임질의 임상적 고찰(제7보) : Cefotaxime과 Kanamycin의 Penicillinase-Producing Neisseria Gonorrhoeae 요도염 치료효과

Recent rapid increase of penicillin-resistant beta-lactamase producing Neisseria gonorrhoeae in Korea makes it urgent to find suitable treatment regimens, especilly because spectionmycin-resistant PPNG has been found in Seoul. Newer generation of cephalosporins are found to be effective in the treatment of gonorrhoea, especially against PPNG urethritis. In this study, the effect of cefotaxime was compared with kanamycin in the treatment of uncomplicated male PPNG urethritis. 85 male patients with uncomplicated PPNG urethritis, which was detected by chromogenic cephalosporin method, at the Veneral Disease Clinic of Chook-Ku Public Health Center in Seoul were subjected to this study. 45 patients were treated with 1.0 gm cefotaxime i.m. plus 1.0 gm probenecid p.o., and 34 patients were followed. There were no treatment failurea and 17(50.0%) postgonococcal urethritis (PGU). Other 40 patients were treated with 2.0 gm kanamycin i.m., and 36 patients were followed. There were 10(27.8%) treatment failures and 11(42.3%) PGU. Cefotaxime gave significantly (p<0.001**) superior result to kanamycin in the treatment of PPNG urethritis.
피부단순포진 진단을 위한 Tzanck 도말, 바이러스배양 및 면역형광검사의 비교 연구

Although Herpes Simplex Virus (HSV) infection is one of the most common infectious diseases, its two main laboratory diagnostic methods-Tzanck smear is a simple, inexpensive and rapid method, but it shows relatively low positivity. Similarly, the viralculture is the most accurate and definitive method, but it is expensive and relatively time-consuming test. There is a demand for more practical laboratory diagnostic methods for cutaneous herpes simplex infections. The purpose of this study is to examine the effectiveness of the indirect immunofluorescence (IIF) staining with monoclonal antibodies, which is suggested as a practical method. For the study, 103 patients suspected HSV infection were sampled at the Department of Dermatology in Hanyang University Hospital from March 1987 to July 1990. In the 60 specimens for Tzanck smear and viral culture, Tzanck smear was 35(58.3%) positivities, and viral culture was 56(93.3%) positivities(p〈0.05). In the 43 specimens for viral culture and IIF staining, viral culture was 40(93%) positivities and IIF staining was 37(86%) positivities(p〉0.05). In the 25 specimens, which viral culture was positivities, IIF staining showed 22(88%) positivities and Tzanck smear was 16(64%) positivities(p〈0.05). In summary, the result of Tzanck smear showed relatively high correlation to the results of IIF staining and viral culture. However, the result of IIF staining showed a significant agreement with the result of viral culture. It is suggested that the IIF staining with monoclonal antibodies is a simple, inexpensive, but accurate and relatively rapid method for the diagnosis of cutaneous HSV infections.
