        임상간호사의 성찰적 사고 수준과 성찰 조건이 간호역량에 미치는 영향

        The aim of this study was to identify the effect of clinical nurses' reflective thinking levels and conditions for reflection on nursing competency. Participants were 235 clinical nurses who worked in 4 general hospitals in Gwangju Metropolitan City. Data were collected by self-reporting questionnaires during June and July, 2011. Reflective thinking levels, conditions for reflection and nursing competency measurements were used as measurement tools. Data were analyzed using frequencies, means, t-test, one-way ANOVA, Pearson correlation coefficients, and backward multiple regression. Reflective thinking levels consist of intensive reflection, reflection, understanding, and habitual action. The mean score of intensive reflection was the highest, the next were reflection, understanding and habitual action in order. There were significant differences in intensive reflection and reflection according to age, total clinical career, and educational background. Intensive reflection, reflection, and conditions for reflection correlated positively with nursing competency. Multiple regression analysis showed that total clinical career, reflection explained 22.3% of nursing competency. This finding suggests that total clinical career and reflection are important factors on enhancing nursing competency, thus we recommend that reflective thinking training is a powerful strategy to promote the professional development for nurses. 본 연구는 임상간호사의 성찰적 사고 수준과 성찰 조건이 간호역량에 미치는 영향을 확인하기 위한 서술적 조사연구이다. 2011년 6월부터 7월까지 설문조사를 통해 G광역시 소재 4개의 종합병원 간호사 235명을 최종 분석하였다. 간호역량은 Jang(2000)의 13개 간호역량의 임상등급별 행동지표를 이용하여 측정하였고, 성찰적 사고 수준과 성찰 조건은 Peltier, Hay, Drago(2005)의 성찰적 학습 과정(reflective learning continuum)을 You(2009)가 번역·수정한 도구를 간호사에 맞게 수정하여 사용하였다. 수집된 자료는 서술적 통계와 t-검정, 분산분석(ANOVA), 피어슨(Pearson)의 상관계수, 후진제거 다중회귀분석을 하였다. 성찰적 사고 수준은 심층적 성찰, 성찰, 이해, 습관적 행위로 구성되는데, 간호사의 성찰적 사고 수준은 심층적 성찰이 가장 높았고 다음으로 성찰, 이해, 습관적 이해의 순서였다. 심층적 성찰과 성찰은 연령, 총 임상경력, 학력이 높은 군이 유의하게 높았으며, 습관적 행위는 연령과 학력이 높은 군이 유의하게 낮았다. 심층적 성찰과 성찰, 성찰의 조건인 동료와의 상호관계와 관리자와의 상호관계는 간호역량과 긍정적 상관관계가 있었고 회귀분석에서 총 임상경력과 성찰은 간호역량의 22.3%의 설명력을 나타냈다. 임상간호사의 성찰적 사고는 간호역량을 향상시키는 중요한 요인이므로 간호사들이 성찰적 사고를 할 수 있도록 교육 및 훈련체계를 마련하고 성찰조건인 실무환경을 능동적이고 협력적인 상호작용이 발생하도록 조성하여야 한다.

        임상간호현장에서의 실행연구 여정

        Purpose: The purpose of this study was to examine the effectiveness of Action Research (AR) approach in nursing. Methods: Participants were 64 perioperative nurses recruited from C hospital in Gwangju, Korea. The nurses were engaged in the project through 2 cycles of planning, acting, observing, and reflecting. A mixed-methods design was used to examine changes in participants and their knowledge management practice. Quantitative data were analyzed using SPSS 20.0 program and qualitative reflection data underwent content analysis. Results: During the project, participants developed standardized pre-operative checklists and opened an Internet Cafe to better manage their perioperative nursing information. At the end of the project, there was a significant increase in nurses' knowledge management (p=.015) and the rate of surgical material prescription errors decreased from 8.0% to 2.9%. Core AR project team members' teamwork skills and organizational commitment increased significantly (p=.040, p=.301, respectively). The main themes that emerged from the qualitative data were learning how to solve problems in practice, facilitating team activities through motivation, barriers of large participation, and rewarded efforts and inflated expectations. Conclusion: The AR project contributed to empowering participants to solve local problems. AR is a useful methodology to promote changes in practices and research participants.

      • 인사고과에 관한 국내 간호연구 동향분석

        Purpose: The authors reviewed researches related to performance appraisal(32 researches), in order to identify the direction for future research and to establish a credible performance appraisal system. Method: Almost all of the theses and research published in 9 major journals of nursing in korea were reviewed. Results: Most of research(84.4%) were theses and all researches were used to non-experimental design; survey 81.3%, methodological research 15.6%, review 3.1%. The major subjects of study were appraisees and appraisers. Psychosocial data collection which only used questionnaire were 81.3% and carried out interview(12.5%) and delphi-method(3.1%). Data analysis methods were used frequency 78.1%, t-test 62.5%, mean/SD 59.4%, Pearson's correlation 50%, and Cronbach α 50%. Appraisal tool used or developed in studies was graphic rating scales in nine studies and ran parallel with forced distribution in 2. Also, MBO and BSC were developed. Total number of korean terms in performance measurement were 11, and english terms 15. The tendency of the terms was toward performance appraisal or evaluation. Conclusion: In the light of results, we expect development of corporate appraisal tool that can evaluate nurse competence and performance. Total performance management system also should be established.

        대학생의 비판적 사고 성향과 정보 활용 능력과의 관계

        Purpose: The purpose of this study was to investigate the relationship between university student's critical thinking disposition and information literacy. Method: Data was collected from convenient sample of 267 University students (the 1 Syear - the 3"\tear) in the Nursing, Philosophy, Physical education, Human ecology classes in C university. The questionnaire measured the level of critical thinking disposition, information literacy. The data were analyzed using descriptive statistics, t-test, One-way ANOVA, Scheffe test, and Pearson's correlation coefficient. Result: The mean score of critical thinking disposition was 3.31 (±O.37), information literacy was 3.S0(±O.44). The critical thinking disposition was a significant positive correlation with information literacy. Conclusions: From the studies reviewed, critical thinking disposition can be expect the positive effect to increase the information literacy of university students. These findings would be expected to use basic study that undergraduate's development of improving learning ability strategies.

        임상간호사가 지각한 병원조직의 새로운 지적자본 창출 능력 영향요인

        This study was designed to identify creation of intellectual capital and factors influencing the creation of intellectual capital in hospital organizations. Methods: A cross-sectional design was used, with a convenience sample of 300 nurses from three general hospitals in Gwangju and South Cholla province. The tools used for this study were scales measuring creation of intellectual capital (8 items), social capital (20 items), knowledge management (8 items). Data was analyzed with descriptive statistics, Pearson`s correlation coefficient and regression using the SPSS WIN 18.0 program. Results: There were significantly positive relationships between social capital and knowledge management with creation of intellectual capital. The mean score for creation of intellectual capital in hospital organizations was 4.59 points. Factors influencing creation of intellectual capital in hospital organization were identified as knowledge management (β=.625), shared values & action (β=.166), and participation in civic activities (β=.118). These factors explained 59.3% of the variance in creation of intellectual capital in hospital organizations. Conclusion: The results indicate which factors are major factors influencing creation of intellectual capital and therefore, serve as predictors of creation of intellectual capital in hospital organizations.

        웹 기반의 멀티미디어 심전도 모니터링 학습 프로그램 개발 및 적용에 관한 연구

        The aim of this study was to develop and evaluate a web-based learning program with multimedia ECG monitoring for clinical nurses. For this, we compared the self-directed, web-based learning method with the traditional lecture method. A quasi-experimental, nonequivalent control group pretest-posttest design was used, the data from 32 nurses(17 for experimental group and 15 for control group) were collected from October 1 to November 30, 2001, and analysed by SPSS program with 2 test, t-test, ANCOVA. The results were as follows : 1) There was significant improvement in the score on learning achievement of the experimental group compared with that of the control group(t=-2.739, p=.010). 2) There was no significant difference between the groups in learning motivation(t=-.054, p=.4785). In conclusion, it is suggested that a higher learning achievement was achieved in self-directed, web-based learning than in the traditional classroom style learning. There was also evidence that two educational styles had same effect in creating motivation. We propose to utilize a self-directed, web-based leaning method as a means to continue life-long education for nurses on a rotational duty system.

        국내 의료서비스디자인연구 동향분석

        Purpose: Purpose of this study was to analyze research on the service design process applied to domestic medical service areas. Methods: A review was made through domestic databases including RISS, KISS, DBpia, and NDSL and for the analysis framework: a medical service classification code which integrated the medical service area and the design fields. Results: In the healthcare service field there were 9 studies (69.2%) in the medical area, 1 study (7.7%) each in nursing and oriental medicine, and 2 studies (15.4%) in healthcare. According to analysis results based on the medical service classification code, there were 5 studies in prevention and management, 6 studies in curative care, 1 each in rehabilitation and ancillary care. Double diamond process was used in 8 studies. Conclusion: Service design was applied mainly to the curative care in the domestic medical settings but little research on service design in the long-term nursing care area was identified. As a strategy to improve the quality of nursing service, it is necessary to adopt the service design process for various nursing service areas in Korea.
