Topical rapamycin: The promising therapeutic agent for lymphangioma circumscriptum

Lymphangioma circumscriptum (LC) is an uncommon lymphatic malformation characterized by grapelike groups of thin-walled translucent lymph-filled vesicles, often compared to frog spawn. The treatment of LC is challenging and complete surgical excision has the lowest recurrence rate but it is sometimes unavailable because of size of the lesion or cosmetic concern of postsurgical scar on the exposed area. Less invasive therapeutic options including laser or sclerotherapy can be tried but the recurrence tends to be common. A 23-year-old woman presented with papules resembling frog spawn on her left neck. The lesions appeared when she was 4 years old and she had treatment with CO2 laser and vascular laser ten more times at local clinic but they had not been improved enough. Skin biopsy was done from papules on her neck and histopathologic finding was consistent with LC. Surgical excision was unavailable because of the cosmetic concern of postsurgical scar. We instructed the patient to apply topical rapamycin cream twice a day. After 4 months of treatment, the lesions significantly improved in size and color compared to the initial presentation. She also had no side effects on the treated site. To the best of our knowledge, there is only one reported proceeding of topical rapamycin used to manage LC. We report this extremely rare experience of treatment and suggest topical rapamycin can be a good therapeutic option for the management of LC.

<P><B>Abstract</B></P><P>The JNK interacting protein, JSAP1, has been identified as a scaffold protein for mitogen-activated protein kinase (MAPK) signaling pathways and as a linker protein for the cargo transport along the axons. To investigate the physiological function of JSAP1 in vivo, we generated mice lacking JSAP1. The JSAP1 null mutation produced various developmental deficits in the brain, including an axon guidance defect of the corpus callosum, in which phospho-FAK and phospho-JNK were distributed at reduced levels. The axon guidance defect of the corpus callosum in the <I>jsap1</I><SUP><I>−/−</I></SUP> brain was correlated with the misplacement of glial sling cells, which reverted to their normal position after the transgenic expression of JNK interacting protein 1(JIP1). The transgenic JIP1 partially rescued the axon guidance defect of the corpus callosum and the anterior commissure of the <I>jsap1</I><SUP><I>−/−</I></SUP> brain. The JSAP1 null mutation impaired the normal distribution of the Ca<SUP>+2</SUP> regulating protein, calretinin, but not the synaptic vesicle marker, SNAP-25, along the axons of the thalamocortical tract. These results suggest that JSAP1 is required for the axon guidance of the telencephalic commissures and the distribution of cellular protein(s) along axons in vivo, and that the signaling network organized commonly by JIP1 and JSAP1 regulates the axon guidance in the developing brain.</P>

<P>In this Letter, we report Ge p-i-n avalanche photodetectors (APD) with low dark current (sub 1 μA below V(R)=5??V), low operating voltage (avalanche breakdown voltage=8-13??V), and high multiplication gain (440-680) by exploiting a point defect healing method (between 600C and 650C) and optimizing the doping concentration of the intrinsic region (p-type ~101? cm?3). In addition, Raman spectroscopy and electrochemical capacitance voltage analyses were performed to investigate the junction interfaces in more detail. This successful demonstration of Ge p-i-n APD with low dark current, low operating voltage, and high gain is promising for low-power and high-sensitivity Ge PD applications.</P>

The intracytoplasmic sperm injection (ICSI) procedure has recently been utilized to produce transgenic animals and may serve as an alternative to the conventional pronuclear microinjection in species such as pigs whose ooplasm is opaque and pronuclei are often invisible. In this study, the effects of sperm membrane disruption and electrical activation of oocytes on in vitro development and expression of transgene green fluorescent protein (GFP) in ICSI embryos were tested to refine this recently developed procedure. Prior to ICSI, sperm heads were treated with Triton X-100+NaCl or Triton X-100+NaCl+NaOH, to disrupt membrane to be permeable to exogenous DNA, and incubated with linearized pEGFP-N1 vector. To induce activation of oocytes, a single DC pulse of 1.3 kV/cm was applied to oocytes for $30{\mu}sec$. After ICSI was performed with the aid of a micromanipulator, in vitro development of embryos and GFP expression were monitored. The chemical treatment to disrupt sperm membrane did not affect the developmental competence of embryos. 40 to 60% of oocytes were cleaved after injection of sperm heads with disrupted membrane, whereas 48.6% (34/70) were cleaved without chemical treatment. Regardless of electrical stimulation to induce activation, oocytes were cleaved after ICSI, reflecting that, despite sperm membrane disruption, the perinuclear soluble sperm factor known to mediate oocyte activation remained intact. After development to the 4-cell stage, 11.8 (2/17, Triton X-100+NaCl+NaOH) to 58.8% (10/17, Triton X-100+NaCl) of embryos expressed GFP. The expression of GFP beyond the stage of embryonic genome activation (4-cell stage in the pig) indicates that the exogenous DNA might have been integrated into the porcine genome. When sperm heads were co-incubated with exogenous DNA following the treatment of Triton X-100+NaCl, GFP expression was observed in high percentage (58.8%) of embryos, suggesting that transgenic pigs may efficiently be produced using ICSI.

OBJECTIVE:: The impact of exaggerated blood pressure response (EBPR) to exercise on left ventricular function and the mechanism of its association are poorly understood. This study investigated the impact of arterial stiffening on left ventricular function in individuals with an EBPR to exercise. We hypothesized that individuals with low pulse pressure (PP) amplification during exercise would have worse left ventricular function than those with high PP amplification in individuals with an EBPR to exercise. METHODS:: Fifty-nine individuals with an EBPR to exercise (18 men, age 57 ± 12 years) and 59 age and sex-matched controls were studied. Radial artery tonometry was performed at rest and immediately after exercise during supine bicycle exercise echocardiography. RESULTS:: There were no differences in left ventricular structure or function between individuals with an EBPR to exercise and controls. When individuals with an EBPR to exercise were divided into two groups on the basis of PP amplification after exercise [Group 1 (n = 30), high PP amplification after exercise; Group 2 (n = 29), low PP amplification after exercise], group 2 showed larger left atrial volume and lower early diastolic (e’) and systolic (S’) mitral annular velocities. Left ventricular apical rotation was also exaggerated in group 2. In multiple regression, PP amplification after exercise was an independent determinant of e’ (&bgr; = 0.16, P = 0.019) and S’ (&bgr; = 0.25, P = 0.009) in individuals with an EBPR to exercise. CONCLUSION:: In individuals with an EBPR to exercise, the degree of left ventricular dysfunction is variable. EBPR to exercise in the presence of arterial stiffening contributes to the deterioration of left ventricular function.
A plaque-type variant of acquired rhabdomyomatous mesenchymal hamartoma on the chin

Rhabdomyomatous mesenchymal hamartoma (RMH) is a rare congenital skin disease. The lesion commonly presents as a solitary sessile papule or nodule on the head and neck in childhood. Its pathologic finding is characterized by aberrant existence of striated muscle fibers in the dermis and subcutaneous tissue. The treatment of typical RMH usually can be a simple excision. However, certain variant of acquired RMH presenting as a plaque on the chin tends to be difficult to be simply excised. To the best of our knowledge, there have been only two reported cases of plaque-type variant of acquired RMH in pubmed searching. A 22-year-old man presented with an asymptomatic erythematous indurated plaque on the chin. The lesion appeared 18 months ago and has shown recent increase of size. Incisional biopsy was performed on the chin and histopathologic finding showed mature striated muscle fibers interspersed haphazardly with collagen fibers and adipocytes in the dermis and subcutaneous tissue. The diagnosis of plaque-type RMH was made based on clinicopathologic finding. We recommended observation with regular follow-up because the lesion was too wide to be excised. We report an extremely rare case of plaque. type variant of acquired RMH and discuss its peculiar clinicopathologic features with the review of literatures.

We report the case of a 23-year-old woman who developed bowenoid papulosis of the vulva and subsequent periungual Bowen`s disease. She had a history of a long standing periungual wart on her right thumb before the outbreak of periungual Bowen`s disease. By HPV DNA chip, human papillomavirus (HPV) 11, 18 and 31 were identified from the periungual lesions, and HPV 11, 18 and 33 from the vulvar lesion. This case supports the theory of anogenital-digital spread of HPV, and proposes that the periungual wart may change into Bowen`s disease by mucosal HPVs. To the best of our knowledge, this case is important as the first Korean case of periungual Bowen`s disease concurrent with bowenoid papulosis of the vulva. (Ann Dermatol 23(4) 493 ~496, 2011)
