the cervical stroma with only scant stromal reaction. An association of MDEA with tubo-endometrioid metaplasia or cervical endometriosis has been suggested by identifying the tubo-endometrioid glands in the vicinity of the MDEA.

Primary small cell carcinoma of the ovary is a rare, aggressive tumor of young women and has two subtypes. The pulmonary type small cell carcinomas of the ovary that resembled small cell carcinoma of the lung and differed from both clinically and pathologically from small cell carcinoma of the ovary, usually associated with hypercalcemia. The histogenesis of the primary ovarian small cell carcinoma has not been established. We have identified two cases of small cell carcinomas primary in the ovary. One has microscopic features of pulmonary type, and the other has hypercalcemic type. Pulmonary type small cell carcinoma patient was a 40-year-old woman. She was admitted due to known ovarian mass. She was well before this episode. Total abdominal hysterectomy and bilateral salpingo-oophorectomy was performed. Gross examination revealed 15cm sized right ovarian mass which was mostly solid with multifocal necrosis. Histologically, the tumor was composed of small to medium sized round cells with scanty cytoplasm, hyperchromatic nuclei, inconspicuous nucleoli, numerous mitotic figures forming large sheets and diffuse pattern. Hypercalcemic type small cell carcinoma patient was a 43-year-old woman. She was also complained of right abdominal swelling and discomfort of 1 month duration. She was received total abdominal hysterectomy due to myoma in 3 years ago. The serum calcium levels was elevated. Right salpingo-oophorectomy and omentectomy was performed. Gross examination revealed 15cm sized huge ovarian mass which was mainly solid with small cystic components and hemorrhagic necrosis. The omentum showed several oval shaped metastatic nodules. Microscopic examination reveals closely packed tumor cells with scanty cytoplasm, vesicular nuclei that contains single nucleoli, and numerous mitotic figures. Many folliculoid spaces are present. Pulmonary type small cell carcinoma patient died 13 months after surgery. Hypercalcemic type small cell carcinoma patient was alive without evidence of disease 2 months after surgery but lost to follow up thereafter.
Diffuse Glomerular Basement Membrane Lamellation in Post-Transplant IgA Nephropathy

Diffuse glomerular basement membrane (GBM) lamellation, reminiscent of Alport's syndrome, has rarely, and exclusively, been reported in renal allografts from pediatric donors to adult recipients. We report on a similar lesion, identified in a 42-year-old male, who received a kidney from an unrelated 21-year-old living male donor. The disease of the recipient was unknown. Renal allograft biopsies were performed 3.5 and 4.8 years after the renal transplantation, due to massive proteinuria and serum creatinine elevation. The histological features of both biopsies were similar, but more advanced in the second biopsy. Glomerular mesangium was widened and had an IgA deposit in the first biopsy. In addition to the presence of mesangial electron dense deposits, the GBM showed diffuse lamellation and splintering on the subepithelial side, but no definite deposits. In the second biopsy, IgA deposits were extended to the peripheral capillary walls, but electron microscopic examination was not available. Two months after the second biopsy, the patient returned for hemodialysis.
노화초기 백서 신장에서의 Nitric Oxide Synthase 발현

Background : Nitric oxide synthase (NOS) has been suggested to have a role in renal injury of aging rats. Methods : Renal function and histology were compared between 12 monthand 7-9 week-old rats. Proliferating activity and cell death were evaluated by PCNA index and apoptosis. Three isoforms of NOS (eNOS, iNOS, and nNOS) were stained by immunohistochemistry. Results : Serum creatinine level was increased in old rats (1.0 mg/dL vs 0.5 mg/dL, p=0.000). 24 h proteinuria and urinary NO were comparable between the two groups. The percentage of global and segmental glomerulosclerosis increased in old rats. PCNA index decreased in the glomeruli (0.1 vs 0.6/glomerulus, p=0.005) and the tubulointerstitium (10.2 vs 19.2/mm2, p=0.019) of old rats compared to that of young rats. However, no difference was observed in the number of TUNEL positive cells. eNOS was not stained in young and old rat kidney, whereas iNOS was stained in the interstitial inflammatory cells of old rats (0.3 vs 0.0 of young rats/mm2, p=0.188). Macula densa nNOS staining significantly decreased in old rats compared to young rats (5.6 vs 9.5/mm2, p=0.009). Conclusions : Proliferating activity is more affected than cell death with aging. Decreased nNOS expression without alteration of eNOS and iNOS expressions may implicate nNOS as a marker of renal injury in the early stage of aging.
