Background: Online hemodiafiltration (OL-HDF) offers considerable advantages in clearance of molecules of various sizes. However, evidence of clinical effects of OL-HDF is scarce in Korea. In this study, we investigated changes in laboratory values over more than 12 months after switching to OL-HDF. Methods: Adult patients with end-stage renal disease undergoing hemodialysis (HD) were prospectively enrolled in a K-cohort (CRIS no. KCT0003281) from 6 tertiary hospitals in South Korea. We recruited 435 patients, 339 of whom were on HD at enrollment. One hundred eighty-two patients were followed for more than 24 months. Among them, 44 were switched to OL-HDF for more than 12 months without conversion to HD. We used a paired t test to compare baseline and 24-month follow-up results. Results: The mean age of the subjects was 61.2 ± 12.2 years, and 62.6% were male. The baseline hemoglobin level was not significantly different between HD and OL-HDF group (10.61 ± 1.15 vs. 10.46 ± 1.03 g/dL, P = 0.437). However, the baseline serum protein and albumin levels were significantly lower in the OL-HDF group (6.82 ± 0.49 vs. 6.59 ± 0.48 g/dL, P = 0.006; 3.93 ± 0.28 vs. 3.73 ± 0.29 g/dL, P < 0.001). In patients switched to OL-HDF, levels of hemoglobin and serum albumin significantly increased (10.46 ± 1.03 vs. 11.08 ± 0.82 g/dL, P = 0.001; 3.73 ± 0.29 vs. 3.87 ± 0.30 g/dL, P = 0.001). The normalized protein catabolic rate decreased after 24 months, but the change was not significant (1.07 ± 0.25 vs. 1.03 ± 0.21 g/kg/day, P = 0.433). Although the dose of erythropoiesis-stimulating agent was lower in patients who converted to HDF, it was not significantly different (-115.7 ± 189.7 vs. -170.5 ± 257.1 P = 0.206). Conclusion: OL-HDF treatment over more than 12 months was associated with no harmful effects on anemia and nutritional status.

We investigated the characteristics of the marine environment and phytoplankton community of surface in the adjoining sea of Yangtze river of the East China Sea in June 2005. According to the analysis of a T-S diagram, three characteristic of water masses were identified. We classified them into the mixed water mass by the Chinese continental coastal waters and Taiwan/Tsushima warm current, Chinese continental coastal waters and Taiwan/Tsushima warm current. The first water mass was characterized by high water temperature, low salinity, low density and very high Chl-a concentration. The second one was characterized by high water temperature, low salinity, low density and high Chl-a concentration and the last one was characterized by low water temperature, high salinity, high density and low Chl-a concentration. The phytoplankton community identified a total of 95 species belonging to 43 genera. The dominant species was 3 species, that is, mainly centric diatom, Skeletonema costatum in the Chinese continental coastal waters. The others one, one dinoflagellates, Prororcentrum donghaiense and one silicoflagellates, Dictyocha speculum var. otonaria in warm current regions. Standing crops of phytoplankton and Chl-a concentration were very high with ranged from 5.9 x 103 cells/L to 1.1 x 106 cells/L, and 2.90 ㎍/L to 26.40 ㎍/L, respectively. Phytoplankton productivity in the East China Sea was controlled by the discharge of Yangtze river waters which include a high concentrations of nutrients.

최적조류계산을 위한 사용 툴은 비선형 방법 및 선형해법 알고리즘을 이용하여 다양하게 개발되어 왔다. 최적조류계산은 조류계산에서 계산하지 못하는 제어변수의 최적 값을 결정할 수 있는 알고리즘으로서 비용 및 효율성 측면에서 반드시 고찰되어야 할 것이다. 본 논문에서는 PDIPM 기법을 이용한 최적조류계산 알고리즘을 개발하였으며 대규모 계통에 적용하기 위하여 수렴성 및 수행속도 향상을 위한 모선변호 ordering 및 고성능 LU분해 기법 등을 개발하여 적용하였다. 또한 실제 전력계통을 현실적으로 검토하기 위해 필요한 발전기 단자전압 고정 방안에 대하여 수렴성 높은 알고리즘을 제안하였다. 개발된 알고리즘은 수행속도 및 정확성을 검증하기 위하여 2006, 2007, 2009년도 한전계통을 통하여 PSS/E OPF 모듈과의 비교 검증을 수행하였다.

The level of social unrest associated with the income distribution is better represented by bi-polarization than inequality. This article uses the bi-polarization index developed by Esteban, Gradin, and Ray(1999) to analyze how the income distribution has evolved in Korea for the 1997-2003 period. First, comparison of Korea with several OECD countries in the speed of polarization confirms the Shin and Cheon`s(2005) conclusion that the problem associated with the income distribution in Korea lies in polarization rather than in rising inequality. Second, we attempt to characterize the dramatic bi-polarization process of the income distribution. Our calculations show that the mobility from the low to the upper class is reduced as the distribution becomes more bi-polarized. Polarization has more unfavorable effects on the low education group or older household heads(45+) than on the high education group or younger ones(44-). It is further observed that income dispersion is typically greater in the non-manufacturing than in the manufacturing sector and that the rising level of polarization is associated with the great exodus of workers from manufacturing to non-manufacturing. Finally, variation of labor force participation rates across regions and over time is better explained by bi-polarization than by rising inequality.

The aim of the present study was to find the optimum dynamic conformal arc (DCA)-based radiosurgery conditions for treatment of patients with intracranial tumors. DCA plans were generated on a commercial treatment-planning system (BrainSCAN v.5.31) for various beam-number and gantry-angle combinations by using computed tomography (CT) scans to delineate the planning target volume (PTV) and the organs at risk (OARs). To evaluate planning quality related to the various planning conditions, we analyzed the dose quality factors (DQFs) for 22 patients as a function of the number and the total angle of arcs related to the target volume, which were determined on the basis of patient-specific features. When the total angle of arcs was less than 250, the DQF values were under 0.98 (worst group). When the total angle of arcs was more than 350 degrees, the DQF values were maximized, and the highest DQF values (best group, DQF > 0.995) occurred in the range between 50 and 120/cm3 for the total angle/target volume condition. In this optimal range of angle/volume, the asymmetric distribution of the arcs resulted in decreased DQF values. For a relatively large target volume (more than 10 cm3), the DQF values were lower than these for a relatively small target volume (less than 10 cm3). Re-planning results showed that the planning quality could be improved using optimized planning conditions. We expect that this study will provide a basis for the evaluation of various DCA conditions prior to actual planning and treatment.

Purpose: Survivors of childhood leukemia are at risk of growth impairment due to intensive chemotherapy and radiation treatments. This study investigated the auxological and biochemical characteristics of childhood leukemia survivors diagnosed with growth hormone deficiency (GHD) and the changes in these parameters after 1 year of growth hormone (GH) treatment. Methods: A total of 24 children diagnosed with GHD after leukemia treatment was analyzed. Clinical and biochemical data were collected retrospectively at leukemia diagnosis, GHD diagnosis, and 1 year after GH treatment. Standard deviation score (SDS) was calculated based on the age- and gender-adjusted population. Results: Of the 24 children included in this study, 19 received GH treatment. The median age at GHD diagnosis was 12.3 years, and the median delay in bone age was 1.46 years. Height SDS decreased from -0.69 at leukemia diagnosis to -2.58 at GHD diagnosis (P<0.001). The change in height SDS with and without GH for 1 year was 0.35 and -0.21, respectively (P=0.044). In regression analyses, higher height SDS at GHD diagnosis and a smaller decrease of the height SDS between leukemia and GHD diagnoses were positively correlated with height SDS after GH treatment. Conclusion: GH treatment could be beneficial and safe for improving height in childhood leukemia survivors with GHD. Height SDS at GHD diagnosis and reduction of height SDS between leukemia and GHD diagnosis could be potential factors in predicting the therapeutic effects. Close auxological monitoring is recommended for any childhood leukemia survivors who experience posttreatment height decline.

Hypopharyngeal reconstruction is a surgically challenging procedure, and postoperative management is important due to a high rate of complications following surgery. In particular, stricture and fistula formation is the most common long-term postoperative complication. Through systematic review and meta-analysis of 21 studies, a significant radiation effect of stricture and fistula formation was found in patients who underwent hypopharyngeal reconstruction. The perioperative radiation must be seen as a critical factor for stricture and fistula formation in hypopharyngeal reconstruction.
