        Congenital Acute Megakaryoblastic Leukemia in a Neonate with Antenatal Hepatosplenomegaly

        Various conditions can lead to antenatal hepatosplenomegaly. In addition to commonly known etiologies like hydrops fetalis or viral illnesses, hematologic disorders like congenital leukemia, although rare, should also be ruled out. We present a case of hepatosplenomegaly detected during antenatal screening in a neonate, in whom acute megakaryoblastic leukemia was diagnosed postnatally based on the immunophenotype.

        Left Ventricular Remodeling in Patients with Primary Aldosteronism: A Prospective Cardiac Magnetic Resonance Imaging Study

        Objective: This study used cardiac magnetic resonance imaging (MRI) to compare the characteristics of left ventricular remodeling in patients with primary aldosteronism (PA) with those of patients with essential hypertension (EH) and healthy controls (HCs). Materials and Methods: This prospective study enrolled 35 patients with PA, in addition to 35 age- and sex-matched patients with EH, and 35 age- and sex-matched HCs, all of whom underwent comprehensive clinical and cardiac MRI examinations. The analysis of variance was used to detect the differences in the characteristics of left ventricular remodeling among the three groups. Univariable and multivariable linear regression analyses were used to determine the relationships between left ventricular remodeling and the physiological variables. Results: The left ventricular end-diastolic volume index (EDVi) (mean ± standard deviation [SD]: 85.1 ± 13.0 mL/m2 for PA, 75.9 ± 14.3 mL/m2 for EH, and 77.3 ± 12.8 mL/m2 for HC; p = 0.010), left ventricular end-systolic volume index (ESVi) (mean ± SD: 35.2 ± 9.8 mL/m2 for PA, 30.7 ± 8.1 mL/m2 for EH, and 29.5 ± 7.0 mL/m2 for HC; p = 0.013), left ventricular mass index (mean ± SD: 65.8 ± 16.5 g/m2 for PA, 56.9 ± 12.1 g/m2 for EH, and 44.1 ± 8.9 g/m2 for HC; p < 0.001), and native T1 (mean ± SD: 1224 ± 39 ms for PA, 1201 ± 47 ms for EH, and 1200 ± 44 ms for HC; p = 0.041) values were higher in the PA group compared to the EH and HC groups. Multivariable linear regression demonstrated that log (plasma aldosteroneto- renin ratio) was independently correlated with EDVi and ESVi. Plasma aldosterone was independently correlated with native T1. Conclusion: Patients with PA showed a greater degree of ventricular hypertrophy and enlargement, as well as myocardial fibrosis, compared to those with EH. Cardiac MRI T1 mapping can detect left ventricular myocardial fibrosis in patients with PA.

        An Efficient Complex Event Processing Algorithm based on INFA-HTS for Out-of-order RFID Event Streams

        With the aim of solving the problems of long processing times, high memory consumption and low event throughput in the current processing approaches in out-of-order RFID event streams, an efficient complex event processing method based on INFA-HTS (Improved Nondeterministic Finite Automaton-Hash Table Structure) is presented in this paper. The contribution of this paper lies in the fact that we use INFA and HTS to successfully realize the detection of complex events for out-of-order RFID event streams. Specifically, in our scheme, to detect the disorder of out-of-order event streams, we expand the traditional NFA model into a new INFA model to capture the related RFID primitive events from the out-of-order event stream. To high-efficiently manage the large intermediate capturing results, we use the HTS to store and process them. As a result, these problems in the existing methods can be effectively solved by our scheme. The simulation results of our experiments show that our proposed method in this paper outperforms some of the current general processing approaches used to process out-of-order RFID event streams.

      • Pemetrexed is Mildly Active with Good Tolerability for Treatment of Patients with Colorectal Cancer

        Purpose: This systematic analysis was conducted to evaluate the efficacy and safety of pemetrexed based salvage chemotherapy for treatment of patients with metastatic colorectal cancer. Methods: Clinical studies evaluating the efficacy and safety of pemetrexed based regimens on response and safety for patients with colorectal cancer were identified using a predefined search strategy. Pooled response rates (RRs) were calculated. Results: For pemetrexed based regimens, 4 clinical studies including 201 patients with advanced colorectal cancer were considered eligible for inclusion. The analysis suggested that, in all patients, pooled RR was 20.4% (41/201). Major adverse effects were neutropenia, anorexia, fatigue, and anemia. No treatment related death occurred with pemetrexed based treatment. Conclusion: This systematic analysis suggests that pemetrexed based regimens are associated with mild activity with good tolerability in treating patients with metastatic colorectal cancer.
