To compare IOL powers which are calculated by post-op keratometric(K) values and refraction derived K values, and by refraction corrected method, respectively, in patients who have been treated with excimer laser photorefractive keratectomy(PRK) for mild to moderate myopia. A total of 38 consecutive eyes of 23 patients who were treated with PRK for mild to moderate myopia have been followed up for more than 6 months. IOL powers from SRK/T formula by post-op K values and refraction derived K values were compared with IOL powers by refraction corrected method. The mean values of IOL powers using post-op K values, refraction derived K values, and refraction corrected method were 20.69±0.93(18.21 to 22.93) diopter(D), 20.81±0.88 D(18.60 to 22.43), and 21.26±0.79 D(19.08 to 23.03) respectively. There was no significant statistical difference between the mean values of IOL powers using post-op K values and refration derived K values (p〉0.05). However, the mean value of IOL powers by refraction corrected method was statistically higher than other two values(p〈0.05). In patients with excimer laser PRK treatment for mild to moderate myopia, IOL power using refraction corrected method was statistically higher than IOL power using post-operative K values and refraction derived K values.
전치봉합이 실리콘기름 제거 후 일시적 초기 저안압에 미치는 영향

목적: 평면부를 통한 실리콘기름제거술 시 시행한 전치봉합이 실리콘기름 제거 후 일시적 초기 저안압에 미치는 영향에 대해 알아보고자 하였다. 대상과 방법: 2000년 1월부터 2011년 6월 사이에 실리콘기름 제거술을 받은 환자를 대상으로 실리콘기름 제거 전 관류관에 전치봉합을 시행한 경우(1군)와 전치봉합을 시행하지 않은 경우(2군)로 나누어 병록 기록을 후향적으로 조사하고, 실리콘기름 제거 후의 안압 변화를 살펴보았다. 실리콘기름 제거 후 가스 또는 공기를 주입한 경우는 안압과 상처 치유 등에 영향이 있을 수 있어 배제하였다. 일시적초기 저안압은 수술 1주 내 안압이 6 mmHg 미만의 저안압과 9 mmHg 미만의 저안압으로 분류하였다. 결과: 1군에는 118안, 2군에는 27안이 포함되었다. 안압 6 mmHg 미만의 저안압은 1군에서 5안(4.23%), 2군에서 3안(11.11%)이었으나 통계적 차이는 없었다(Fisher’s exact test, p=0.168). 안압 9 mmHg 미만의 저안압은 1군에서 13안(11.01%), 2군에서 7 안(25.92%)이었으며, 1군에서 유의하게 적었다(Chi-square, p=0.038). 결론: 실리콘기름 제거 전 전치봉합을 사용하여 초기 저안압을 낮출 수 있을 것으로 생각된다. Purpose: To report the effects of a preplaced suture on postoperative transient hypotony after pars plana silicone oil (SO) removal. Methods: We retrospectively reviewed the medical records of patients who underwent pars plana silicone oil removal between January 2000 and June 2011. The patients were divided into two groups: the preplaced suture group before SO removal (group 1) and the no preplaced suture group (group 2). The changes of intraocular pressure (IOP) after surgery were recorded. Patients who underwent tamponade of gas or air were excluded. Transient postoperative hypotony was defined as an IOP less than 6 mmHg, or less than 9 mmHg within 1 week after surgery. Results: 118 eyes and 27 eyes were included in group 1 and group 2, respectively. The number of eyes with transient hypotony according to an IOP less than 6 mmHg was 5 eyes in group 1 (4.23%) and 3 eyes in group 2 (11.11%). However, the difference was not statistically significant (Fisher’s exact test, p = 0.168). The number of eyes with transient hypotony according to an IOP less than 9 mmHg was 13 eyes in group 1 (11.01%) and 7 eyes in group 2 (11.11%), and the difference was statistically significant (Chi-square p = 0.038). Conclusions: A preplaced suture before SO removal could prevent the incidence of postoperative transient hypotony.
중심와를 침범한 감염성 포도막염에서 발생한 황반원공의 자연폐쇄 1예

Purpose: To report a case of macular hole secondary to presumptive infectious posterior uveitis involving the fovea that spontaneously resolved after medical treatment. Case summary: A 33-year-old male visited our clinic for decreased visual acuity in his left eye. He was treated with immunosuppressive therapy including steroid after bone marrow transplantation. Best corrected visual acuity (BCVA) was 0.05 in the left eye. Slit lamp examination showed mild anterior vitritis, and fundus examination showed a macular hole with surrounding whitish infiltration at the fovea. Spectral domain optical coherence tomography (SD-OCT) revealed a full thickness macular hole with surrounding hyper-reflective masses with an infiltration-like appearance involving all retinal layers. Serum anti-toxocara IgG was positive (ELISA), and eosinophil count and immunoglobulin E was elevated. Under diagnosis of presumptive ocular toxocariasis, the patient was treated with albendazole. After medical treatment for toxocariasis, the whitish foveal infiltration became smaller and more discrete. SD-OCT revealed spontaneous closure of the macular hole, and BCVA was improved to 0.4 after a 4-month follow-up. Conclusions: We report a macular hole complicated by presumptive infectious posterior uveitis that experienced spontaneous closure after medical treatment for underlying infection.

PURPOSE:: To analyze the changes in subfoveal choroidal thickness (SFChT) before and after resolution of central serous chorioretinopathy (CSC) and their association with recurrence during follow-up. METHODS:: Seventy-six eyes with CSC that were completely resolved after treatment with either intravitreal bevacizumab (IVB, 42 eyes) or with half-fluence photodynamic therapy (34 eyes) were included. Best-corrected visual acuity and spectral domain optical coherence tomography were performed at baseline, after complete resolution, and at regular intervals thereafter. RESULTS:: Subfoveal choroidal thickness was similar in the IVB-treated and half-fluence photodynamic therapy–treated eyes at baseline, as well as after complete resolution of the CSC. However, recurrence was more frequent in the IVB-treated eyes (19.0% vs. 2.9%, P = 0.037). The reduction of SFChT after CSC resolution was greater in the nonrecurrent eyes than in the recurrent eyes (91.35 ± 46.40 vs. 19.25 ± 16.47 μm, P < 0.001), and the extent of SFChT reduction was associated with the rate of recurrence of CSC (odds ratio = 0.877, P = 0.019). When CSC recurred, SFChT increased toward the baseline value. CONCLUSION:: Treatment of idiopathic CSC by both IVB and half-fluence photodynamic therapy can reduce SFChT when subretinal fluid is completely resolved. Recurrence is more frequent after IVB and specifically in eyes with a smaller reduction in SFChT after resolution of the CSC.

<P>The effect of metal particles on the photoluminescence (PL) and the Raman spectra of functionalized SWCNTs in aqueous solutions was systematically investigated by studying three different metal particles (gold, cobalt, and nickel) on three different SWCNT suspensions (DNA-, RNA-, and sodium deoxycholate salt (DOC)-functionalized SWCNTs). Substantial enhancement of the PL intensities was observed, while the Raman spectra remained unchanged, after gold, cobalt, or nickel particles were introduced into RNA-SWCNT aqueous suspensions. Almost the same results were obtained after the same metal particles were added to DNA-SWCNT aqueous suspensions. However, both the PL and the Raman spectra did not exhibit any change at all after the same metal particles were introduced into DOC-SWCNT aqueous suspensions. The unusual PL enhancements observed in this work cannot be accounted for by the three well-known mechanisms in the literature: surface-enhanced Raman scattering effect, Förster resonance energy transfer in a rebundling of isolated SWCNTs, and pH changes of the aqueous solutions.</P>
