고령의 대퇴 전자간 골절에서 인공관절 치환술시 전자 재부착 기구(AO 전자 재부착기구)를 이용한 대전자 고정술

목적: 65세 이상의 환자의 불안정성 대퇴골 전자간부 골절의 치료로 인공 고관절 반치환술 시행 시대전자부 골절에 대하여 전자 재부착 기구를 이용한 내고정술의 효과를 알아보고자 하였다. 대상 및 방법: 2010년 10월부터 2011년 4월까지 65세 이상 고령환자의 대퇴골 전자간부 골절 중 Evans 분류상 2형 이상(AO A1.3 이상)에서 인공 고관절 반치환술 시 전자 재부착 기구를 이용한 전자 고정술을 시행한 19예(남/여: 1/18)에 대해 최소 12개월(12-29개월) 이상 추시 관찰하여 분석하였다. 결과: 전자 재부착 기구를 이용한 고정술을 시행한 19예 중 1예에서 기구의 이완이 발생하여 재수술을 시행하였으며, 19예 모두에서 탈구는 발생하지 않았다. 19예에서 12개월 이상 추시관찰 시행하였으며, 최종 추시 상 골절부의 완전 유합을 확인하였다. 결론: 고령 환자의 불안정성 대퇴골 전자간부 골절의 치료로 인공 고관절 반치환술 시행 시 대전자 고정 방법 중 전자 재부착 기구를 이용한 전자 고정술은 쉽게 확고한 고정력을 얻을 수 있으며, 골절된 대전자를 유합시킴으로 외전력을 복원하게 되어 인공관절의 탈구 및 지속적인 통증 등의 합병증을 방지할 수 있는 좋은 수술방법으로 사용될 수 있다. Purpose: The purpose of this study is to evaluate the efficacy of the trochanter reattachment device (TRD) as a firm internal fixation method for bipolar hemiarthroplasty in unstable intertrochanteric femur fracture for elderly patients over 65 years old. Materials and Methods: From September 2010 to April 2011, 19 patients (M/F: 1/18) over 65 years old were treated with bipolar hemiarthroplasty using the TRD as a fixation method for intertrochanteric femur fracture with above Evans-Jensen classification 2nd (above AO/OTA A1.3). They were followed up for more than 12 months(12-29 months). Results: Out of 19 patients, only one had loosening of the TRD plate and reoperation was performed. There was no dislocation after surgery. Complete fracture union was observed in 19 patients with follow up of more than 12 months. Conclusion: In bipolar hemiarthroplasty for intertrochanteric femur fracture, TRD produced easy and firm fixation. Additional fixation with TRD restoring abduction force by union of greater trochanter can be a good choice of surgery for avoidance of dislocation and chronic pain due to trochanteric nonunion after arthroplasty.
