노인장기요양서비스 전달체계 개선을 위한 기독교노인복지의 제도적 활성화에 관한 연구 : 현직 활동가의 사례를 중심으로

1. 국문초록 우리나라의 급속한 고령화에 대한 노인부양정책으로 2008년 도입한 노인장기요양보험제도는, 많은 긍정적인 성과에도 불구하고 복지의 시장화 정책으로 인하여 서비스의 공공성 문제와 전달체계의 지역적 불균형 문제 등을 중심으로 하는 부작용이 나타나고 있다. 본 논문에서는 그 원인이 과도한 민간장기요양기관 비율로 인한 전달체계의 불안정에 있다고 보아, 장기요양제도의 전달체계 개선을 위한 정책적 제안을 목적으로 기술되었다. 그 제안의 내용으로 특별히 교회의 역할에 주목하여, 기독교 노인복지의 필요성과 적합성을 검토하였고 그를 토대로 장기요양제도에서의 역할을 제안하였다. 이를 위하여 노인장기요양보험제도 일반에 대하여 검토하였고, 지금까지 진행되어왔던 기독교 노인복지의 역사와 실천을 정리하였다. 또한 장기요양제도의 전달체계의 문제점을 정리하기 위하여 많은 통계자료와 학술논문 및 보건복지부의 제2차 장기요양 기본 계획 및 정책제안 등의 자료들을 검토하였고, 그 결과 서비스의 질과 공급 인프라의 지역적 불균형 해결이 가장 시급한 과제임을 알게 되었다. 그렇다면 교회가 전달체계를 담당할 수 있는 적합성을 갖추고 있는지가 확인되어야 하며, 특별히 교회가 지역 친화적 특성 및 공공서비스 적합성을 갖추고 있는지를 정리하였다. 한편, 이런 논의를 보완하기 위하여 민간장기요양기관을 운영하고 있는 기관장들과, 교회 사회복지재단에서 노인복지를 담당하고 있는 책임자의 견해를 참고하고자 사례연구를 진행하였다. 그들의 요구와 문제의식들을 반영하여 두 가지 정책을 제안하면 다음과 같다. 첫 번째는, 재력 있는 중대형교회에서와 교단차원에서 비영리사회복지법인을 설립하여, 전자는 직접 운영하고 후자는 산재한 소규모교회들에게 위탁하여 경영하는 형태로 노인장기요양기관을 운영하도록 하였다. 두 번째는, 건강보험공단은 이로 인하여 확보한 재정과 인력을 사용하여 기존의 민간 중소형 장기요양기관을 적극적으로 지원·육성하여야 한다는 것이다. 장기적으로 그들을 독자적인 영리사업으로 분리시키고, 장기요양보험제도는 사회복지제도의 비영리복지사업이라는 특징을 선명하게 구축하여야 한다. 주제어:장기요양보험, 전달체계, 서비스의 공공성, 지역균형성, 기독교사회복지 활성화 2. A Study on Institutional Activation of the Christian Welfare for Senior Citizens for the Improvement of the Social Welfare Delivery System in the Long-term Care Service - Based on Experiences by Directors of Long-term Care Service Centers - Ha Il-Seon Department of welfare management Graduate school of Baekseok University The Long-term Care Insurance System for the Elderly in Korea, which was introduced in 2008 as the elderly support policy for rapid aging, has many problems as well as its positive outcomes. That is, the marketization of welfare services has resulted in side effects such as the problem of public service and regional imbalance in the delivery system. This paper was described for the purpose of a policy proposal to improve the delivery system of the long-term care system, given that instability in the delivery system was caused by excessive proportion of private long-term care institutions. Especially based on the role of the church, the needs and suitability of Christian welfare for the elderly were reviewed, and then its role in the long-term care system was proposed. To do this, overall system of the long-term care insurance was reviewed and the history and practice of Christian welfare for the elderly were summarized. In addition, a number of statistic data, academic papers and the Ministry of Health and Welfare’s second basic long-term care plan and policy proposal were reviewed to address the problems of the delivery system of the long-term care system. This research showed that the quality of service, and regional imbalance in supply infrastructure was the most urgent task to be resolved. Then, the next step to cover was to check if the church can handle the delivery system, especially if the church is region-friendly and suitable for the public service. Meanwhile, a case study was conducted by referring to the views of directors operating private long-term care service centers, and those in charge of the welfare of the elderly at the church social welfare foundation. Based on their needs and awareness of problems, two policies are proposed. First, it is necessary that medium and large sized churches and the Church group set up a non-profit social welfare foundation, the former managing long-term care institutions directly and the latter entrusting small churches with management. Second, the Health Insurance Corporation should actively support and nurture existing small and medium long-term care institutions using the funds and care-workers that has been saved. As a long term policy, it is important that they should be separated into their own for-profit businesses and the long-term care insurance system should clearly build up the characteristics of the nonprofit welfare business.
