Background Even amid the coronavirus disease-19 (COVID-19) pandemic, the spread of multidrug-resistant bacteria and infection control are still important tasks. After recognizing the carbapenem-resistant Acinetobacter baumannii (CRAB) outbreak that occurred in the isolation room for COVID-19, we would like to introduce what infection control measures were implemented to eradicate it. Materials and Methods All COVID-19 patients with CRAB in any specimen admitted to the COVID-19 isolation ward of the tertiary hospital in Korea from October to November 2021 were analyzed. Results During the outbreak, 23 patients with COVID-19 and CRAB infections were identified. The index case was an 85-year-old female referred from a long-term care facility. CRAB was identified in sputum culture in most patients (91.3%). The CRAB outbreak occurred mainly in the rooms around the index case. Environmental cultures on the floor, air inlet, air outlet, and window frame of the rooms were performed. The antimicrobial resistance patterns of CRAB from patients and the environment were identical; whole-genome sequencing analyses revealed isolated clonality. Infection control measures with enhanced environmental cleaning using 1,000 ppm sodium hypochlorite and phenolic compounds, enhanced hand hygiene, additional education, and mandatory additional gowning and gloving of COVID-19 personal protective equipment (PPE) were applied on 29 October. No CRAB infection cases occurred from 2 November for two weeks. Conclusion In addition to applying PPE and COVID-19 precautions in COVID-19 isolation wards, adhering to strict contact precautions along with environmental control can help prevent the spread of multidrug-resistant bacteria. <i>Idh2</i> deficiency accelerates renal dysfunction in aged mice

<P><B>Abstract</B></P> <P>The free radical or oxidative stress theory of aging postulates that senescence is due to an accumulation of cellular oxidative damage, caused largely by reactive oxygen species (ROS) that are produced as by-products of normal metabolic processes in mitochondria. The oxidative stress may arise as a result of either increased ROS production or decreased ability to detoxify ROS. The availability of the mitochondrial NADPH pool is critical for the maintenance of the mitochondrial antioxidant system. The major enzyme responsible for generating mitochondrial NADPH is mitochondrial NADP<SUP>+</SUP>-dependent isocitrate dehydrogenase (IDH2). Depletion of IDH2 in mice (<I>idh2</I> <SUP>-<I>/</I>-</SUP>) shortens life span and accelerates the degeneration of multiple age-sensitive traits, such as hair grayness, skin pathology, and eye pathology. Among the various internal organs tested in this study, IDH2 depletion-induced acceleration of senescence was uniquely observed in the kidney. Renal function and structure were greatly deteriorated in 24-month-old <I>idh2</I> <SUP> <I>-/</I>-</SUP> mice compared with wild-type. In addition, disruption of redox status, which promotes oxidative damage and apoptosis, was more pronounced in <I>idh2</I> <SUP> <I>-/</I>-</SUP> mice. These data support a significant role for increased oxidative stress as a result of compromised mitochondrial antioxidant defenses in modulating life span in mice, and thus support the oxidative stress theory of aging.</P> <P><B>Highlights</B></P> <P> <UL> <LI> IDH2 is a major enzyme to maintain mitochondrial redox status. </LI> <LI> Depletion of IDH2 in mice shortens life span and accelerates aging. </LI> <LI> Renal function was greatly deteriorated in 24-month-old <I>idh2</I> <SUP>-<I>/</I>-</SUP> mice. </LI> </UL> </P>
A case of Sweet's syndrome localized in photodistributed area

Sweet's syndrome is characterized by fever, pain, and an increase in neutrophil counts in peripheral blood with erythematous papules or plaques with sudden pain. Histologically, it has a characteristic feature of neutrophilic infiltration in the dermis. Fever and increased neutrophil count in peripheral blood may vary from patient to patient. A 68-year old man visited our clinic with 2-month history of erythematous papules and pseudovesicles with sudden onset itching sense on the bilateral forearms. The lesions have some pain and mainly located on the sun exposed areas. Skin biopsy was performed on the forearm lesion. Histopathological examination revealed vacuolated basal keratinocyte and neutrophil populations in the dermis layer and solar elastosis. The treatment with antihistamines, topical steroids and sunscreen were effective and the patient was improved. Sweet's syndrome usually occurs on the face, neck, trunk, arms and legs. In this case, the lesions appeared only the sun exposed part of both arms. Herein, we report a case of photodistributed sweet's syndrome that is confined to the sun exposure area.

Hypertrichosis lanuginosa acquisita (HLA) is a very rare condition characterized by the appearance of fine lanugo hairs on the face, which are relatively long and have a few pigment. The lanugo hairs grow from the face, spread to the other part, which become darker and coarse. A 46-year-old woman presented with dense hair growth on face, neck, shoulder and back, which began a year ago. Blood tests and imaging studies were performed to exclude the possibility of malignancy or hormonal and autoimmune disorders associated with sudden onset of acquired hypertrichosis. There was abnormal in liver function test and positive in autoimmune antibody test. Furthermore, the hepatitis viral markers were negative and liver biopsy confirmed the prominent autoimmune hepatitis. Finally, the patient was diagnosed with HLA associated with autoimmune hepatitis. HLA is often associated with endocrine or metabolic disorders (e.g. hyperthyroidism and porphyria), malignancy and medication (e.g. cyclosporine, phenytoin, minoxidil). HLA with autoimmune hepatitis is rare and the precise mechanisms of autoimmune hepatitis and HLA have not been elucidated yet. Our observation expands the spectrum of diseases associated with HLA.
Bullous fixed drug eruption due to topiramate

Fixed drug eruption is a well-demarcated, erythematous, patchy or blistering skin lesion that occurs repeatedly at the same site. The lesion occurs when exposed to the causative agent, leaving hyperpigmentation after healing. A 32-year-old young woman visited our clinic with a wrinkled blister-like lesion on her wrist 3-4 days earlier. She had a similar lesion occurred in the same site and the scar was left. She had no specific illness but was taking acetaminophen, alginic acid, caffeine and topiramate for the purpose of weight loss. Based on the fact that the lesion recurred mainly in similar locations, patch tests were performed on the normal skin of the patient for the four suspected drugs. Patch test showed positive finding for topiramate. In the case of a fixed drug eruption showing a blister lesion observed on the wrist, it may be necessary to distinguish between friction blister and other bullous diseases. In recent years, the use of weight-loss drugs has been increasing, so it is necessary to recognize the drug eruption of these drugs. Therefore, we report a rare case of bullous fixed drug eruption caused by topiramate for the weight loss.

This study aims to empirically examine the effect of strain on their negative emotion of inmates in the Military Correctional Institution, and to suggest an improvement plan. The survey was given to all inmates in the institution (N=95). Five cases were removed due to incomplete responses, and 90 cases were utilized for the analyses. Results show that failure in achieving goals is a measurement of strain and it influences negative emotions in the inmates. However, negative life events and removal of positively valued stimuli are not associated with negative emotion. The inmates may feel anxiety because they are young (in their 20s) and are expected to achieve some educational or occupational goals and because they may feel a great sense of failure as first-time inmates. Given the results, policy implications were suggested including plans for reliving inmates’ strain and preparing various educational and occupational programs for inmates’ reentry into society.
