        세균독소로 자극시킨 사람 섬유아 세포에서의 Interleukin-6와 Interleukin-8의 생성

        Cytokines are hormone-like proteins which mediate and regulate inflammatory and immune responses. Interleukin-6 (IL-6) is involved in the final differentiation of B cells into antibody-producing cells. Interleukin-8 (IL-8) is a neutrophil chemotactic factor that plays an important role in the recruitment of neutrophil to inflammatory loci. Inflammatory mediators by cells in the gingiva have been implicated in the initiation and progression of periodontitis and oral infection. The purpose of this study was conducted to investigate the effect of lipopolysaccharide (LPS), staphylococcus enterotoxin B (SEB) on production ofIL-6 and IL-8 by human gingival and facial dermal fibroblasts. Primary cultured human gingival and facial dermal fibroblasts were incubated with LPS(0.01, 0.1, 1.0㎍/ml), SEB (0.01, 0.1, 1.0㎍/ml) or LPS (0.1㎍/ml) plus SEB (0.1㎍/ml). Culture supernatants were collected at 24, 48, and 72 hrs and assessed for IL-6 and IL-8 production by enzyme-linked immunosorbent assay. IL-6 production in gingival fibroblasts stimulated with LPS was higher than that with SEB. IL-6 production by double exposure with LPS plus SEB was amplified in comparison with single exposure of LPS or SEB. IL-6 production in facial demal fibroblasts was increased only by stimulation with a high concertration of LPS (1.0㎍/ml). Its production in facial dermal fibroblasts by exposure with SEB was decreased in comparison with control, nontreated cells. Therefore, gingival fibroblasts showed higher sensitivity than facial dermal fibroblasts in response to low concentration of LPS. Also, IL-6 production by double exposure with LPS plus SEB was amplified in comparison with single exposure of LPS or SEB.IL-6 production in facial dermal fibroblasts was increased only by stimulation with a high concentration of LPS(1.0㎍/ml). Its production in facial dermal fibroblasts by exposure with SEB was decreased in comparison with control, nontreated cells. Therefore, gingival fibroblasts showed higher sensitvity than facial dermal fibroblasts in response to low concentration of LPS. Also, IL-6 production by double exposure with LPS plus SEB was amplified in comparison with single exposure of LPS or SEB. IL-8 production in gingival fibroblasts was enhanced greatly only by stimulation of high concentration of LPS (1.0㎍/ml). That by exposure with SEB was increased only in 24 hrs cultivation. IL-8 production by double exposure with LPS plus SEB was amplified in comparison with single exposure of LPS or SEB. IL-8 production in facial dermal fibroblasts was decreased by LPS and increased only in 48 hrs cultivation by SEB. IL-8 production by double exposure with LPS plus SEB was enhanced only in 48 hrs cultivation in comparison with single exposure of LPS or SEB. therefore, IL-6 and IL-8 production were released at various quantities according to bacterial toxin applied and site of fibroblast harvested. These results suggest that gingival fibroblasts may be concerned with IL-6 and IL-8 related inflammatory response more than facial dermal fibroblasts.

        Shaphylococcus enterotoxin B와 lipopolysaccharide를 작용시킨 사람 섬유아 세포에서 생성된 Transforming Growth Factor-β₁의 정량적 분석

        TGF-β1 is a potent chemotactic factor for inflammatory cells and fibroblasts. It also stimulates the celluar source and components of extracellular matrix and the production of proteinase inhibitors. Collectively, these biologic activities lead to the accumulation and stabilization of the nascent matrix, which is vital to infection control. The objective of this study is to investigate production of TGF-β in vitro fibroblast culture in the presence of Staphylococcus enterotoxin B(SEB) and/or lipopolysaccharide(LPS) and to elucidate the role of TGF-β1 which may be responsible for infection control. The fibroblasts were originated from gingiva and facial dermis in 26 year-old male patient. In the presence of LPS(0.01㎍, 0.1㎍, 1.0㎍), SEB(0.01㎍, 0.l㎍, 1.0㎍) respectively, cells(5×103ml) were cultivated in vitro. At 1, 3, and 5 days after incubation, cells were counted. Also, cells(2.5×105ml) were cultivated in EMEM with LPS(0.01, 0.1 and 1.0㎍), SEB(0.01, 0.1 and 1.0㎍) respectively and LPS(0.1㎍) and SEB (0.1㎍) in combination for 24,48, and 72 hours respectively. Culture supernatants were harvested at 1, 2, and 3 days after incubation period and triplicate culture supernatants were pooled and TGF-β1 was assayed in duplicate. The results were as follows. 1.In gingival fibroblast induced with SEB and LPS respectively or in combination, the suppression of cell Proliferation occurred very significantly since 3 days after incubation, compared with the control and the production of TGF-β1 occurred very significantly at 1 day after incubation, compared with the control. 2.In facial dermal fibroblast induced with SEB and LPS respectively or in combination. the suppression of cell proliferation occurred very significantly at 1 day after incubation, compared with the control. In SEB exposure, the production of TGF-β1 was decreased very significantly at 1 day after incubation, compared with the control. However, in LPS, SEB and LPS exposure, the production of TGF-β1 was increased very significantly at 1 day after incubation, compared with the control. In conclusion, the concentration of bacterial toxins and the incubation period correlated with cell proliferation and production of TGF-β1 very significantly. The gingival and facial dermal fibroblasts have different phenotype each other The orchestrated understanding of fibroblast proliferation and TGF-β1 production play an important part in host defense against the bacterial Infection and may prevent tissue necrosis such as necrotizing fasciitis and life-threatening syndrome such as multiple organ failure.

        Acute mesenteric venous thrombosis (MVT) is an uncommon form of intestinal ischemia with high mortality and usually occurs in the setting of preexisting comorbidities including thrombophilia and abdominal inflammatory conditions. Hyperhomocysteinemia has been known to be a risk factor for thromboembolism, often located on an unusual site. Considering that homocysteine metabolism is determined genetically to a high degree, a mutant of methylenetetrahydrofolate reductase (MTHFR) C677T causes hyperhomocysteinemia, leading to thrombophilia. Until now, there have been few reports of MVT associated with MTHFR gene mutation. We, herein, report a case of small bowel infarction associated with MVT by MTHFR gene mutation in an adult without any other risk factors of thrombophilia.

        Two new benzophenones, acredinones A(1) and B(2), were isolated from a marine-sponge-associated Acre-monium sp. Fungus. Their chemical structures were elucidated on the interpretation of spectroscopic data. The structure of 1 was confirmed by palladium-catalyzed hydrogenation, followed by spectroscopic data analysis. Acredinones A (1) and B (2) inhibited the outward K currents of the insulin secreting cell line INS-1 with IC50 values of 0.59 and 1.0 um, respectively.

        쯔쯔가무시병 환자에서의 장내 단백 상실

        Objectives: Tsutsugamushi disease is an acute, febrile illness of humans that is caused by Rickettsia tsutsugamushi. Hypoalbuminemia was reported in some cases of tsutsugamushi disease, but its frequcncy and etiology were not clarified. Therefore we prospectively evaluated fourteen cases of Tsutsugamushi disease patients to idendify intestinal protein loss as a cause of hypoalbuminemia and to assess the value of 99mTc-HSA abdominal scintigraphy for detecting intestinal protein loss. Methods: From November 1994 to December 1994, in fourteen patients with tsutsugamushi disease, serum albumin level and fecal a-1-antitrysin excretion were measured at admission. Anterior abdominal images were obtained at 2, 4, and 24hours after intravenous injection of 30 mCi of Tc-Human serum albumin. Results: 1) The age distribution was mainly in the forties, and fifties. Geographically, 10 cases in the northern part of Kyung-Ki Do and 4 cases in Seoul were occurred. 2) Laboratory findings showed hypoalbuminemia in 5 cases(35.7%), elevated AST and ALT in 11 cases (78.6%), prolonged prothrombin time in 1 cases (7.1%), and proteinuria(30mg/dl) in 9 cases(64.3%). 3) Fecal a-1-antitrypsin concentration was measured in 10 cases of 14 patients with tsutsugamushi disease, of which 8 cases revealed abnormally elevated concentration of fecal a-l-antitrypsin(2.6mg/g dry weight). 4) 99mTc-HSA scintigraphy was positive in 11 of 1478.696) patients with tsutsugamushi disease. 5) In patients with positive 99mTc-HSA scintigraphy, protein loss sites were small bowel(6/11, 54.5%), descending colon(3/14, 27.3%), ascending colon(2/11, 18.1%). Conclusion: Intestinal protein loss was idendified in some patients with tsutsugamushi disease by the measurement of fecal a-1-antitrpysin concentration. 99mTc-HSA scintigraphy is easy and readily available study for detecting intestinal protein loss sites. The comparison of the severity of intestinal protein loss before and after treatment is recommended in patients with tsutsugarnushi disease who have abnormal intestinal protein loss.

        Usefulness of Endoscopic Hill Grade in Evaluating Children Suspected of Having Gastroesophageal Reflux Disease

        Background/Aims This study aim to evaluate the relationship between the Hill grade confirmed by esophagogastroduodenoscopy (EGD) and the degree of gastroesophageal reflux (GER) by 24-hour multichannel intraluminal impedance-pH monitoring (MII-pH) in children suspected of having gastroesophageal reflux disease (GERD). Methods A retrospective review of 105 children and adolescents who underwent EGD and MII-pH for the evaluation of GERD from March 2013 to July 2019 was performed. Clinical features and results of EGD and 24-hour MII-pH were collected and statistically analyzed. Results Hill grades 1, 2, 3, and 4 were identified using EGD in 56 (53.3%), 22 (22.0%), 16 (15.2%), and 11 (10.5%) patients, respectively. As the Hill grade increased, the proportion of neurological diseases (P < 0.001) and endoscopic erosive esophagitis (P < 0.001) increased significantly. The acid exposure index, bolus exposure index, number of reflux episodes, and number of GER reaching proximal extent on MII-pH increased significantly as the endoscopic Hill grade increased (all P < 0.001). Linear regression analysis revealed an increase in the Hill grade by 1 increased the acid exposure index by 2.0%, bolus exposure index by 0.7%, number of reflux episodes by 18.9 episodes, and the number of GER reaching the proximal esophagus increased by 10.5 episodes on average (all P < 0.001). Conclusions Hill grade on EGD was associated with GER in children. Estimating the degree of GER by applying Hill grade in the retroflexion view may be useful in practice when evaluating children suspected with GERD. (J Neurogastroenterol Motil 2021;27:191-197)

        Differential Gene Expression in GPR40-Overexpressing Pancreatic β-cells Treated with Linoleic Acid

        “G protein-coupled receptor 40” (GPR40), a receptor for long-chain fatty acids, mediates the stimulation of glucose-induced insulin secretion. We examined the profiles of differential gene expression in GPR40-activated cells treated with linoleic acid, and finally predicted the integral pathways of the cellular mechanism of GPR40-mediated insulinotropic effects. After constructing a GPR40-overexpressing stable cell line (RIN-40) from the rat pancreatic β-cell line RIN-5f, we determined the gene expression profiles of RIN-5f and RIN-40. In total, 1004 genes, the expression of which was altered at least twofold, were selected in RIN-5f versus RIN-40. Moreover, the differential genetic profiles were investigated in RIN-40 cells treated with 30 μM linoleic acid, which resulted in selection of 93 genes in RIN-40 versus RIN-40 treated with linoleic acid. Based on the Kyoto Encyclopedia of Genes and Genomes Pathway (KEGG, http://www.genome.jp/kegg/), sets of genes induced differentially by treatment with linoleic acid in RIN-40 cells were found to be related to mitogen-activated protein (MAP) kinase- and neuroactive ligand-receptor interaction pathways. A gene ontology (GO) study revealed that more than 30% of the genes were associated with signal transduction and cell proliferation. Thus, this study elucidated a gene expression pattern relevant to the signal pathways that are regulated by GPR40 activation during the acute period. Together, these findings increase our mechanistic understanding of endogenous molecules associated with GPR40 function, and provide information useful for identification of a target for the management of type 2 diabetes mellitus.

        The Insulin Resistance but Not the Insulin Secretion Parameters Have Changed in the Korean Population during the Last Decade

        <P><B>Background</B></P><P>This study aimed to compare the patterns of insulin secretion and resistance between Korean subjects in the 1990s and 2000s.</P><P><B>Methods</B></P><P>Insulin secretion and resistance indices were calculated from subjects who underwent 75-g oral glucose tolerance tests in the year 1997 to 1999 and 2007 to 2011 at the Seoul St. Mary's Hospital, Korea.</P><P><B>Results</B></P><P>A total of 578 subjects from the 1990s (mean age, 48.5 years) and 504 subjects from the 2000s (mean age, 50.2 years) were enrolled. Compared with the subjects from the 1990s, those from the 2000s exhibited increased insulin resistance (increased homeostatic model assessment for insulin resistance), and reduced insulin sensitivity (reduced Matsuda index and quantitative insulin sensitivity check index), regardless of their glucose tolerance status. However, insulinogenic index did not reveal significant differences between the 2 decades in subjects with or without diabetes. A distinct relationship was confirmed between Matsuda index and total area under the curve (insulin/glucose) in each glucose tolerance group. The mean product of the Matsuda index and the total area under the curve (insulin/glucose) as well as the oral disposition index, was lower in subjects with normal glucose tolerance from the 2000s than in those from the 1990s.</P><P><B>Conclusion</B></P><P>After rapid economic growth and changes in lifestyle patterns, insulin resistance has worsened across the glucose tolerance status; however, the insulin secretory function remained unchanged, which resulted in an increase in the susceptibility to the development of type 2 diabetes mellitus among Korean subjects without diabetes. We could not rule out the potential selection bias and therefore, further studies in general Korean population are needed.</P>

      • 데이터 웨어하우스에서 클러스터링 기법을 이용한 실체화 뷰 선택 알고리즘

        In order to acquire the precise and fast response for an analytical query, proper selection of the views to materialize in data warehouse is very crucial. In traditional view selection algorithms, the whole relations are considered to be selected as materialized views. However, materializing the whole relations rather than a part of relations results in much worse performance in terms of time and space cost. Therefore, we present an improved algorithm for selection of views to materialize using clustering method to overcome the problem resulted from conventional view selection algorithms. In the presented algorithm, ASVMRT(Algorithm for Selection of Views to daterialize using Iteduced Table). we first generate reduced tables in clata warehouse using automatic clustering based on attrihute-values density, then we consider the combination of reduced tables as materialized views instead of the combination of the original hase relations. For the justification of the proposecl algorithm. we show the experimental results in which both time and space cost are approximately 1.8 times better than the conventional algorithms. 데이터 웨어하우스에서 실체화 할 뷰들을 알맞게 선택하는 것은 분석적인 질의에 대한 정확하고 신속한 응답을 얻기 위해서 대단히 중요한 문제이다. 기존의 뷰 선택 알고리즘들에서는 릴레이션 전체가 실체화 뷰들로서 고려되었다. 그러나, 릴레이션의 부분 대신 전체를 실체화한다는 것은 시간과 공간 비용측면에서 좋지 못한 성능을 초래한다. 따라서, 우리는 기존 뷰 선택 알고리즘들에서의 문제점을 극복하기 위해서 개선된 실체화 뷰 선택 알고리즘을 제안한다. 제안된 알고리즘 ASVMRT(Algorithm for Selection on Views to Materialize using Reduced Table)에서는 먼저 속성-값들의 농도에 기반을 둔 자동 클러스터링을 사용하여 축약 테이블들을 데이터 웨어하우스에서 생성한 다음, 원래의 베이스 릴레이션들의 조합 대신에 축약 테이블들의 조합을 실체화 뷰들로 고려한다. 제안한 알고리즘의 타당성 검증을 위하여 우리는 실험결과에서 시간 및 공간 모두에서 기존 알고리즘들보다 약 1.8배의 성능향상이 있음을 보인다.

      • Safety of modified double-stapling end-to-end gastroduodenostomy in distal subtotal gastrectomy

        <B>Background and Objectives</B><P>Double-stapling end-to-end gastroduodenostomy (DS-BI) has several potential advantages over other anastomotic techniques in that it is a simple procedure, with no additional gastrotomy on the remnant stomach, and less tension on the anastomosis site. We evaluated the safety of DS-BI by comparing it with the hand-sewn Billroth II gastrojejunostomy (B-II).</P><B>Methods</B><P>Medical records of 933 consecutive patients (DS-BI 428, B-II 505) who underwent distal subtotal gastrectomy were retrospectively reviewed. Several clinicopathological features and treatment results were compared between the two groups.</P><B>Results</B><P>The overall complication rates were 9.3% in the DS-BI group and 15.2% in the B-II group (P = 0.007). Anastomosis-related complications, such as anastomosis-site leakage, stenosis, and intraluminal bleeding, did not differ between the two groups (1.2% in the DS-BI group and 1.8% in the B-II group, P = 0.59). All the anastomosis-related complications were managed conservatively. Postoperative mortality rates were 0% in the DS-BI group and 0.4% (2/505) in the B-II group.</P><B>Conclusions</B><P>Modified DS-BI is a safe procedure, with short-term results similar to those of hand-sewn Billroth II anastomosis.
