      • Clinical Characteristics and Treatment Outcomes of Chronic Pulmonary Aspergillosis: a Retrospective Analysis of 70 Patients

        Backgroud: Chronic pulmonary aspergillosis (CPA) is uncommon and there are few data on the clinical characteristics, optimal therapeutic strategies, and outcomes. Methods: We retrospectively reviewed the records of patients with newly diagnosed CPA and assessed the clinical, radiologic, and laboratory responses, and outcomes between January 2008 and January 2012. Results: A total of 70 CPA patients were included. Median age was 55 years and 51 patients (72.9% ) were men. Fifty-seven patients (81.4%) had a history of pulmonary tuberculosis and 32 patients (45.7%) had non-tuberculosis mycobacterium (NTM) lung disease, 17 of whom had a history of NTM treatment and 15 was under NTM treatment. Sixty-nine patients (98.6%) received oral itraconazole, except one with oral voriconazole for a median duration of 6.2 months. Symptomatic improvement occurred in 52 patients (74.3%), radiologic improvement occurred in 29 (41.4%), and laboratory tests improved more than 60% of all patients. Thirty-six patients (51.4%) achieved treatment success, defined as symptomatic improvement after at least 6 months of therapy regardless of radiological improvement, and 4 of whom needed retreatment after a median of 8.4 months. There was no difference in clinical characteristics and outcomes according to NTM status and 10 (14.3%) discontinued therapy due to adverse reactions. Death occurred in 10 (14.3%) and median follow-up duration was 11.2 months. Conclusion: Antifungal therapy for about 6 months and reassessment of disease might be feasible treatment strategy in managing patients with CPA.

        초음파 기관지 내시경으로 진단한 기관기관지 골연골형성증 1예

        Tracheobronchopathia osteochondroplastica (TO) is a rare benign disease of unknown etiology characterized by accumulation of calcium phosphate in the submucosa of large airways and benign proliferation of bone and cartilage resulting in nodular formation. We report a case of tracheobronchopathia osteochondroplastica diagnosed by Endobronchial ultrasonography in a 56-year-old man. Chest Computed Tomography revealed thickening of tracheal and bronchial wall, and multiple nodules through whole trachea. Endobronchial ultrasonography showed numerous submucosal nodules with hetero-echogenecity in the third and fourth layers. Histopathological examination revealed nonspecific bronchitis with squamous metaplasia and metaplastic ossification. We confirmed tracheobronchopathia osteochondroplastica. The patient's symptoms were successfully treated with antibiotics and oxygen supplyment. endobronchial ultrasonography can helpful diagnosis in tracheobronchopathia osteochondroplastica.

        The purpose of this study was to investigate if there is any difference in the level of competitive state anxiety according to the level of motor skill before the game and to provide the players with basic data for the mental practice. The subjects of this study consist of 20 men's a middle school tennis players. The experimental group was divided into superior players group and inferior players group. And then by using a CSAI-2 questionnaire, three subfactors of competitive state anxiety, or cognitive state anxiety, somatic state anxiety and state self-confidence were measured. The method of measurement was carried out three times, or one week, one day and two hours before the game. The results of this study were as follows. First, About both cognitive state anxiety and somatic state anxiety, inferior players were significantly higher than superior players. Second, About state self-confidence, superior players were significantly higher than inferior players. Third, As the game was around, both cognitive state anxiety and somatic state anxiety which all the players including superior and inferior players had increased. But the state self-confidence of all players decreased.

        Asarum consists low-growing herbs and is a genus in the Aristo-lochiaceae family with species found in the north temperate zones with most species in Asia. Any species with a highly structured variation pattern must show different distributions of phenotypic characteristics at different locations. I analysed intra- and interspecific phylogenic relationships within genus Asarum in Korea. I evaluated a representative sample of the fourteen taxa (eight Korean taxa and six obtained taxa from GenBank) with the trn L intron and the trn L - trn F spacer region of the chloroplast genome to estimate genetic relationships within genus Asarum. No variable positions were found within the trn L 5` exon and trn L 3` exon. Variable positions within the intron region were mostly distributed outside the structural elements required for intron processing, i.e., not within the first 140 bp at the 5` end and not within the 90 bp at the 3` end of the intron region. Within the spacer region, variable positions were evenly distributed. Alignment of the DNA sequences required adding numerous gaps. Sequence variation within the Asarum is mostly due to nucleotide substitutions, although several small indels and 20 inserts can be found. Another source of sequence divergence is length variation due to stretches of short repeats that occur at the intergenic spacer region ITS2 in all the Asarum. A + T content for eight Korean species of genus Asarum ranged between 64.3% and 64.6%. The base furtherance did not showed the significant difference to the by a total taxon. These values are similar to those (64.5%~67.1%) for the angiosperm alignments of the total trn L and trn F region. The main phylogenetic analysis revealed two distinct clades. The first clade includes ten species (eight Korean species, A. nipponicum, and A. tamaensis). The second clade includes four species (A. canadense , A. cautatum, A. chingchengense , and A. yakusimense). However, some internal nodes are poorly supported. Within Korean Asarum, four species were unsolved portions.

        Background: Recently the health insurance review agency tried to cut off the cost of unnecessary hospital stay. It is important to reduce the prolonged occupation of the hospital beds. However, there are some obstacles of the management for the prolonged bed occupation. We report our experiences and the problems of bed-control. Methods: To increase the rate of effective bed-usage, a task force was made. This team was consisted of 4 dctors, 2 nurses and 2 personnel of the administrative department. From December 20th 2001 to May 17th 2002, the task force analysed the rate, number and causes of the prolonged (more than 60 days, regardless of the disease entities) bed occupation. We announced and explained the necessity of bed-control and actively pressed to reduce the prolonged bed occupation. Results: The number of the prolonged bed occupation was 71 at the beginning of the management. It was reduced to 46 at best, then turned back to 65 at the end. It seems to be impossible to reduce the prolonged bed occupation until there are sufficient numbers of long-term care facilities. Aggressive bed control makes the sick patients to move every two or three months, it is never desirable. Conclusion: We cannot expect any Quality Improvements without improvement of the wrong system. Prior to the management for the prolonged bed occupation, it is necessary to build a sufficient number of long-term care facilities.

        Catechol-O-methyltransferase (COMT; EC 2.1.1.6) is the enzyme which catalyzes the transfer of methyl group to the catecholamine neurotransmitters from its methyl donor S-adenosyl-L-methionine. Partially purified COMT from rat liver has been used to measure the concentration of catecholamines in the blood through solvent extraction and thin layer chromatography after converting ^3H-methyl derivatives of catecholamines. To improve this inconvenient and fluctuating method, an attempt was made to use COMT gene for the measurement of catecholamines. Specific primers, COMT5P (5'-TGC TCA GAG GTG CTT TGA AG-3') and COMT3P (5'-GGA GCC GCA GAA GGT CA G-3'), were used to amplify COMT gene from human placenta cDNA library. The amplified COMT gene through 35 cycles of polymerase chain reaction was cloned into T-vector and the nucleotide sequences are determined by automatic sequencer. Human COMT has two common variants, a thermostable high activity form which has valine at amino acid 158 and a thermolabile low activity form which Has methionine at the same position. The cloned COMT gene in this study has both variants. Clone 4 and 5 have valine at amino acid 158 (guanine at nucleotide 472) and clone 1-3 have methionine at this site (adenine at nucleotide 472). In addition to this, there are other DNA polymorphisms in COMT gene at nucleotide 101, 102 (amino acid 34, a structural mutation cysteine/serine) and nucleotide 186 (a silent mutation). All of the clones have cysteine at amino acid 34 (guanine and adenine at nucleotides 101 and 102) and cytosine at nucleotide 186 except clone 3 which has thymine at this nucleotide. Clone 4 and 5 are thermostable high activity variants, suggesting being more useful for the measurement of catecholamines after expressing the gene.

        In order to calculate the radiation characterization of Black Body, Cr₂O₃, MnO₂, Fe₂O₃, CoO, NiO and CuO transition metal oxides for the Far Infrared radiator at 523K we calculated spectral radiant density(SRD,U), spectral radiant intensity(SRI,N), and spectral radiant emittance(SRE,W) which is expressed in terms of wavelength(λ), frequency(ν), wavenumber(?). Also, we investigated integral spectral radiant density(ISRD), percentage of integral spectral radiant density(PISRD), regional emissivity, and total emissivity. As a result, the spectral radiant density, Uλ, Uυ, U? which was the radiant energy functions with wavelength, frequency and wavenumber were different maxima in regions of the SRD spectrum. And the maximum of radiation function with wavelength was different from those with frequency and wavenumber. For wavelength(μm) the ISRD increased in the order Fe₂O₃〈MnO₂〈CoO〈NiO〈Cr₂O₃〈CuO oxide, the PISRD increased in the order CoO〈CuO〈NiO〈Fe₂O₃〈MnO₂〈Cr₂O₃ Oxide, the regional emissivity increased in the order MnO₂〈CoO〈Fe₂O₃〈MnO₂〈≒ Cr₂O₃〈 CuO oxide, and the total emissivity increased in the order Cr₂O₃≒Fe₂O₃〈MnO₂〈CoO〈NiO〈CuO oxide in the Far Infrared radiator at 523K.

      • Bacillus lkicheniformis IAM 11054가 生産하는 耐熱性 α- Amylase의 特性과 分解産物에 關한 硏究

        These experiments were performed to investigate the purification, the characteristics and the hydrolysis products on various Polysaccharides and oligosaccharides of thermophilic α-amylase produced by Bacillus licheniformis IAM 11054. The optimum culture time, the optimum initial pH and the optimum culture temperature for the production of α-amylase were 72hrs, pH 6.5 and 40℃, respectively. The crude enzyme was purified to 568- fold by starch adsorption, ammonium sulfate fractionation, DEAE-cellulose column chromatography and CM cellulose column chromatography. The specific activity of the purified enzyme was 312.5u /mg ㆍ protein and the yield of enzyme activity was 13.0%. The purified enzyme showed a single band (relative mobility 0.25) on polyacrylamide gel electrophoresis and the molecular weight was estimated to be approximately 145,000 by SDS-polyacrylamide gel electrophoresis, and isoelectric point was pH6.0. The optimum pH, the stable pH range, the optimum temperature and the stable temperature of the purified enzyme were pH6.0∼7.5, pH6.0∼10.5. 80℃ and below 75℃, respectively. The purified enzyme was greatly inhibited by Hg++ Ag+ and I2, and the thermostability of it was greatly increased by Ca++. The purified enzyme hydrolyzed soluble starch, corn starch, glutinous rice starch, green banana starch, arrow root starch, potato starch, yam starch, oyster glycogen, maltopentaose and maltohexaose, but did not hydrloyze α, β, γ -cyclodextrin, pullulan, maltose, maltotriose, maltotetraose, raffinose, lactose and sucrose. The purified enzyme hydrolyzed various starch to G1, G2, G3, G4, G5, G6 and others, and G5 to G2 and G3 and G6 to G1, G2, G3, G4 and G5. Among various gelatinized starches, potato starch and green banana starch were favorably hydrolyzed by the purified enzyme.

        The electronic states of the antimicrobial phenzine and its derivatives were examined by Extended H?uckel Method. It was found that the sum of nucleophilic reactivity index for trans 1,2, and 4 positions, ?? in a molecule was found to be related antimicrobial activity. The index, ??, should be applied to biological activity as well as antimicrobial activity.

      • Phellinus linteus 균사 배양물로부터 분리한 단백다당체 Kp의 항암활성

        A protein-polysaccaride fraction Kp(53.9% polysaccharide, 14.2% protein) was separated from the shake-cultured mycelia of a basidiomycetous fungus, Phellinus linteus, and its antitumor activity against sarcoma 180 in ICR mice was investigated. When administered after the tumor implantation. Kp exerted antitumor activity by inhibiting the growth of the sarcoma 180 solid tumor by 71.5% and increasing the life span of the sarcoma 180 ascitic mice by 51.5% at 100 ㎎/㎏. In pretreatment tests, in which Kp was administered once daily for 9 days before the tumor implantation. Kp inhibited the growth of the solid and ascites form of sarcoma 180, respectively, by 35.4% and by 80.3% at 100 ㎎/㎏. However, Kp showed no in vitro cytotoxic activity against a murine leukemia L1210 and a human gastric tumor SNU.1 upto the concentration of 200 ㎍/㎖. From these results, it is clear that the antitumor activity of Kp is exerted through its immunomodulating activity on the host's immune system.
