Chlorpromazine으로 유발된 음경지속발기증 1례

저자들은 항정신병약물 chlorpromazine 치료 중에 음경지속발기증이 발생하여 수술까지 시행하여 회복된 환자 1례를 체험하였기에 문헌고찰과 함께 보고하였다. 항우울제 trazodone과 함께 chlorpromazine, thioridazine과 같은 항정신병약물이 음경지속발기증의 원인과 관련이 있는 것으로 알려지고 있다. 이러한 약물에 의해 유발되는 음경지속발기증의 기전에 대해서는 알파-아드레날린 차단으로 인한다는 설이 많은 지지를 받고 있다. 치료는 보존적인 치료와 외과적인 치료가 있으며, 외과적인 치료로는 음경해면체 천자와 흡입 세척술 그리고 분로를 만들어 주는 방법들이 있다. 음경지속발기증은 신속한 진단과 치료를 하지 않을 경우 영구적인 발기불능을 초래할 수도 있는 부작용이므로 향정신약물 투여시 이에 대한 관심과 주의를 가져야 할 것이다. Antipsychotic drugs can induce several sexual side effects. Priapism, one of the side effects, is defined as "the persistent abnormal erection of the penis, which usually occurs without sexual desire". There has been an increasing number of reports recently linking the etiology of priapism with psychotropic medication. The drugs known to be associated with priapism are psychotropic drugs such as trazodone, chlorpromazine, thioridazine as well as antihypertensives, anticoagulants and so on. The mechanism of a drug which induces priapism is proposed to be mediated by its alpha-adrenergic blocking effect. Prompt diagnosis and treatment are essential. Priapism is a severe side effect, which can lead to impotence if prompt diagnosis and treatment are not carried out. There are medical and surgical treatments for priapism. The surgical treatments usually involve aspiration, irrigation and the creation of shunt. We report a case of priapism which developed after two years of chlorpromazine treatment. He is a 20-year-old man with schizophrenia. He experienced two times of brief episode of prolonged penile erection before developing intractable priapism. He was treated by shunt operation which showed satisfactory relief of priapism. It seemed by this case that duration and dosage of chlorpromazine were not closely related with priapism. Psychiatrist is needed to give attention to the priapism as one of sexual side effects due to antipsychotics though it is not common.
Humanoid Path Planning From HRI Perspective: A Scalable Approach via Waypoints With a Time Index

<P>This paper proposes a path planner for a humanoid robot to enhance its performance in terms of the human-robot interaction perspective. From the human point of view, the proposed method uses the time index that can generate a path that humans feel to be natural. In terms of the robot, the proposed method yields a waypoint-based path, the simplicity of which enables accurate tracking even for humanoid robots with complex dynamics. From an environmental perspective through which interactions occur, the proposed method can be easily expanded to a wide area. Overall, the proposed method can be described as a scalable path planner via waypoints with a time index for humanoid robots. Experiments have been conducted in test beds where the robot encounters unexpected exceptional situations. Throughout these trials, the robot successfully reached the goal location while iteratively replanning the path.</P>
Salmonella choleraesuis 감염에 의한 흉부 농양 1예

저자들은 17세의 양성 흉선종을 동반한 남자 환자에서 Salmonella choleraesuis에 의한 흉부 농양을 경험하였기에 문헌고찰과 함께 보고하는 바이다. Salmonella are motile, gram-negative, non-spore-forming members of the family Enterobacteriaceae. Among nontyphoid Salmonella serotypes, Salmonella choleraesuis shows a high predilection to cause systemic infections in humans. Thoracic infection is a rare complication of Salmonella infection. So far, most of reported cases of empyema caused by Salmonella spp. have involved immunocompromised patients. Herein, as we had experienced one case of thoracic empyema due to Salmonella choleraesuis related thymoma, we report it with review of literature.

Background/Aims: Antibiotic usage and increasingly aging populations have led to increased incidence of Clostridium difficile infection (CDI) in worldwide. Recent studies in Korea have also reported increasing CDI incidence; however, there have been no reports on the long-term outcomes of CDI. We therefore investigated the long-term clinical outcomes of patients with CDI, including delayed recurrence, associated risk factors and mortality. Methods: Hospitalized patients diagnosed with CDI at Seoul Paik Hospital between January 2007 and December 2008 were included. Their medical records were retrospectively investigated. ‘Delayed recurrence’ was defined as a relapse 8 weeks after a successful initial treatment. Multivariate logistic regression analysis was performed to identify risk factors for the delayed recurrence. Kaplan-Meier curves were used to analyze mortality rates. Results: A total of 120 patients were enrolled; among them, 87 were followed-up for at least 1 year, with a mean follow-up period of 34.1±25.1 months. Delayed recurrence was observed in 17 patients (19.5%), and significant risk factors were age (over 70 years, P=0.049), nasogastric tube insertion (P=0.008), and proton pump inhibitor or H2-blocker treatments (P=0.028). The 12- and 24-month mortality rates were 24.6% and 32.5%, respectively. No deaths were directly attributed to CDI. Conclusions: Delayed recurrence of CDI was not rare, occurring in 19.5% of the study population. Although CDI-related mortality was not reported, 2-year (32.5%) mortality rate of CDI patients implies that a CDI diagnosis may predict severe morbidity and poor prognosis of the underlying disease. Dependable Humanoid Navigation System Based on Bipedal Locomotion

<P>In this paper, a dependable humanoid navigation system is proposed by considering many difficulties in humanoid navigation based on bipedal locomotion in an uncertain environment. In particular, we propose a layered architecture to resolve complicated problems through a hierarchical manner. Within the proposed software architecture, a walking path planner, a walking footstep planner, and a walking pattern generator are integrated in a hierarchy to create a reliable motion that overcomes foot slippage and localization sensor noise. Each layer is designed to overcome difficulties originating from bipedal locomotion such as unstable dynamics, inclusion of a sinusoidal noise component in the localization sensor measurement, and disturbance regarding discrete footstepping. The designed navigation system is implemented on a human-sized experimental humanoid platform and is tested for the evaluation of its reliability and robustness in various tasks.</P>
Electrically exploded silicon/carbon nanocomposite as anode material for lithium-ion batteries.

<P>In this work, silicon (Si) containing carbon coated core-shell nanostructures were synthesized by electrical explosion of Si wires in ethanol solution followed by high energy mechanical milling (HEMM) process. Material characterization was carried-out using transmission electron microscopy (TEM), field-emission scanning electron microscopy (FESEM), energy dispersive X-ray spectroscopy (EDS), and X-ray diffraction (XRD) analysis. HEMM led to very fine and amorphous Si particles in the presence of carbon and inactive Silicon-Carbide (SiC) matrix. These Si based nanocomposites, obtained through electrical explosion followed by HEMM (milled sample), exhibited enhanced electrochemical performance than unmilled nanocomposites, when evaluated as anode material for lithium-ion batteries (LIBs). On completion of (the) 1st cycle, milled and unmilled sample(s) showed specific discharge capacities around 825 mAh/g and 717 mAh/g, respectively. Interestingly, the coulombic efficiencies of milled and unmilled samples were 98.5% and 97% after 60th cycle respectively. The enhanced electrochemical performance is attributed to fine and amorphous Si based nanocomposite obtained through HEMM process.</P>
