        Is Alcohol Swabbing Sufficient to Clean Otoscopes in Hospitals?

        Background and Objectives An otoscope is a basic instrument used by otorhinolaryngologists. An inappropriately sterilized otoscope has been reported to be a possible bacterial vector for infection. In this regard, we decided to investigate contaminated otoscopes for possible bacterial contamination and evaluate the efficacy of the otoscope disinfection methods. Materials and Method We randomly drew 22 otoscope cones from university hospitals and 10 from private hospitals. Cones were divided into three groups accordingly to their sterilization methods: group 1 was wiped with 70% isopropyl alcohol, group 2 was soaked for 20 min in 70% isopropyl alcohol, and group 3 was soaked in CIDEX solution. The samples were cultured twice, first before the disinfection process and then after the disinfection process. Otoscopes were cleaned for a week by employing these techniques. Results Most of the pre-sterilized otoscopes (20/22) were obtained from the hospitals which demonstrated contamination with microorganisms. Staphylococcus was the most common bacteria found (16/22). After a week of cleansing, no bacteria were seen in group 1 (0%, 0/8), whereas group 2 (14.3%, 1/7), and group 3 (28.6%, 2/7) still showed remaining microorganisms. The three methods were significantly effective on sterilizing microorganisms. Conclusion An otoscope can be a vector for spreading infection. We found that disinfection by alcohol-swabbing alone is sufficient for sterilizing otoscope cones. Clinically, this information may be useful to otorhinolaryngologists. However, further studies are required to establish the most appropriate disinfection protocol to prevent infection from microorganisms. Korean J Otorhinolaryngol-Head Neck Surg 2016;59(8):578-82

        Isolated Oculomotor Nerve Palsy Caused by Onodi Cell Sinusitis

        Onodi cell sinusitis is a rare cause of isolated oculomotor nerve palsy. The sphenoid sinus andOnodi cell mucocele or invasive fungal sinusitis are often associated with multiple cranial-nerve symptoms simultaneously. However, isolated cranial nerve palsy due to simple sinusitisis rare. Sinus disease associated with cranial nerve palsy is difficult to diagnose based on pa-tient history or physical examination alone; CT and MRI help in correctly assessing these le-sions. Here we report a case of Onodi cell sinusitis observed in a patient who presented withisolated oculomotor nerve palsy. The patient underwent endoscopic sinus surgery and recov-ered completely at 2 months postoperative.

        Horizontal Dorsal Septal Incision for High Deviation Correction During Septoplasty

        When performing septoplasty in patients with a high deviation of the nasal septum, effectivecorrection is difficult and postoperative complications such as a saddle nose may result if thebone or cartilage is removed inordinately. Although several surgical techniques have been introduced,some are difficult to apply easily. Furthermore, the deviation may persist despite theapplication of surgical techniques due to the rebound memory of the remaining cartilage. Thisstudy aimed to describe a simple and safe surgical technique for crooked nasal septa with ahigh deviation. This method using horizontal dorsal septal incision allows easy separation ofthe highly deviated portion from the upper lateral cartilage. Furthermore, it is less traumaticthan other methods, and predictably preserves the keystone area.

        Hidrocystomas, eccrine or apocrine are rare ductal cystic benign lesions of the sweat gland. These uncommon cystic lesions are extremely rare in the external ear. Here, we discuss the clinical features and management of eccrine hydrocystoma in the external ear by describing the clinical, radiographic, pathologic features and management of an external auditory canal eccrine hidrocystoma in a 73-year-old Korean man. To the best of our knowledge, this tumor has not been reported previously in this site. Five month after surgery, there has been no evidence of local recurrence.         Validation of Korean-Language Version of Chronic Otitis Media Questionnaire to Reflect the Disease Activity of Chronic Otitis Media

        Background and Objectives The Chronic Otitis Media Questionnaire-12 (COMQ-12) wasdeveloped in the United Kingdom to assess disease specific Health-Related Quality of Life (HRQoL)of chronic otitis media (COM) patients. We assessed the validity of Korean version of COMQ-12(K-COMQ-12) and its correlation with the disease activity of COM classified according to thetype of surgery. Materials and Method We translated the original COMQ-12 into Korean and also performeda back-translation of the translated text into English. Enrolled were 106 COM patients and 106healthy subjects. Cronbach alpha was used to evaluate internal consistency, and factor analysiswas performed to prove reliability. We compared K-COMQ-12 scores between normal subjectsand COM patients to assess validity. The scores were also compared by dividing groups accordingto disease activity of COM. Results Cronbach’s alpha showed good internal consistency (0.939). There was a significantdifference in K-COMQ-12 scores between healthy subjects (1.99±4.34) and COM patients(22.69±12.41). Also, scores go higher as the disease activity of COM increased. A cut-off scoreof 6 of K-COMQ-12 sets 0.915 of sensitivity and 0.934 of specificity. Conclusion The K-COMQ-12 is a reliable and valid tool to assess HRQoL in patients withCOM. The K-COMQ-12 could be also used as an objective tool that reflects the disease activityof COM.
