Author performed choledochojejunostomy and cholecystectomy for the purpose of biliary decompression of 5 experimental dogs, after 10 days following complete ligation of the common bile duct. Various changes in hematologic values, liver function tests, bacteriological findings, biliary-intestinal reflux and histopathological findings of biliary tract were observed in the groups of ligated common bile duct and choledochojejunostomy. The results obtained were as follows; 1) There were no remarkable changes except leukocytosis in the group of ligated common bile duct and choledochojejunostomy. 2) Each value of alkaline phosphatase activity, S-G.O.T., S-G.P.T., total bilirubin and thymol turbidity was markedly increased in the group of ligated common bile duct. These serum components were slightly decreased after choledochojejunostomy but revealed higher level than those of the control group. 3) The presence of bacteria in bile of the control group and the ligated common bile duct was not demonstrated, however in the choledochojejunostomy group, E. coli, enterococcus, paracolon baccilli and proteus were demonstrated. 4) The anastomosed stomas between common bile duct and jejunum were stenosed slightly to moderately. Infection and reflux of contents of intestine into the biliary tract were found in all cases of choledochojejunostomy. 5) In the group of the ligated common bile duct, hepatic cell and Kupffer's cell which contained much bile pigments and bile plugs in the bile canaliculi were very remarkable around the centrilobular area. Above mentioned phenomena were less prominent in the group of the choledochojejunostomy than in that of the ligated common bile duct. 6) The common bile duct demonstrated marked infiltration of the inflammatory cells in the group of the choledochojejunostomy.
幽門形成術과 幽門形成 및 迷走神經切斷倂行施術이 胃液電解質에 미치는 影響에 關한 實驗的 硏究

This study is to evaluate the electrolyte changes in canine gastric contents following pyloroplasty and pyloroplasty with vagotomy. Six dogs were used. In 2 dogs, gastric fistula only was created and used for control. Pyloroplasty and pyloroplasty with vagotomy were performed in 2 each dogs respectively. The gastric content was obtained 1 week postoperatively, 4 times thereafter with 1 week interval. The result obtained were as follows: 1) In pyloroplasty group, the levels of sodium and potassium of the gastric content increased by 28.6% and 28.2% of the control group respectively. Choloride, however, was not remarkable changed. 2) In pyloroplasty with vagotomy group, the levels of chloride and potassium of gastric content were decreased by 33.0% and 30.6% of the control respectively. Sodium, however, was not remarkably changed.

Objectives:Keyhole surgery through supraorbital route with eyebrow incision is minimal invasive technique for anterior cranial fossa pathology. Traditional method need to remove supraorbital rim, result in postoperative ecchymosis around eye. The authors performed surgery without removal of supraorbital rim and want to introduce the surgical technique and results. Methods:Among the patients who were found to have the tumor in the anterior cranial fossa and parasellar area, 9 patients were operated by supraorbital approach. There are 2 tuberculum sellar meningiomas, 3 anterior skull base meningiomas, 1 parasellar cavernous hemangioma, 1 Rathke`s cleft cyst, 1 metastatic tumor and 1 optic glioma. Skin incision was made along lateral two third of eyebrow. Small size craniotomy was made just upper part of supraorbital rim. The opening size was 1.5×2.5 cm. Next technique is almost same with traditional supraorbral approach. The surgical results were studied retrospectively. Results:Largest tumor was 4×5×5 cm in size. Gross total removal was possible in all the patients. There was no surgical mortality and imaging study showed no surgical complication without one case. One patient suffered from hypothalamic dysfunction and sustained moderately disabled state for 6 months but the others showed excellent outcome(modified Rankin Score 0 to 1). The average postoperative hospital day was 9.6 days except 1 patient. All patients were satisfied with cosmetic results. Short Form-36® health survey showed excellent self estimation after surgery. Conclusion:Minimal invasive approach through eyebrow incision can achieve same results like conventional craniotomy in selected case. Supraorbital rim removal is not an obligation during this procedure.
