An 51-year-old woman presented with microscopic hematuria without protenuria for long time. Laboratory studies demonstrated the presence of red blood cells in urine, a normal serum IgM level, the absence of antinuclear antibodies, and a normal complement level. Renal biopsy revealed that some glomeruli are enlarged with endocapillary cell proliferation and a few glomeruli exhibit prominent vascular pole of the tufts and segmental increase in mesangial cell and matrix. Immunofluorescence studies demonstrated segmental granular deposits for IgM. Electron microscopy showed well-preserved foot process associated with focal effacement. Biopsy findings were consistent with IgM nephropathy. We present this case to promote understanding of the pathogenesis of IgM nephropathy. (Korean J Nephrol 2002;21(2):317-321)
各種 脊椎動物의 平滑筋에 對한 非아드레날린性 및 非콜린性 神經支配에 關한 硏究

Strips of various smooth muscle from some vertebrate species were dissected and suspended in organ bath which was filled with nutrient solution. Oxygen was continuously supplied with bubbling. Responses of these preparations to the perivascular or transmural stimulation and effects of some autonomic drugs on the responses were observed to investigate the innervation of the smooth muscle, which was neither adrenergic nor cholinergic. Effects of exogenously applied adenosine triphosphate(ATP) on these preparation were also studied. The results obtained were as follows: 1. Transmural stimulation produced marked contraction of the vas deferens and seminal vesicle muscle of the rabbit, and perivascular stimulation also produced marked contraction of the rectal muscles of the hen. Exogenously applied ATP evoked similar response in these muscles. 2. Perivascular stimulation produced marked contraction of the detrusor muscle of the rabbit and transmural stimulation also produced marked contraction of the detrusor muscle of the tortoise(Amyda japonica), and exogenously applied ATP evoked similar response in these muscle. 3. The contractile response of the detrusor, vas deferens, seminal vesicle and rectal muscles to the perivascular or transmunal stimulation was not affected in the presence of regitine, propranolol or atropine. 4. These results summarized above suggest that various smooth muscle of some vertebrates investigated in this study are innervated by non-adrenergic and non-cholinergic excitatory fibers that mean neurotransmitter involved in the smooth muscle is not acetylcholine or norepinephrine.
腎臟學 硏究 : 第1報 腎症候群에서의 血淸脂質에 關한 臨床的 硏究

The authers studies clinically at the profile of hyperlipidemia in 26 cases adult nephrotic syndrome, consist of 17 male and 9 female, ranging from 16 to 49 years old in recent at Jeonbug National University Hospital. 1. The mean value of serum total cholesterol and triglyceride of nephrotic patients were 471.1±201.3㎎% and 285.6±97.9㎎% respectively, and serum triglyceride levels were directly correlated with their serum total cholesterol levels. 2. It was observed tendency that serum cholesterol levels of nephrotic patients increase as their serum albumin levels decrease but no significants. 3. The significant relationship were no observed among glomerular filtration rate(GFR) and serum cholesterol and triglyceride levels changes. 4. The significants negative relationship were found between GFR and urine total protein amounts per day changes.
A Study on the Renal Functions of the Acute Pyelonephritis

한국인은 정상신장에 있어서도 요농축능이 서구인에 비하여 저하되어 있다. 그 원인이 현재까지는 우리의 특이한 식이습성때문이라고 사료된다. 저자들은 급성신우신염으로 확진된 환자 95예를 대상으로 여러가지 신장기능의 변화를 검토하여 그 성적을 제시하는 바이다. 1. 검토된 급성신우신염의 년령군은 20대가 전체의 31.6%로 가장 많았고, 30대가 21.1%로, 20~30대가 전체의 52.7%였으며 남녀비는 1:9.5로 임신적령기 여성에 많았다. 2. 대조군으로 정상인 혈청 osmolality는 319±9.2mOsm/L이고 혈청 Na 및 K는 각각 145.1±8.2mEq/L 및 4.6±0.7mEq/L이었다. 또한 정상인 24시간 요로배설되는 총 Na 및 K는 각각 188.4±0.9mEq 및 37.1±23.6mEq이였고 요 osmolality는 702.0±222.6mOsm/L이었다. 3. 급성신우신염시 혈청 osmolality는 297.1±16.4mOsm/L이고 혈청 Na 및 K는 각각 136.6±8.6 mEq/L 및 3.9±0.9mEq/L이었다. 한편 요 osmolality는 377.7±151.5mOsm/L이고 24시간 배설되는 총 요중 Na 및 K는 각각 165.6±40.8mEq 및 30.3±18.1 mEq이었다. 4. 정상인 신사구체 여과율은 113.6±24.9ml/min인데 반하여 급성신우신염시는 91.9±35.7ml/min이었다. 5. 정상인 요pH는 6.09±0.9였는데 급성신우신염시는 6.39±0.7이었다. 이상의 결론으로는 급성신우신염시 요 osmolality농도에 유의한 변화가 있었는데 이는 신장의 요농충능의 감소에 기인한 것으로 사료된다.
