        Dysplasia Epiphysealis Hemimelica - 술후 재발 1예 보고 -

        Dysplasia epiphysealis hemimelica, also known as Trevor's disease or tarsoepiphyseal aclasis, is characterized by asymmtric cartilaginous overgrowth (histologically identical to that of an osteochondroma) in one or more epiphysis or a tarsal or carpal bone. A painless, bony swelling or deformity about a joint, usually in the lower extremities and especially adjacent to the ankle or knee, becomes evident, typically when the patient is 2 to 4 years of age. Characteristic radiographic features include the early appearance and excessive growth of the involved epiphysis, with a resultant irregularly calcified mass projecting from the epiphsis or the tarsal or carpal bone. This disorder is generally accepted as nongenetic in origin. Although the prognosis of this disease generally is regarded as good with benefit derived form local excision of the lesion or, rarely, arthrodesis, persistent deformities and secondary osteoarthritis are evident in some cases. A case of dysplasia epiphysealis hemimelica recurred two years after local excision in 3 year old female is reported with a brief review of literature.

      • 근 이동술 및 피부 이식을 이용한 하퇴부 연부조직 결손의 치료

        개방성 골절로 인한 연부 조직 결손 및 그 후유증으로 생긴 속발성 골수염 환자들에서 근 이동술 및 피부 이식을 동시에 시행하여 그 치유 기간의 단축 및 만족할 만한 결과를 얻었기에 문헌 고찰과 함께 보고하는 바이다. The incidence of open comminuted fractures are increasing because of frequent high energy injuries. The tibia is Particularly, vulnerable to open fracture and results in secondary osteomyelitis because of poor soft tissues. The management of soft tissue defect with bone exposure is extremely difficult. Recently, the muscle transposition technique has gained an accepted place in treating wide soft tissue defect with bone exposure because this technique is generally simple, safe and reliable. Eighteen patients who had open fractures or osteomyelitis of tibia wore treated with gastrocnemius and soleus muscle transposition at department of orthopaedic surgery. Seoul Paik Hospital from March 1987 to February 1990. The results were as follows : 1.Male predominate in 83.3%. 2.The main cause of lesion were open fracture in 88.9%. 3.The commonest muscle used for muscle transposition is medial head of gastrocnemius muscle, ie 83.3% of cases. 4.Mean duration from injury to operation is 30 months. 5.Skin graft was done immediately in 88.9%. 6.All cases were healed within 2 to 3 weeks after muscle transposition. We think this is the good procedure for the treatment of open fracture of the low leg.

      • 골반환 골절의 임상적 고찰

        294례의 골반환 골절환자에서 각종 임상소견을 분석하고 전산화 단층촬영의 효능을 관찰하여 문헌고찰과 함께 보고하는 바이다. The growing interet about the fractures of the pelvic ring is a result of the increased mechanization and increased utilizaion of motor vehicles, and the growing impression is that the incidence of these fractures is preceded only by the incidence of the skull fracture in terms of motility and complications. The authors reviewed 294cases of pelvic ring fractures treated at Inje University, Seoul Paik Hospital during the period from Jan., 1985 to Dec., 1989. The injury was classified according to the data analyzed clinically and the role of the computed tomographic scanning in evaluation of the major pelvic ring fractures was analyzed also. The results were as follows : 1.In the age and sex distributions, those fractures were most frequent in the thirties and (66.7%), forties more slightly frequent in the males. 2.The most common cause of the injury was traffic accident(88.7%) 3.Of 294cases, 170 cases had stable fractures, and 124 cases had unstable fractures of the pelvic ring. 4.The CT scan was useful in the evaluation of pelvic ring fractures, especially of the posterior losions and in the classification and planning of the treatment also. 5.The associated injuries were found in 252cases(85.7%) out of 294, among which other musculoskeletal injuries were most cmmon. 6.Among 294 cases, 8 cases (2.7%) died and the causes of deaths were hypovolemic shocks(6cases) and head injuries(2cases). And an important aspect is that the first step in managing the patients bleeding from the pelvic ring fractures is the replacement of bloodloss.

        한국산 놀래기과 어류의 분류학적 검토

        The perciform suborder Labroidei comprising six families (Cichlidae, Embiotocidae, Pomacentridae, Labridae, Odacidae, and Scaridae) are characterized by having the specialized pharyngeal jaws for food processing, i.e., united fifth ceratobranchials and upper pharyngeal jaw articulating with the basicranium via diarthroses (Stiassny and Jensen, 1987). They usually inhabit in the most tropical and subtropical seas and comprise about 235 genera and roughly 2,274 species worldwide (Nelson, 2006). Concerning the Korean labroid fishes, Mori (1952) had listed 18 genera and 26 species belong to four families in his check list of Korean fishes since Jordan and Metz (1913) firstly reported six genera and seven species in only two families (Embiotocidae and Labridae). Chyung (1977) added two species, Tilapia mossambica and Cirrhilabrus temmincki, to Mori’s list and also classified them into three suborders, i.e., Embiotocina (containing only Embiotocidae), Pomacentrina (Cichlidae and Pomacentridae), and Labrina (Labridae and Scaridae). Subsequently, Lee and Kim (1996) reviewed the Korean labroidfishes taxonomically resulting in 22 genera and 32 species in five families with some taxonomical modifications including a new Korean record. It is remarkable to be added many new Korean recordsto the pomacentrids or the labrids for recent 10 years (Koh et al., 1995; Yoo et al., 1995; Koh et al., 1997; Myoung, 1997; Choi and Kim, 2000; Choi et al., 2002; Kim and Go, 2003). Recently, Kim et al. (2005) briefly described all members of the Korean Labroidei with a color photograph or a figure, recognizing 27 genera and 42 species in five families. In the present study, the current taxonomical status of the Korean labroid fishes including distributional features is summarized based both on specimens collected from the Korean waters and on literature survey to provide bio-information of the Korean native fish species. As a result, the Korean labroid fishes totally consist of 27 genera and 44 species in five families, that is, Cichlidae (1 species), Embiotocidae (3), Pomacentridae (15), Labridae (22), and Scaridae (2). They distributed mainly in the coastal waters of the South Sea, Korea, however, most pomacentrids or labrids occur in the coastal waters of Jeju Island only, although some species were observed in their larval or juvenile stages only from coastal waters of the island. Interestingly, several species are expanding their distribution north to Ulreung and Dok islands in the East Sea, Korea lately.

      • 상위 요추부에 발생한 추간판 탈출증-2례 보고-

        요추 추간판 탈출증은 하요추에 발생하는 예가 대부분이고 상위 요추에 발생하는 경우는 극히 드물다. 또한, 임상 증상의 모호함으로 인해 진단적 어려움과 척수가 끝나지 않은 부위로 치료에 있어서도 하요추와는 전혀 다른 의미를 지니고 있다. 저자들은 제1-2 요추간에 발생한 추간판 탈출증 2례를 경험하였고 이를 전방 감압술 및 전방 유합술로 치료하여 좋은 결과를 얻었기에 문헌 고찰과 함께 보고하는 바이다. Lumbar disc herniation is commonly caused by the degenerative disc and traumatic incidents. It is most prominent in mobile segments, such as L4-5, L5-S1, L3-4, in descending order of frequency. Disc herniation in high lumbar spine is very rare, especially at L1-2. Herniation at this is of particular importance because of the difficulty in early diagnosis due to variability of clinical presentation, also difficulty in treatment. It is usually decompressed anteriorly since posterior decompression such as laminectomy is not feasible. The authors experienced two cases of herniated intervertebral disc occuring at L1-2, which needed operative treatment. Favourable results were achieved by anterior decompression and anterior interbody fusion.

        Modification of the kinetics of base-model (hemicellulose directly to xylose) was made to account for presence of xylo-oligomer. Since soluble xylo-oligomer is formed from xylan and is further hydrolyzed to xylose, xylo-oligomer was added to the base model as one of the ecognizable components. Material balances on xylo-oligomer and xylose for a percolation reactor were set up and simplified as partial differential equations.The partial differential mequations were again solved by the Laplace transform method. The solution provides the information for the time-and-position dependent concentrations for the respective components. Upon integration of the concentration over a specified time period, one obtains the yield information for xylose and xylo-oligomer. A kinetic pattern including xylo-oligomer was incorporated into the reported simulation program(10,11). A sensitivity analysis was made to verify the effect of the oligomer kinetic parameter on xylose yield.
