In order to study the toxic effect of adriamycin on the formation of dentin of the rat, adriamycin(5㎎/㎏ body weight) was injected intravenously, and animals were sacrificed at 1, 3, and 10 days after administration. In the light micrographic observation, aggregation of densely stained cells were observed along the wall of the pulp canal and the odontoblastic layer disappeared after 1 and 3 days. The predentin was not even thickness and irregular in boundary compared to normal predentin. The formation of osteodentin, which has cellular inclusions in the matrix, was noted after 10 days. Electron microscopic observation revealed that the cells in aggregation had long slender cytoplasmic processes and contained more profiles of endoplasmic reticulum and secretory granules. The odontoblasts showed degenerative changes such as cytoplasmic vacuolation and fragmentation of rEr. The cells in the osteodentin and on the surface of irregular osteodentin contained dilated cisternae of rEr and secretory granules. Some cells showed necrotic and degenerative changes. There were no odontoblastic processes and dentinal tubules in the osteodentin matrix, central region of which was densely packed with collagen fibers. It therefore appears that osteodentin formation was observed in the pulp of root canal after adriamycin administration and it is the result of an abnormal differentiation of pulp mesenchymal cells.
Clinical and immunological characteristics of psoriasis patients with latent tuberculosis infection

Background: Latent tuberculosis infection (LTBI) is a growing concern for the treatment of psoriasis, as the use of biologics has recently increased. LTBI may also be associated with chronic inflammatory status that results in a change in immune status. Objectives: To investigate the association between the clinical comorbidities of psoriasis and the expressions of inflammatory cytokines induced by LTBI. Methods: We conducted a retrospective analysis for 300 patients with psoriasis using clinical information, including the comorbidities and presence of LTBI. Serum cytokine levels were measured for immunological analysis. Results: The prevalence rates of psoriatic arthritis (p = 0.001) and nail psoriasis (p = 0.014) were significantly higher in patients with LTBI than in those without LTBI. The serum levels of interleukin (IL) 6, IL-8, and IL-23A were greater in the LTBI-positive group than in the LTBI-negative group (p = 0.014 for IL-6, p = 0.025 for IL-8, and p = 0.004 for IL-23A). Conclusion: LTBI may be a risk factor for the development of clinical comorbidities in psoriatic patients through an increase in the levels of inflammatory cytokines, including IL-6, IL-8, and IL-23A.

The ERETIC (Electronic REference To access In vivo Concentrations)2 method is a new qNMR experimental technique to measure analytes based on the signal of the reference compound without additional hardware equipment. In this study, ERETIC2 method was validated, and we sought to identify whether it would be possible to apply this method to a specific compound analysis of metabolites in plant. The $90^{\circ}$ pulse value (P1) and spin-lattice relaxation time ($T_1$) of each compound were measured for ERETIC2. The $9^1H$ of 3-(trimethylsilyl) propionic-2,2,3,3-$d_4$ acid (TSP) was used as a reference peak for ERETIC 2, and then, a suitable solvent and pulse sequence for each compound were selected. Under the NOESY-presat sequence, the relative accuracy error for quantitative analyses of primary metabolites was within the range of 5%, with the exception of glucose, which showed ${\geq}$ 55% error due to saturation. It showed excellent results for the quantification of glucose by using a $30^{\circ}$ pulse sequence, which did not suppress the water peak. In addition, the quantitative accuracy for secondary metabolites was extremely accurate, with an error ${\leq}$5% when considering the purity of the standard sample. The ERETIC2 method showed outstanding linearity, precision, and accuracy.
A case of acral persistent papular mucinosis treated with carbon dioxide laser

Acral persistent papular mucinosis (APPM) is a rare subtype of localized lichen myxedematosus. The lesions of APPM are limited to the skin and usually do not require treatment, except for the cosmetic concern. There is no specific treatment of APPM. Topical and intralesional corticosteroids, topical tacrolimus, and laser therapy have been used with variable success. A 63-year-old woman presented with a 10-year history of 2-5mm sized, multiple, discrete, and skin-colored papules on the extensor aspects of her distal forearms. The lesions were asymptomatic and had increased slowly in number. Her medical history was significant only for diabetes and dyslipidemia. Mucin deposition in the upper and mid dermis without epidermal involvement was shown on histopathologic examination. On the diagnosis of APPM, we treated the patient with carbon dioxide laser for some of the lesions which responded well but left behind mild scars. Herein, we report a rare case of APPM treated by carbon dioxide laser. This report would be helpful to suggest a potential treatment option for APPM patients who desire therapy.
A case of malignant melanoma with atypical feature previously treated by laser therapy

Laser therapy of pigmented lesions is a commonly performed procedure. However, it should be performed prudently as it can complicate the diagnosis and lead to the diagnosis delay. If there is any doubt whether the pigmented lesion is benign, then a biopsy for histologic evaluation is obligatory. A 44- year-old woman visited our clinic with a coin sized, dark bluish-red, and protruding tumor on her thigh. The patient reported that a pigmented lesion had been treated with laser at a local clinic a year ago without prior histological assessment. A skin biopsy was performed at our clinic, and the biopsy confirmed the diagnosis of malignant melanoma. 18F-FDG PET/CT showed no evidence of distant metastases. Herein, we report a case of malignant melanoma presenting after laser therapy of a pigmented lesion. This case emphasizes the importance of careful examination before the laser therapy, and the value of histopathologic review for accurate diagnosis and treatment.

Reactive angioendotheliomatosis (RAE) is a rare benign angioproliferative condition of the skin. RAE manifests as diverse and nonspecific cutaneous lesions such as macules, papules, plaques and ulceration. Although the pathogenesis has not been fully elucidated, various diseases that cause vascular occlusion and hypoxemia are related to RAE. However, RAE after varicose vein surgery is rarely reported. A 26-year-old man visited our clinic with a 2-year history of painful skin lesions affecting the left lower limb. The lesions were poorly delimited, with deep purple colored plaques and macules. He had a varicose vein surgery by unlicensed non-medical personnel 5 years ago. Prominent proliferation of endothelial cells within the lumina of dermal vessels was observed in histologic examination. Immunohistochemistry showed that endothelial cells stained positively for CD31 and CD34. On the basis of the above findings, the patient was diagnosed with RAE. We referred the patient to rehabilitation center in order to alleviate the chronic leg edema worsening after varicose vein surgery.

Parapsoriasis en plaque (PSEP) belongs to the cutaneous T-cell lymphoproliferative disorder. PSEP is clinically benign and typically presents as slightly scaly red-brown patches. However, when PSEP presented as a rare hypopigmented lesions, it can be easily be misdiagnosed as other diseases such as vitiligo, pityriasis alba, or hypopigmented mycosis fungoides. Therefore, suspicious skin lesions should be biopsied to make appropriate diagnosis. A 5-year-old girl visited our clinic with hypopigmented macules and patches developing gradually over the past four years. The lesions were covered with scanty scales. Superficial perivascular infiltrate of lymphocytes and histiocytes as a band-like pattern were observed in histologic examination. Immunohistochemistry showed that T-cell lymphocyte stained positively for CD3, whereas T-cell receptor gamma gene rearrangement study was negative in the clonal population. On the basis of the above findings, a diagnosis of PSEP was made. The hypopigmented lesions were improved after 308-nm excimer laser twice a week for 12 weeks. We herein report a 5-year-old child with hypopigmented PSEP lesions who has been successfully treated with 308-nm excimer laser.

Background: H1-antihistamines have been used widely to treat pruritus in atopic dermatitis (AD). Although the evidence for H1-antihistamine monotherapy for pruritus in AD is inconclusive, combination therapy with topical glucocorticoids are particularly considered as to be effective as both short-term intensive therapy and maintenance treatment in patients with frequent relapse. Objectives: We conducted a meta-analysis for confirming the efficacy of antihistamine as an adjunct to topical corticosteroid application in the treatment of AD. Methods: We identified 1,206 studies through systematically searching Medline, Embase, and CENTRAL databases for articles published from 1967 to 2015. Random effects meta-analysis was used to calculate mean differences (MD) with 95% confidence intervals (CI). Results: Two studies satisfying the inclusion criteria of antihistamine therapy with mandatory topical steroid use were selected. Comparing antihistamine monotherapy with combination therapy, patients treated with the addition of antihistamine to topical corticosteroids showed a statistically significant clinical improvement (standard MD, -0.24; 95% CI, -0.42 to -0.05; p = 0.01). Conclusion: This meta-analysis based on two studies suggests that antihistamine therapy as an adjunctive therapy for pruritus in AD may have beneficial effects in combination with topical corticosteroid treatment.

Background: Cyclosporin A (CsA) is an immune modulating agent in treating psoriasis. Considering its nephrotoxicity, however, serum creatinine (SCr) levels should be monitored carefully. Objectives: The objective of this study is to evaluate the safety of CsA and risk factors associated with a renal impairment in treatment of psoriasis with CsA. Methods: The 284 patients with psoriasis treated with CsA between January 2011 and December 2016 were analyzed. Results: The median duration of treatment was 10 weeks (interquartile range: 4-21) and the mean dose of CsA was 3.14±0.38 mg/kg/day. Although there was a significant increase of SCr levels after CsA treatment (P = 0.01), the overall change was largely reversible on cessation of the drug. More than 25% increase of SCr above baseline was reported in 17 patients (6.0%). Comparing those with increased SCr to the others, there was no significant difference in sex, body mass index, psoriasis area and severity index, disease duration, mean dose of CsA except the age (P = 0.005). Patients older than 60 years resulted at higher risk for a rise in SCr levels when compared with younger patients (P < 0.001). The treatment duration more than a year and hypertension also found to be significantly related to the SCr elevation. Conclusion: Careful SCr monitoring is needed in patients who have factors that might increase the SCr levels, such as old age, treatment duration more than a year and the hypertension.
