치료위채득을 위한 Gothic arch tracer의 개발

It was clinically important to substitute the physiologic centric relation to the therapeutic position of the patients who needed the oral rehabilitation or occlusal treatment. There were several methods for recording the centric relation. One of the known methods was to use the Gothic arch tracer. However the existing Gothic arch tracer was difficult to adjust the three dimensional angulation of the recording plate and recording stylus depending on the hinge movement are of the individual. The purpose of this study was to develop new Gothic arch tracer which had adjustable stylus within hinge movement arc for the record of centric relation and to evaluate the clinical application of this device. The results were as follow; 1. A stylus of new developed Gothic arch tracer was so adjustable that the recording of mandibular positions could be reporducible within the hinge movement arc. 2. A record plate of new developed Gothic arch tracer was so adjustable to parallel with the occlusal plane that lateral recording of mandibular position was able to obtain stably. This study was shown that new developed Gothic arch tracer allowed the determination of the treatment position which can be used in the full mouth rehabilitation and occlusal treatments.

The biological and esthetic concept must be applied not only for anterior restoration but also for all the prostheses. It a patient has occlusion on the prosthodontic restoration that is not harmonized with the stomatognathic system the occlusal interference on the restoration can cause temporomandibular disorder including continuing pain in the stomatognatic region and the limitation of mouth opening. As a result, patients will have poor esthetic profile and psychological disturbances. In conclusion, even though a simple artificial crown is placed in oral cavity, the esthetics must be considered together with by occlusion itsely, periodontium, TMJ and muscle system.

NF-κB is a eucaryotic transcription factor that exists in virtually all cell types. More than a decade after its discovery, remains an exciting and active area of study. A primary level of control for NF-κB is through interactions with an inhibitor protein called IκB. recent evidence confirms the existence of multiple forms of IκB that appear to regulate NF-κB by distinct mechanisms. NF-κB can be activated by exposure of cells to LPS or inflammatory cytokines such as TNF or IL-1, viral infection or expression of certain viral gene products, UV irradiation, B or T cell activation, and by other physiological and nonphysiological stimuli. Activation of NF-κB to move into the nucleus is controlled by the targeted phosphorylation and subsequent degradation of IκB. Exciting new research has elaborated several important and unexpected findings that explain mechanisms involved in the activation of NF-κB. In the nucleus, NF-κB dimers bind to target DNA elements and activate transcription of genes encoding proteins involved with immune or inflammation responses and with cell growth control has been used as one of the mouth preparations for restorative treatment. The objectives of occlusal splint are as follows; 1. To use as preliminary application for periodontal-occlusal treatment 2. To provide proper vertical dimension 3. To control abnormal habits and parafunction 4. To treat the temporomandibular disease and myofascial pain, etc. In some cases, the patients had improper vertical dimension and occlusal interferences caused by prostheses reconstucted using centric relation occlusion recorded without considering the health of TMJ and manticatory muscle. And these prosthesis act as primary source that cause pathologic phenomenon in periodontal ligament, muscles and TMJ. Physiologically, in order to make the treatment occlusion guided by proper centric relation method, the method should be guided after the use of occlusal splint for some period. The main objective of prosthetic treatment is to maintain the function and health of stomatognathic system. So, one of the most important things that have to be performed by clinicians is a clinical ability to do the correct diagnosis and treatment planning based on the stomatognatic function and occlusion.
