Poe`s The Fall of the House of Usher demonstrates indeterminacy resulting in self-division. In the text, self-multiplication is realized by doubling the divided self of the narrator in the persons of Roderick Usher, and his twin sister, Madeline Usher. In Usher`s room, which is the indeterminate location and has an indeterminate form, the narrator paces, keeping time with the pacing of his double Roderick who suffers from schizophrenia. After the premature burial of Roderick`s double, Madeline, his mental disorder noticeably deepens, until he becomes a victim of the terror that exists only in his mind. In the final scene, as with the physical structure of the House of Usher, Roderick sinks into the abyss of the tarn. Death, selflessness, nothingness, and indeterminacy are inevitable result of the self-division. As the voice of a thousand waters become silent, the House of Usher (family, mansion, and story) as world of words is no more: the story is over, the text consumes itself into the tarn. Roderick`s, the narrator`s, and reader`s terror can be considered in the postmodern aesthetic context of Lyotard`s sublimity, which puts forth the unpresentable in presentation itself. In this fashion, fear and self-division become representative of the unpresentable and inscrutable nature of reality.

실험연구 : 허혈/재관류 시 Amiodarone이 허혈 전처치에 의한 심근보호에 미치는 영향

Background: Ischemic preconditioning (IPC) has protective effects on myocardial ischemia. These effects may be caused by ATP (adenosine triphosphate) sensitive potassium (KATP) channel activation. Amiodarone also has pharmacologic preconditioning effects, especially suppression of reperfusion arrhythmia. The mechanisms of amiodarone effects are different from that of IPC. The aim of this study using the isolated rat working heart was 1) to confirm whether IPC and amiodarone protected against reperfusion injury; and, if so, 2) to example whether combination of IPC and amiodarone demonstrated additive effect or not. Methods: Isolated rat hearts were stabilized for 30 minutes and were subdivided into four groups. Control group was subjected low-flow ischemia of 5% dextrose water for 30 minutes. IPC group was pretreated with IPC. In Amiodarone group, whole process was same as control group except amiodarone administration during low-flow ischemia. Amiodarone-IPC group, IPC was applied and also administrated amiodarone. Isovolumetric left ventricular end systolic pressure (LVESP), left ventricular end diastolic pressure (LVEDP), dP/dtMAX and heart rate were measured. Results: Group II showed more recovered LVESP, less developed ventricular stiffness, more preserved coronary effluent flow, earlier termination of arrhythmia and smaller infarct size than Group I. Group III showed preserved LVESP and dP/dtMAX, less developed ventricular stiffness, shortest arrhythmia sustaining time, smallest infarct size among all groups. These myocardial protective effects were abolished in Group IV. Conclusions: We observed that IPC and amiodarone administration, each has myocardial protective effects against myocardial ischemia, but when they are dealt at the same time, their beneficial effects diminished. From ancient times to the nineteenth century, most Western philosophers presupposed a chain of being, reaching down from God to angels to men to animals to vegetables to fossils. Leibniz suggested the three ideas ruling this paradigm were plenitude, continuity, and hierarchy(linear gradation). The paradigm suggested man`s duty was to keep his place, and not to leave it. and the beginning of wisdom for him was to remember and to hold fast to his limitations. The conception of the chain of being could be made to serve the purpose of maintaining the political status quo and the accepted religion. And the paradigm of the chain of being plays a big role in Kenneth Grahame`s The Wind in the Willows. published in 1908 near the end of what has been called the ``golden age`` of Children`s Literature. The Robinson affair of 1903 seems to have had a formative impact on Grahame`s view of class and its effect on culture. Grahame, a conservative by nature, warns the dangers of forgetting their own place, specifically the danger posed by the lower class the weasels and stoats in The Wind in the Willows. But that`s nut everything we get from the work. Self-described as a mid-Victorian, Grahame subtly conveys violence and hierarchy in his nostalgia for Arcadia lost or in his ecological vision.

임상연구 : 복강경하에 담낭절제술을 받는 환자에서 Propofol과 Remifentanil을 이용한 전 정맥마취와 Sevoflurane과 Alfentanil을 이용한 마취의 비교

Background: The aim of this prospective, double-blind randomized study was to compare the clinical properties of sevoflurane-alfentanil with propofol-remifentanil anesthesia in patients undergoing a laparoscopic cholecystectomy. Methods: Forty patients (ASA physical status 1, 2) scheduled for elective surgery received total intravenous anesthesia (TIVA group) with remifentanil and propofol or sevoflurane-alfentanil. The TIVA group was induced with propofol 5μg/ml and remifentanil 4.5 ng/ml. The anesthesia was maintained with a continuous infusions of propofol 2.5μg/ml and remifentanil 3 ng/ml. The sevoflurane-alfentanil group was induced with alfentanil 15μg/kg and propofol 1.5 mg/kg IV. Maintenance was obtained with 2.0 vol% sevoflurane and a bolus of alfentanil 10μg/kg IV where needed. Results: There were no significant differences in the systolic and diastolic blood pressure and heart rate between the two groups. There were no significant differences in the time to eye opening, the time to extubation, post-anesthetic recovery room stay time and the incidence of postoperative nausea and vomiting between the two groups. Conclusions: Propofol-remifentanil (TIVA) and sevoflurane-alfentanil both provided satisfactory anesthesia for a laparoscopic cholecystectomy. (Korean J Anesthesiol 2006; 51: 24~8)

아동의 정치학: 마크 트웨인의 『톰 소여의 모험』 과 『허클베리 핀의 모험』에 나타난 아동, 문화, 이데올로기

This paper investigates the childhood, dominant culture and dominant ideology in Mark Twain`s The Adventures of Tom Sawyer and Adventures of Huckleberry Finn. The two stories present the author`s pictures of the ‘real’ 19th century American childhood. Clearly, he focuses on childhood issues rather than race and freedom even in Adventures of Huckleberry Finn. However, the images of childhood described in the stories are not so positive. The master narratives show the atmospheres are surcharged with imminent violence and amorality. Tom Sawyer and Huck Finn come from broken families. They are branded as a ‘good’ child or a ‘bad’ child according to their behaviors. The white adults, including Aunt Polly, Miss Watson, Widow Douglas and Aunt Sally, believe that their duties are to educate and ‘civilize’ the likes of Tom and Huck. But the dominant adult ideology itself is tainted by violence, corruption and moral decay. Children are often victims of it. Twain criticizes the dominant adult culture and ideology by examining the politics of the childhood?the notion of ‘good’ and ‘bad’ child in the two stories.

서열의 정치학: 영미 아동문학에 나타난 아동, 권력, 이데올로기

This paper investigates images of the childhood, power, and ideology in some of the most successful children’s books of all time: Lewis Carroll’s Alice’s Adventures in Wonderland, Kenneth Grahame’s The Wind in the Willows, and A. A. Milne’s Winnie-the-Pooh and The House at Pooh Corner. We tend to assume that children’s literature creates worlds of naive and innocent childhood. However, the respective writers portray violent and hierarchical relationships between adults and children in and through the dominant culture. The adult(like) characters involved in the aforementioned stories politically exploit the ideology of hierarchy to justify their domination over the child(like) characters, and to satisfy their strong desire to manipulate children with their own authority. The moral of the stories is: a child’s duty is to keep to her or his own place, and not to leave it, and the beginning of wisdom for them is to recognize and to accept their limitations. The ideology of hierarchy in children’s literature reinforces inferior images of children and thus marginalizes them.

직장암환자에서 지속성 경막외차단 후 발생한 경막외 농양

Although the incidence of epidural abscess is low, patient requiring continuous epidural analgesia for control of acute and chronic pain is increasing rapidly. Therefore we anticipate more frequent encounters with epidural abscess patients in future. Once epidural abscess formation begins, early diagnosis and treatment is very important to prevent permanent neurologic damage. The authors encountered a case of epidural abscess after continuous epidural analgesia for control of perineal pain due to rectal cancer. Forty-eight hours after the block, patient began to suffer severe low back pain, local tenderness, and fever. So the catheter was removed and culture sensitivity test was done with blood and local drainage. The test results identified methicillin susceptible staphylococcus aureus. Antibiotics were administered. Ten days after the block, left ankle jerk disappeared, and force of dorsiflexion of great toe decreased, but numbness or anesthesia appeared at L5 dermatome. Laminectomy was performed, and abscess and necrotic fat material was removed from left L nerve root. The patient was discharged 12 days after operation without any neurologic sequalae.

Complex regional pain syndrome (CRPS) is quite difficult to comprehend and manage, of which etiology and pathophysiological mechanisms have not been fully understood. CRPS is classified as either type 1 (without any known nerve injury) or type 2 (with apparent nerve or tissue injury). Spinal cord stimulation is a restorative therapy that currently offers the best chances of obtaining long-term pain relief in CRPS patients with pain that has not responded to other treatments such as physical therapy or analgesic medications. A 45 years old male patient referred to our pain clinic due to uncontrolled pain at the middle finger for 4 years after crushing injury. He was diagnosed with CRPS type 2 and treated with medication, stellate ganglion block, thoracic sympathetic block, and pulsed radiofrequency therapy, but their effects were transient and not satisfactory. A percutaneous spinal cord stimulation (SCS) with a single quadripolar electrode was tried and the patient`s finger pain was improved significantly (from VAS 10 to 2). SCS is an effective treatment for CRPS type 2 which results from crushing injury when alternative therapies fail. (Korean J Anesthesiol 2007; 52: 733~6)
