      • A case of erosive adenomatosis of nipple

        Erosive adenomatosis of the nipple is an uncommon benign tumor originating from the lactiferous duct. It usually occurs unilaterally in women in their 40s. The lesions frequently present as eczematous lesions such as erythematous edema or erosion during the initial stage, and thereafter, serous secretions, ulceration, or crust may be accompanied. A 38-year-old female presented with solitary verrucous match-head to pea sized plaque on the left nipple. She had a 3-year history of skin lesion with pus-like discharge on her left nipple. Biopsy specimen from the nipple showed proliferation of irregularly shaped tubular structures in the dermis. Papillary projections of ductal cells into the lumina were seen. Ducts were lined by both an inner columnar epithelium and an outer cuboidal epithelium. Erosive adenomatosis of nipple has been reported rarely - 7 cases were previously reported in Korea. We report this interesting case for educational purpose.

      • Chemical leukoderma of the scalp caused by the hair dye

        Chemical leukoderma is defined as an acquired, hypopigmented dermatosis induced by repeated exposure to specific chemical compounds that destroys epidermal melanocytes. Early diagnosis of chemical leukoderma is crucial, as it could halt the progression of depigmentation unlike idiopathic vitiligo. Three Korean men presented with multiple, irregular, progressive hypopigmentated patches over their scalp after hair dying. Patch tests were conducted on the two patients, and both of them showed positive results to para-phenylenediamine(PPD). Histopathological evaluation from three patients revealed decreased melanin pigmentation in basal layer, and the patients were finally diagnosed as chemical leukoderma induced by hair dye. Although there is no formal diagnostic criteria about chemical leukoderma at present, clinical appearance, history, and histopathologic findings may be useful in distinguishing chemical leukoderma from other hypomelanotic diseases such as idiopathic vitiligo or post-inflammatory hypopigmentation. Chemical leukoderma occurring as a toxic effect has been reported secondarily to a number of substances, especially when the substances has benzene ring. The PPD also has a benzene ring in their chemical structure. There is only one case report of chemical leukoderma induced by PPD in Korean literature. Therefore, we herein report these three interesting cases of chemical leukoderma that developed after exposure to hair dye.

      • A case of nevus spilus with congenital melanocytic nevus

        Nevus spilus is a relatively common skin lesion characterized by multiple pigmented macules or papules within a pigmented patch. It is controversial whether NS is congenital or acquired. Congenital melanocytic nevus is defined as a cutaneous lesion appeared at birth, containing melanocytic nevus cells. It is characterized by the diffuse infiltration of melanocytes in the deepest reticular dermis and subcutis, and by the presence of melanocytes around skin appendages. A 16-year-old female patient presented with a congenital pigmented skin lesion on the left upper arm. Speckled brownish to black colored macules with background light brown patch was noticed. Skin biopsy was performed on her left upper arm, and showed pigmentation over dermal-epidermal junction. It also showed mild elongated rete ridge over diffuse junctional nest and nevus cell of upper dermis, with proliferating melanocyte along elongated rete ridge. Nevus cells were also present around hair follicles, sweat glands and vessels in the deep dermis. Immunohistochemical staining showed melanocytes on upper dermis which is positive in HMB-45 staining and basal layer cells with positive in S-100 protein staining. Therefore, the lesion was diagnosed with a nevus spilus combined with congenital melanocytic nevus. This case presents the evidence for the congenital nature of nevus spilus. Herein, we report a case of nevus spilus with congenital melanocytic nevus.

      • Nevus lipomatosus cutaneous superficialis: an unusual presentation on the scalp

        A 76-year-old female presented with a gradually enlarging skin lesion on the scalp with severe itching over 50 years. Physical examination revealed localized skin colored skin-fold patches on the scalp. Histopathological examination revealed an orthokeratosis and scattered lobules of fat cells entrapped between bundles of dermal collagen fibers on the dermis. Based on the histopathological features, a diagnosis of a solitary form of Nevus lipomatosus cutaneous superficialis(NLCS) was made. NLCS is a rare benign hamartoma of adipose tissue in the dermis. It is clinically divided into two types, the classical Hoffman-Zurhelle or the multiple form and the pedunculated solitary form. Histopathologically, it is characterized by the presence of ectopic mature fat in the dermis mostly around the perivascular area and varies from 10% to 50% of the lesion. Excessive, loose, or irregular organization of the connective tissue has been noted in many cases. In most cases, staining with alcian blue shows substantially increased deposition of mucopolysaccharides in the reticular dermis and fatty tissue, which was not evident in this case. We report this case as it is an uncommon condition with the intent to highlight its clinical and histopathological features, and differential diagnosis.
