        낙하 착지 시 FRT가 하지의 관절의 시상각과 강직도에 미치는 효과

        Objective: To investigate effects of Fibular Repositioning Taping (FRT) on lower extremity joint stiffness and angle during drop-landing. Method: Twenty-eight participants (14 healthy, 14 with chronic ankle instability [CAI]) performed droplandings from a 60 cm box; three were performed prior to tape application and three were performed post-FRT. Three-dimensional kinematic and kinetic data were collected using an infrared optical camera system (Vicon Motion Systems Ltd. Oxford, UK) and force-plate (AMTI, Watertown, MA). Joint stiffness and sagittal angle of the ankle, knee, and hip were analyzed. Results: The hip [Healthy: p<.05; M ± SD: 29.43 ± 11.27 (pre), 33.04 ± 12.03 (post); CAI: p<.05; M ± SD: 31.45 ± 9.70 (pre), 32.29 ± 9.85 (post)] and knee [Healthy: p<.05; M ± SD: 53.44 ± 8.09 (pre), 55.13 ± 8.36 (post); CAI: p<.05; M ± SD: 53.12 ± 8.35 (pre), 55.55 ± 9.81 (post)] joints demonstrated significant increases in sagittal angle after FRT. A significant decrease in joint angle was found at the ankle [Healthy: p<.05; M ± SD: 56.10 ± 3.71 (pre), 54.09 ± 4.31 (post); CAI: p<.05; M ± SD: 52.80 ± 6.04 (pre), 49.86 ± 10.08 (post)]. A significant decrease in hip [Healthy: p<.05; M ± SD: 1549.16 ± 517.53 (pre), 1272.48 ± 646.73 (post); CAI: p<.05; M ± SD: 1300.42 ± 595.55 (pre), 1158.27 ± 550.58 (post)] and knee [Healthy: p<.05; M ± SD: 270.12 ± 54.07 (pre), 239.13 ± 64.70 (post); CAI: p< .05; M ± SD: 241.58 ± 93.48 (pre), 214.63 ± 101.00 (post)] joint stiffness was found post-FRT application, while no difference was found at the ankle [Healthy: p >.05; M ± SD: 57.29 ± 17.04 (pre), 59.37 ± 18.30 (post); CAI: p>.05; M ± SD: 69.15 ± 17.63 (pre), 77.24 ± 35.05 (post)]. Conclusion FRT application decreased joint angle at the ankle without altering ankle joint stiffness. In contrast, decreased joint stiffness and increased joint angle was found at the hip and knee following FRT. Thus, participants utilize an altered shock absorption mechanism during drop-landings following FRT. When compared to previous research, the joint kinematics and stiffness of the lower extremity appear to be different following FRT versus traditional ankle taping.

        Correlation between Proprioception and Dynamic Postural Control during Ovulatory Phase of the Menstrual Cycle

        Decreased proprioception and dynamic postural control (DPC) have been reported to be a contributing factor in anterior cruciate ligament (ACL) injuries in females. Understanding the relationship between proprioception and DPC during different phases of the menstrual cycle may provide a better understanding of gender-specific characteristics of ACL injury risk. The primary purpose of this study was to assess the relationship between DPC and both joint kinesthesia (JK) and joint position sense (JPS) in females during the ovulatory phase of the menstrual cycle. Nine moderate to highly active females. Subjects made two visits to the research facility over a two-month period. DPC, JK, and JPS were measured over two consecutive menstrual cycles during ovulatory phase (24-48 hours after LH surge detection). Joint position sense was tested using a starting angle of 90° when moving into extension, and 0° when moving into flexion. Target angles were 30°, 50°, and 70° moving into flexion, and 20°, 40°, and 60° moving into extension. Joint kinesthesia was tested moving into flexion and extension at both 15° and 45°. Moderate to high correlations were observed on six of 60 variables in the ovulatory phase of the first menstrual cycle evaluated. Although a few statistically significant correlations were identified, the clinicians may focus on those meaningful variables when they plan the rehabilitation program for prevent ACL injury. Also, while DPC deficits may increase risk of ACL injury, the data does not support that reduced DPC is due to menstrual phase-specific deficits in proprioception.

        Rearfoot and Tibial Motion during Gait Associated with Medial Tibial Stress Syndrome: A Systematic Review

        PURPOSE: The purpose of this study was to confirm the kinematic characteristics of overuse leg injuries during gait through a systematic review. METHODS: Electronic databases from 1960 to 2018 were used to search studies. Physiotherapy Evidence Database Scale was used for quality of studies. Effect sizes (ES) of between-groups and its 95% confidence intervals were calculated. RESULTS: Seven studies were selected for review. The average Physiotherapy Evidence Database Scale score was 4.3 of 10-point scale. The ES for maximum rearfoot eversion from 5 studies ranged from -0.34 to 0.67 with 3 of 5 studies statistically significant. The ES of eversion excursion from 4 studies ranged from -0.24 to 2.37 and 3 of 4 studies showed statistically significant. The ES of maximum eversion velocity ranged from 0.29 to 0.38 from all of 2 studies. With abduction, the ES for abduction excursion ranged from -0.04 to 0.35, and 2 of 3 studies revealed statistically significant. The ES of maximum abduction velocity showed statistically significant with the ES of 0.23 and 0.61. However, maximum abduction from 3 studies showed non-significant with ES ranges from 0.00 to 0.62. CONCLUSIONS: The rearfoot motion (eversion) associates with overuse leg injuries during gait more than tibial motion (internal rotation).

        Deletion of Chitinase-3-like 1 accelerates stroke development through enhancement of Neuroinflammation by STAT6-dependent M2 microglial inactivation in Chitinase-3-like 1 knockout mice

        <P><B>Abstract</B></P> <P>Chitinase 3-like 1 (Chi3L1) plays a major role in the pathogenesis of inflammatory diseases. We investigated the effect of Chi3L1 knockout on stroke development. Ischemia/reperfusion was induced by middle cerebral artery occlusion (MCAO) in Chi3L1 knockout and wildtype mice. Significantly increased infarct volume and decreased neurological deficit scores at 24 h after ischemia/reperfusion were found in Chi3L1 knockout mice compared to wildtype mice. Moreover, ischemic neuronal cell death was increased in Chi3L1 knockout mice through increased oxidative stress and release of IL-6 and IL-1β but IL-10 and IL-4 were reduced. Furthermore, expression of inflammation-related proteins (iNOS, COX-2, Iba-1, and GFAP) was significantly increased in Chi3L1 knockout mice compared to wildtype. In microglia isolated from MCAO-injured Chi3L1 knockout mice, expression of M1 markers (iNOS, CD86, IL-1β, and IL-6) was increased and M2 markers (Arg1, Mrc1, IL-10, and IL-4Ra) was decreased. In BV-2 cells, knockdown of Chi3L1 increased TNF-α- and INF-γ-induced expression of iNOS, COX-2, and Iba-1, but decreased the expression of Arg1, MRC1, and IL-4 receptor-alpha (IL-4Rα). Expression of IL-4Rα, an important factor of M2 polarization, and its downstream signals p-JAK1, p-JAK3, and p-STAT6, was much reduced in the knockout mice. Additionally, in BV-2 cells, knockdown of Chi3L1 by siRNA Chi3L1 decreased rhTNF-α- and INF-γ-induced expression of IL-4Rα, p-JAK1, p-JAK3, and p-STAT6. Furthermore, treatment with AS1517499 abolished Chi3L1 knockdown-induced reduced IL-4Rα and Arg1 but not CD86 expression. Our results indicate that deletion of Chi3L1 accelerates stroke development through enhancement of neuroinflammation by markedly decreasing STAT6-dependent M2 macrophage polarization.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Chi3L1 is important for IL-4Rα expression and phosphorylation of STAT6, affecting micorglial M2 polarization. </LI> <LI> Knockdown of Chi3L1 decrease IL-4Rα, p-JAK1, p-JAK3, and p-STAT6 expression level which blocking microglial M2 polarization. </LI> <LI> Absence of Chi3L1 accelerates stroke development through STAT6 dependent M2 microglia inactivation. </LI> </UL> </P>

        Updates on the Sedation for Gastrointestinal Endoscopy

        Sedation, defined as the depressed level of consciousness, induced by drug administration, is widely used for gastrointestinal endoscopyto relieve a patient’s anxiety and discomfort. In addition, successful procedure is anticipated with control of unintended movements. Endoscopic sedation, however, cannot be free from the risk of serious adverse events, e.g., cardiopulmonary compromise. Therefore,principles on personnel, facility and equipment, as well as performance itself, should be followed to prevent unfavorable incidents. Inthis article, sedation guidelines for the Accreditation of Qualified Endoscopy Units, issued by the Korean Society of GastrointestinalEndoscopy, are presented.

        Atrophic mandible fracture due to dental implants: Case report

        Implant placement in an edentulous area is a common restorative approach. However, in atrophic posterior mandible cases, the low bone level and the presence of the inferior alveolar nerve can introduce many limitations, along with complications such as mandibular fractures, dysesthesia, and osteomyelitis. This case report discusses an elderly patient who underwent mandibular fracture that was accompanied by osteomyelitis after multiple standard length implant placements in an atrophic mandible. In such cases, clinicians should be aware of the high risk of mandibular fractures and therefore recommended to place short implants without compensating with wide implants. Despite the efforts when mandible fractures accompanied by osteomyelitis occurs, it is crucial that timely reduction and active surgical treatment for osteomyelitis along with long-term antibiotic treatment is done in order to avoid further bone loss.

        Asymmetry analysis of deformable hippocampal model using the principal component in schizophrenia

        <P>The hippocampus is thought to play an important role in learning and memory processing, and impairments in memory, attention, and decision making are found commonly in schizophrenia. Although many studies have reported decreases in hippocampal volume in the left hemisphere in schizophrenia, regionally specific hippocampal volume loss has not been revealed consistently using volume analysis. Recently, many studies have analyzed shape asymmetry using 3-D models; however, inconsistent results have been reported, mainly due to methodologic differences. We therefore used an active, flexible, deformable shape model for surface parameterization, and compared shape asymmetry based on principal component analysis (PCA) in the hippocampi of schizophrenic patients with those of the normal controls. Although the overall pattern of the statistical results did not change according to the number of principal components, the reconstructed results based on six major components were much more distinguishable. Although the left hemispheric hippocampal volume was larger than the right hemispheric was in this study, the difference was not significant. In shape asymmetry analysis, the right hemisphere hippocampus was bilaterally larger than the left hemisphere hippocampus was in the head of the superior CA1 and smaller in the tail and head of the inferior CA1. The asymmetry in the schizophrenia group was statistically smaller than that in the control group through reduction of the left hemisphere hippocampus volume.         Hepatic portal venous gas (HPVG), a rare radiologic finding, is associated in some cases with severe or lethal conditions requiring urgent surgical intervention. Computed tomography has recently demonstrated a wider range of clinical conditions associated with HPVG, some of which are benign and do not necessarily require surgery. However, HPVG remains an ominous sign in cases of bowel ischemia or necrosis. We report on a case of massive HPVG caused by a fatal intestinal infarction, which showed rapid disease progression, eventually resulting in death due to septic shock. The HPVG in this case was huge and could therefore be confused with an air-biliarygram. (Intest Res 2011;9:153-157)

        A sapphire single crystal growth process by the Vertical-Horizontal Gradient Freezing (VHGF) method was studied by a numerical analysis technique. The heater power was controlled in order to follow the given arbitrary target heater temperature profile through the whole series of the process. Due to the influence of radiation heat transfer, the heater had spatial temperature deviation in spite of the fact that the heater power was imposed uniformly through its whole body. As a structural feature of the VHGF method, temperature at the lower part of crucible was lower than that at the upper part of crucible because it was cooler at the bottom of the furnace. Thus, the movement of the solid/liquid interface was driven in the bottom-to-top direction. (Received February 4, 2014)

        The Adequacy Analysis for Installation of High Speed Grounding Switches on the Korean 765kV Single Transmission Line

        This paper analyzes arc phenomena including the secondary arcs elongation in theKorean 765kV single transmission line (79km) between Sin-Ansung S/S and Sin-Gapyeong S/S, which dependent line models are compared to make our final decisions. As wel, the significant simulation results are investigated by the EMTP program. As a result, there is no need of HSGS in the Korean 765kV single transmission line from both a practical and an economical point of view.
