A case of cellulitis-like foreign body reaction after hyaluronic acid dermal filler injection

A 58-year-old female presented with 3 weeks history of painful skin lesion on the right cheek. The lesion was erythematous and edematous patch on the right cheek. Impression at first visit was cellulitis based on the clinical manifestation and mild elevated ESR and CRP levels in laboratory test. But her symptom and skin lesion were not improved with antibiotics so that biopsy was performed on her right cheek. Histologic examination showed diffuse inflammatory cells infiltration dominantly lymphocytes and some histiocytes. Multiple, clear vacuoles were remarkably observed forming “Swiss cheese” appearance, which is the histologic feature of foreign body reaction. Based on these findings, additional information of previous intradermal filler injection on both cheeks was obtained from the patient after biopsy. Since the treatment with oral antibiotics, systemic steroid, intralesional steroid and hyaluronidase injection, her cutaneous lesion and symptom have been improved. Previously reported foreign body reactions induced by hyaluronic acid dermal filler were typically manifested as nodular lesions at injection site, but cellulitis-like cutaneous manifestation as seen in this case has not been reported. Therefore, we report this interesting case of cellulitis-like foreign body reaction after hyaluronic acid dermal filler.
Chemical leukoderma of the scalp caused by the hair dye

Chemical leukoderma is defined as an acquired, hypopigmented dermatosis induced by repeated exposure to specific chemical compounds that destroys epidermal melanocytes. Early diagnosis of chemical leukoderma is crucial, as it could halt the progression of depigmentation unlike idiopathic vitiligo. Three Korean men presented with multiple, irregular, progressive hypopigmentated patches over their scalp after hair dying. Patch tests were conducted on the two patients, and both of them showed positive results to para-phenylenediamine(PPD). Histopathological evaluation from three patients revealed decreased melanin pigmentation in basal layer, and the patients were finally diagnosed as chemical leukoderma induced by hair dye. Although there is no formal diagnostic criteria about chemical leukoderma at present, clinical appearance, history, and histopathologic findings may be useful in distinguishing chemical leukoderma from other hypomelanotic diseases such as idiopathic vitiligo or post-inflammatory hypopigmentation. Chemical leukoderma occurring as a toxic effect has been reported secondarily to a number of substances, especially when the substances has benzene ring. The PPD also has a benzene ring in their chemical structure. There is only one case report of chemical leukoderma induced by PPD in Korean literature. Therefore, we herein report these three interesting cases of chemical leukoderma that developed after exposure to hair dye.

<P>Vertically conformal ZnO/CuO core–shell nanowire (NW) arrays with high aspect ratios were synthesized by electrophoresis-assisted electroless deposition (EAELD) of CuO onto preformed ZnO NWs. Using this method, conformal CuO seeds were successfully formed on long ZnO NWs by the electrostatic attachment of colloidal Cu<SUB>2</SUB>O NPs under the solution and subsequent thermal oxidation. Using the CuO seeds, conformal CuO shell to core ZnO NWs were successfully fabricated by kinetic-limited deposition in the solution, which overcame the inherent diffusion limitations of conventional electroless deposition. This experimental evidence and thermodynamic modeling results are presented to validate the proposed EAELD mechanism. The kinetics of low-temperature tempering in a dissimilar resistance spot weldment between medium Mn transformationinducedplasticity (MT) steel and dual-phase (DP) steel were investigated to interpret the change of failure mode after apaint-baking process. After the isothermal heat treatments with various soaking conditions, the Johnson–Mehl–Avrami (JMA)model describing nucleation and growth kinetics was adopted. The results indicated that the tempering kinetics parameterswere different for each area of the welds. Especially in the coarse-grained heat-affected zone (CGHAZ) in the MT steel,carbon diffusion from martensite to austenite acts as a tempering mechanism at an earlier stage of tempering. Thereafter,the tempering mechanism changes to epsilon carbide precipitation and coarsening. In comparison, the change was relativelyslow or absent in the DP steel CGHAZ and fusion zone because of its lower kinetics parameters. Based on the results ofthis study, ways to improve the cross-tension strength of entire welds in automotive applications of MT steel are discussed.

Kaposi’s sarcoma (KS) is a multifocal proliferative vascular neoplasm commonly associated with human immunodeficiency virus infection or immunosuppressive agent. Iatrogenic KS can be developed after long-term treatment with immunosuppressive agents for autoimmune diseases or after solid organ transplantation. A 69-year-old man presented with multiple purplish brown colored macules and patches on both lower legs for two months. Skin biopsy showed vascular proliferation with bland spindle cells in a dermis and immunohistochemical staining for CD31, D2-40 and human herpesvirus-8 were positive. He was diagnosed as KS based on the histologic findings. We noticed he had a 10-year history of nummular type psoriasis treated with several systemic immunosuppressive agents including cyclosporine, methylprednisolone and azathioprine. Therefore, it was assumed that long-term use of immunosuppressive drugs could be the cause of iatrogenic KS in this patient. The lesions were improved by cryotherapy in addition to dose-reduction of cyclosporine. To our knowledge, this is the first report of iatrogenic KS developed in psoriasis patient treated with immunosuppressive agents. Not only transplantation surgeon or rheumatologist but also dermatologist should be aware of the possible occurrence of iatrogenic KS when treating patients with immunosuppressive agents.
