        Proposal for a Standardized Pathology Report of Gastroenteropancreatic Neuroendocrine Tumors: Prognostic Significance of Pathological Parameters

        Background: There is confusion in the diagnosis and biological behaviors of gastroenteropancreatic neuroendocrine tumors (GEP-NETs), because of independently proposed nomenclatures and classifications. A standardized form of pathology report is required for the proper management of patients. Methods: We discussed the proper pathological evaluation of GEP-NET at the consensus conference of the subcommittee meeting for the Gastrointestinal Pathology Study Group of the Korean Society of Pathologists. We then verified the prognostic significance of pathological parameters from our previous nationwide collection of pathological data from 28 hospitals in Korea to determine the essential data set for a pathology report. Results: Histological classification, grading (mitosis and/or Ki-67 labeling index), T staging (extent, size), lymph node metastasis, and lymphovascular and perineural invasion were significant prognostic factors and essential for the pathology report of GEP-NET, while immunostaining such as synaptophysin and chromogranin may be optional. Furthermore, the staging system, either that of the 2010 American Joint Cancer Committee (AJCC) or the European Neuroendocrine Tumor Society (ENETS), should be specified, especially for pancreatic neuroendocrine neoplasms. Conclusions: A standardized pathology report is crucial for the proper management and prediction of prognosis of patients with GEP-NET.

        진행성 위암 전절제후 5-Fluorouracil로 치료한 환자에서종양내 Thymidylate Synthase 과발현의 예후적 의의

        Background : The expressions of thymidylate synthase (TS), E2F-1, pRb, and p53 are correlated with DNA synthesis. The significance of their expressions is still controversial for predicting the outcome of 5-fluorouracil (5-FU) therapy in the patients with advanced gastric carcinoma. Furthermore, their prognostic value in the metastatic lesions of gastric carcinoma has not yet been confirmed. Methods : To ascertain their prognostic value, we immunohistochemically analyzed the expressions of TS, E2F-1, pRb, and p53 in the primary tumors and the related metastatic lymph nodes, and we then compared the survival between the high and low expression group of each protein. Ninety four patients with advanced gastric carcinoma who were treated by complete resection and adjuvant 5-FU chemotherapy were analyzed. Results : The TS expression in primary tumors was significantly correlated with that of E2F-1. The expression of these genes showed no significant difference between the primary tumors and the metastatic lymph nodes except for E2F-1, which was significantly higher in the lymph node metastasis than in the primary tumors. After complete resection and 5-FU-based adjuvant chemotherapy, patients with a high TS expression in the primary tumors showed a longer survival than those patients having primary tumors with a low TS expression (p=0.0392). Conclusion : A high TS expression in the primary tumors may be related to a better outcome for advanced gastric cancer patients who were treated with 5-FU chemotherapy after curative resection.

        EBV Encoded RNA 양성 위암에서 EBV Latent Membrane Protein 1 유전자 변화와 TGF-β1, TGF-βRII, p21,p16, E2F1, Thymidylate Synthase 및 NF-κB 단백 발현에대한 면역조직화학적 연구

        Background : Although clinicopathologic differences have been described between Epstein- Barr virus (EBV)-positive and negative gastric adenocarcinomas, the pathogenetic basis for these differences remains unclear. In this study, efforts were made to confirm that expression of EBV-latent membrane protein (LMP1) and immunohistochemical characteristics of EBVpositive gastric adenocarcinomas. Methods : We investigated genomic deletion, and RNA & protein expression of the EBV-LMP1, as well as immunohistochemical protein expression of transforming growth factor (TGF)-β1, TGF-βRII, p21, p16, E2F1, thymidylate synthase, and NF-κB in relation to EBV positive gastric adenocarcinoma. Results : A total of 38 Epstein-Barr Virus Encoded RNA-positive and 80 negative gastric carcinomas were examined. A 30 bp DNA deletion in the EBV-LMP1 gene, initiating at codon 342, was detected in 94.4% of EBVpositive cases. By RT-PCR and western blotting, EBV-LMP1 mRNA and protein expressions were absent in all cases, re- gardless of DNA deletion. No significant differences in TGF-β1, TGF-βRII, p21, NF-κB, E2F1, or thymidylate synthase expression were identified. However, the decreased expression of p16 was found in 84.2% of EBV-positive carcinomas, relative to only 57.5% of EBV-negative tumors (p=0.024). Conclusion : EBV-LMP1 DNA deletion, mRNA and protein losses are highly prevalent in EBV-positive gastric adenocarcinoma among Korean patients, along with decreased p16 expression.

        First Report of a Gangliocytic Paraganglioma Arising in a Tailgut Cyst

        Here we present the first report of a gangliocytic paraganglioma arising in a tailgut cyst; it occurred in a 56-year-old man. Tailgut cysts are uncommon congenital hamartomatous lesions that arise in the retrorectal presacral space in infants or adults. Benign or malignant tumors associated with tailgut cysts are rarely described; the most common tumors are adenocarcinomas and carcinoid tumors. A gangliocytic paraganglioma is a rare benign tumor that occurs nearly exclusively in the second portion of the duodenum. Rare cases have been reported at other locations, but a tailgut cyst has never been described. In our case, a resected 3.9 × 3.3 × 3 cm mass was composed predominantly of a solid yellow white neuroendocrine tumor within the area of a tailgut cyst. The neuroendocrine component of this tumor was different from previously described carcinoid tumors with respect to the histologic findings of neural differentiation as well as the intermixed typical gangliocytic features highlighted by immunohistochemical stains for S-100 protein and neurofilament. Although an intermixed area of the tailgut cyst and gangliocytic paraganglioma were found in some areas, the pathogenesis of this tumor remains to be elucidated.

      • 한국 설화에 내재된 인간과 동물의 생태적 관계 고찰

        예부터 내려오는 속담이나 금기표현이 오늘날까지 그 교훈을 이어오고 있듯 이 설화 역시 그 속에 내재된 교훈적 가치는 여전히 유효하다. 다만 예전과 달 리 사회․문화적 환경이 달라져 내용의 변이가 불가피한 경우도 있지만 자연에 대한 의식과 생태적 삶의 모습은 현대인들에게 충분히 많은 가르침과 깨달음을 주고 있다. 설화는 구비문학으로 옛사람들의 생활모습, 가치관, 사유체계 등 다양한 유형의 내용을 담고 있다. 본고는 이러한 설화에 담긴 인간과 동물의 생태적 관계를 통해 설화에 내재된 옛사람들의 생태관을 엿볼 수 있었다. 한국 설화에 담긴 인간과 동물의 관계는 자연이 훼손되기 훨씬 이전부터 존재해왔고, 인간중 심주의에 대한 반성으로 나온 새로운 의식의 개념이 아니라 판단하여 본고에서는 ‘생태학적’이라는 용어보다 ‘생태적’이라는 용어가 한국 설화를 분석하는데 더 적합하다고 생각하였다. 이는 인간과 자연을 공존하는 관계로 보고 그 생명적 가치와 존재 가치를 존중하고 인정하는 자연중심주의적 태도이다. 인간과 동물의 생태적 관계를 밝히기 위해 본고에서는 크게 세 가지 차원으 로 구분하였다. 우선 인간과 동물은 분명 생태적 습성이나 개체성은 다르지만 생태계를 이 루는 존재적 가치는 동등하다. 그러므로 인간 외의 다른 개체들의 생명 역시 소중하다고 인식하는 생명적 평등의 관계를 살펴보았다. 옛사람들은 인간과 동 물은 생명적으로 평등한 관계로 서로의 생명을 존중하고 함부로 그 생명을 경 시하지 않았다. 두 번째로 상호적 배려의 관계를 통해 인간이 동물의 입장이나 처지를 배려 하고 존중해주면 동물 역시 인간을 배려하고 그에 대한 보답으로 인간에게 혜 택을 베푸는 공생적 관계임을 확인하였다. 세 번째로 생태계를 구성하는 모든 존재들은 상호의존적인 관계로 연결되어 있기에 인간과 자연 역시 선인선과, 악인악과와 같은 유기적 순환의 관계를 맺고 있다. 그러나 자연을 대하는 인간의 행위가 인간의 입장에서는 선한 행위일지라도 생태적 질서를 거스르는 경우에는 좋지 못한 결과를 초래할 수도 있다. 이를 통해 인간과 동물이 유기적으로 얽혀 인간 행동의 결과가 주변 생명체에게까지 미칠 수 있다는 순환적 관계를 보여주고 있다. 지금까지 살펴본 인간과 동물과의 관계를 통해 우리는 옛사람들이 자연을 단순히 인간을 둘러싼 환경의 일부로 보지 않고 그 생명적 가치를 소중히 여기 고 상호의존적 관계에서 유기적으로 연결된 공생적 존재로 인식했음을 알 수 있었다. 이것이 한국 설화가 담고 있는 인간과 동물의 생태적 관계이다.

        Electrical Characteristics of Poly(ethylene oxide)-urea Complex Films

        The electrical characteristics of complex films composed of poly(ethylene oxide) (PEO) and urea as a function of the urea concentration were examined in this study. Moreover, their structural characteristics were also compared. Depending on the urea concentration, the structural phases were classified as PEO+β-phase composite, β-phase+α-phase composites, or α-phase composite+urea. At urea concentrations below ~0.064 M, the β-phase was dominant in the complex film. Moreover, the conductance increased rapidly with an increase in the urea concentration. For urea concentrations ranging from ~0.064 to ~0.25 M, the β-phase was gradually substituted by the α-phase. As the film was composed entirely of the α-phase at urea concentrations greater than ~0.25 M, its conductance was decreased. In this study, the electrical characteristics observed for the different phases are analyzed and discussed.
