Purpose: To investigate the possibility of expanded polytetrafluoroethylene (e-PTFE) as a wrapping material for porous polyethylene orbital implant. Methods: Two groups of 6 female New Zealand albino rabbits underwent enucleation with placement of a 12 mm porous polyethylene orbital implant wrapped in simple e-PTFE in one group (group 1) and macro-porous e-PTFE in the other group (group 2). Histopathologic sections of the implants obtained at 4, 6, 8 weeks were compared on the extent of fibrovascular ingrowth, percentage of the area of proliferated vessel, and grade of fibrosis between the two groups. Results: There was no implant exposure or severe inflammation in group 1 and 2 during the observation. The extent of fibrovascular ingrowth differs only at 4 weeks. Group 1 showed less fibrovascular ingrowth (until outer 1/3) at anterior half compared with posterior one, but there was no difference between anterior and posterior half in group 2 (until middle 1/3). The percentage of the area of the proliferated vessels was higher in group 2 than group 1 at both anterior and posterior half, and the anterior/posterior ratio of the area of vessels was also higher in group 2 than group 1. The grade of fibrosis was higher in group 2 than group 1 at 4, 6, and 8 weeks. Conclusions: Macro-porous e-PTFE almost didn`t disturb fibrovascular ingrowth into the orbital implant, so macro-porous e-PTFE can be used safely as a wrapping material for porous polyethylene orbital implant.
아래눈꺼풀뒤당김 교정수술시 이식재료로서 폴리프로필렌 부직포의 이용에 관한 연구

Purpose: To evaluate the usefulness of a polypropylene non-woven sheet as a spacer graft for the correction of lower lid retraction Methods: Fifteen New Zealand white rabbits were randomized into three groups. Polypropylene non-woven sheets were implanted between the tarsus and lower lid retractor via a transcutaneous approach (Group 1), a transconjunctival approach (Group 2), or a transconjunctival approach with amniotic membrane transplantation (Group 3). Numerous large pores were made in the sheet with a 21-gauge needle before implantation. Rabbits were sacrificed serially after 1, 2, 4, 6, and 8 weeks in each group. Gross and histologic examinations were performed. Results: In Group 1, the polypropylene sheets stayed in place well, without any exposure on gross observation except at 2 weeks. In Group 2, the conjunctival wound was not healed on the polypropylene sheet, and the sheet was exposed to the ocular surface and extruded gradually. In Group 3, the conjunctival wound was well healed on the surface of polypropylene sheet, and the sheet remained stationary at 4, 6, and 8 weeks on gross examination. Histologic examinations showed gradual fibrovascular ingrowth into the polypropylene sheets in Groups 1 and 3, but not in Group 2, in which only severe inflammatory infiltrations were observed. Conclusions: The polypropylene non-woven sheet is a good candidate for a spacer material in lower lid retraction operation, and either the transcutaneous approach or the use of an amniotic membrane is necessary for best postoperative outcome.

Purpose: We report our experience with a case of a nodular lesion of the lower palpebral conjunctiva with histologic features consistent with mucoepidermoid carcinoma. Methods: A 72 year-old man presented with a left lower palpebral conjunctiva lesion. 3X3 mm sized flat pedunculated yellowish red mass was observed. Incisional biopsy proved the mass a mucoepidermoid carcinoma and wide local resection was conducted. Results: Hematoxylin and eosin-stained sections showed lobules of tumor cells, consisting of predominantly epidermoid cells and scattered islands of mucin-secreting cells. Mucicarmine-stained sections demonstrated strong positivity, consistent with the diagnosis of mucoepidermoid carcinoma. Conclusions: In any case presenting palpebral conjunctival lesion, mucoepidermoid carcinoma should be differentiated, and it is necessary to confirm the diagnosis through biopsy.

Purpose: To report the results obtained after fixation of the eyeball to the periosteum over the posterior lacrimal crest in 5 cases of inveterate exotropia. Methods: From September 2011 to January 2013, 5 patients with inveterate exotropia and a history of surgery for exotropia underwent fixation of the eyeball to the periosteum over the posterior lacrimal crest. Results: The mean preoperative exotropia of 35 ± 10.61 PD changed to esotropia of 5.8 ± 17.28 PD at the one week postoperative visit and exotropia of 13.2 ± 11.34 PD at the final postoperative visit (mean, 6.05 months after surgery). At the final postoperative visit, 2 patients who had ocular adhesion due to trauma showed 25 PD exotropia. None of the patients showed injuries to the lacrimal system, but 2 patients showed conjunctival granuloma. Conclusions: Fixation of the eyeball to the periosteum over the posterior lacrimal crest is an effective approach for the management of inveterate exotropia, except in cases of exotropia combined with ocular adhesion.
