노인의 기능성 변비 유병률 및 위험인자 -서울 시내 3개 복지관 이용 노인을 대상으로-

연구배경: 위장관 증상은 우리나라 사람들이 흔히 호소하는 증상이며, 변비는 대표적인 위장관 증상이라고 할 수 있다. 그러나 우리나라에서, 특히 노인을 대상으로 한 변비의 유병률 및 위험인자에 대한 연구는 알려져 있지 않다. 이에 저자들은 복지관 이용 노인을 대상으로 기능성 변비의 유병률과 변비 관련 위험인자에 대하여 연구를 시행하였다. 방법: 2003년 12월 서울지역 3개의 노인 복지관을 방문한 65세 이상 노인 854명 중 852명(암 환자 1명, 치매환자 1명 제외)을 대상으로 간호사가 직접 설문조사 하였다. 기능성 변비 유병률은 1999년에 발표된 로마기준 II (Rome II criteria)를 사용하였으며 변비와 관련이 있다고 알려져 있는 여러 요인들을 조사하였다. 결과: 변비의 유병률은 26.6%로 남녀별 차이는 없었다. 자신이 변비가 있다고 생각하는 사람은 30.3%였으며, 이 중 기능성 변비에 해당되는 경우는 72.9%였다. 단변량 분석에서 변비 관련 인자 중 운동량, 항문질환, 수분 섭취량, 연령, 그리고 야채 섭취, 과일 섭취가 변비와 관련이 있는 것으로 나타났다. 로짓 회귀분석 결과에서도 운동, 항문질환, 수분 섭취량, 연령, 야채 섭취, 과일 섭취가 독립적으로 기능성 변비의 중요한 위험인자였다. 결론: 기능성 변비는 노인들에서 매우 흔한 문제이며, 변비 관련 인자로 연령, 운동량, 항문 질환, 수분 섭취량, 섬유질 섭취가 중요하다. 제주의 안도 다다오 건축에 나타난 지역성 구현에 관한 연구

본 연구는 제주건축의 지역성 논의의 연장선 상에서 제주의 이타미 준 건축에 나타난 지역성의 개념들이 제주지역에 있는 다른 건축가의 작품에도 어떻게 발현되는지를 추적하여, 제주건축의 지역성에 대한 보편적 개념틀을 구축하고자 한다. 이를 위해 본 연구는 일본 건축가 안도 다다오가 제주에 남긴 건축을 통해 그가 제주의 지역성을 어떻게 건축에 담고 있는지를 살펴보고자 한다. 이를 위해 안도 다다오가 제주에 남긴 세 개의 건축물을 분석하였다. 또한 2009년부터 현재까지 5회 이상의 현장답사와 현장분석을 진행하였고 이를 통해 이타미 준이 구현한 지역성의 의미를 분석하였다. 안도 다다오의 지역성에 대한 사유와 건축적 표현이 세계적으로 인정받고 있다는 점에서 그가 남긴 제주건축에서 지역성에 대한 유의미한 분석과 결과를 찾을 수 있었다. 그 결과, 이타미 준과 마찬가지로 그가 구현한 지역성은 의미와 현상이 아닌 중합적 지역성과 가치적 지역성으로 나타났다. 중합적 지역성은 건축의 의미와 현상이 복합적으로 구현된 지역성을 뜻하고, 가치적 지역성은 제주의 고유 풍경처럼 그 지역만 가지고 있는 고유 가치에 대한 존중과 배려를 통해 구현된 지역성이었다. This study seeks to construct the universal concept of locality in architectures of Jeju, by tracking and analysing how the locality of Itami Jun’s architectures in Jeju manifested itself in works of other architects. For this, this research reveals how Ando Tadao put the concept of locality in his architectures by analysing three works in Jeju, which designed by Ando Tadao. The meanings in the locality constructed by Itami Jun was also analysed over five times of site survey and building site analysis in related concepts. The analysis in the Ando Tadao architectures of Jeju is significant in fact that his concept and architectural visualization is widely credited. In result, along as Itami Jun, the locality he realized turned up as the form of superimposed and valued locality, not just in forms of meanings and phenomenons. Superimposed locality stands for architectural meanings and the locality-implemented phenomenon, and valued locality means unique value which has implemented in the respect and consideration in the critical aspects of special areas such as the original landscape of Jeju.

Purpose: We previously developed learning models for predicting the need for intensive care and oxygen among patients withcoronavirus disease (COVID-19). Here, we aimed to prospectively validate the accuracy of these models. Materials and Methods: Probabilities of the need for intensive care [intensive care unit (ICU) score] and oxygen (oxygen score)were calculated from information provided by hospitalized COVID-19 patients (n=44) via a web-based application. The performanceof baseline scores to predict 30-day outcomes was assessed. Results: Among 44 patients, 5 and 15 patients needed intensive care and oxygen, respectively. The area under the curve of ICUscore and oxygen score to predict 30-day outcomes were 0.774 [95% confidence interval (CI): 0.614–0.934] and 0.728 (95% CI:0.559–0.898), respectively. The ICU scores of patients needing intensive care increased daily by 0.71 points (95% CI: 0.20–1.22) afterhospitalization and by 0.85 points (95% CI: 0.36–1.35) after symptom onset, which were significantly different from those in individualsnot needing intensive care (p=0.002 and <0.001, respectively). Trends in daily oxygen scores overall were not markedlydifferent; however, when the scores were evaluated within <7 days after symptom onset, the patients needing oxygen showed ahigher daily increase in oxygen scores [1.81 (95% CI: 0.48–3.14) vs. -0.28 (95% CI: 1.00–0.43), p=0.007]. Conclusion: Our machine learning models showed good performance for predicting the outcomes of COVID-19 patients andcould thus be useful for patient triage and monitoring.
