동국대학교 경주병원과 포항병원에서 진단된 암등록환자의 특성과 직업 관련성
동국대학교 의과대학 부속 경주병원과 포항병원에서 1996년부터 1997년까지 2년간 암으로 진단 받아 보건복지부 한국중앙암등록본부에 보고하였던 경주병원 암환자 426명(남자 256명, 여자 170명)과 포항병원 암환자 196명(남자 112명, 여자 84명), 총 622명(남자 368명, 여자 254명)을 연구 대상으로 암의 일반적인 특성을 파악하고, 인체에 발암성이 확정된 물질에서 발생하는 암질환자를 선별하여 직업 관련성 암의 정도를 파악하고자 하였다. 연구 방법은 암등록 조사보고서를 검토하여 대상자 선정을 한 후 일반적 특성을 조사하였다. 다음으로 사용물질 및 작업 공정의 발암 가능성에 따라 발암물질을 구분한 국제암연구기구(International Agency of Research on Cancer; IARC)의 분류에 근거하여 직업성 암으로 분류되는 폐암, 비강암, 방광암, 피부암, 백혈병, 간암 환자 중 40세 이상인 190명(남자 149명, 여자 41명)을 대상으로 의무기록을 검토하여 직업력과 흡연력, 음주력 기재 여부에 대하여 조사하였고, 대상자의 직업력은 개별 전화 면접을 이용하여 조사하였다. 암 환자의 평균 연령은 남자는 61.6±14.6세이었고 여자는 59.7±16.2세이었다. 연령 분포는 남녀 모두 60대, 50대, 70대, 40대, 80대의 순으로 감소하였다. 암발생 부위에 따른 분포는 남자는 위암이 28.0%로 가장 많았고, 폐암 20.7%, 간암 13.9%, 방광암과 대장암 각각 6.3%의 순이었고, 여자는 위암이 26.0%로 가장 많았고 자궁경부암 11.8%, 대장암 10.2%, 간암 7.1%, 유방암 6.3%의 순이었다. 암 진단방법은 조직학적 검사를 실시하여 확진된 경우가 남자는 74.5%, 여자는 83.1%이었다. 직업성 암 조사 대상자에 대한 의무기록 검토에서 직업력은 의사 의무기록지에는 6.1%, 간호일지에는 35.2%에서 기술되어 있었다. 흡연력과 음주력은 폐암환자에서는 93.0%, 간암 환자에서는 85.5%, 방광암 환자에서는 76.2%에서 기록되어 있었다. 전화 면접 조사의 응답률은 직업성 암 조사 대상자 중 남자는 84.6%, 여자는 90.2%이었다. 이들의 직업 분포는 남자는 농업 54.0%, 사무직 19.0%, 상업 15.1%, 생산직 7.9%(9명), 노무직 4.0%(5명)이었고, 여자는 주부 70.3%, 농업 18.9%, 상업 10.8%이었다. 이들에 대하여 세밀하게 직업력을 분석한 결과, 발암물질에 폭로가 확실한 대상자는 없었다. 그러나 폐암환자 중에서 1명이 과거에 주물공장에서 10년간 근무하여 직업적으로 발암 물질에 폭로가 의심 되었다. 본 연구에서 직업성 암을 확진하지 못하였으나 의심이 되는 경우가 있었다. 앞으로 직업성 암은 사회 문제화 될 가능성이 있으며, 암의 발생 원인을 파악하기 위하여 개인의 생활 습관과 직업 관련성에 대한 의사의 관심이 필요하다.
전국 초·중·고등학교의 홍역 예방접종률 및 항체 양성률에 관한 연구
BACKGROUNDS: In Korea, there was a nationwide epidemic of measles in 2000 and approximately 32,000 cases of measles were reported to the KNIH. We proposed a measles seroepidemiologic study to determine the most susceptible age groups for measles and then to implement a catch-up campaign for them. METHODS: Approximately 23,000 students from all 16 provinces in Korea were sampled by a modified population based cluster random sampling method, in which the parents of the selected students provided informed consent. 18,139 students (boys 49.1 %, girls 50.9 %) were enrolled in this study. They were given a questionnaire for their vaccination histories and vaccination records, if they existed, and a serologic test on the measles-specific IgG by an enzyme linked immunosorbent assay. RESULTS: The vaccination record-keeping rates were 33.1 % of elementary school subjects, 14.0 % of middle school students and the vaccination record-present rate was 14.7 % of elementary school subjects. The vaccination rates for the MMR at 12-15 months of age were 86.2 % of elementary school subjects, 80.2 % of middle school subjects. The rate for the MMR at 4-6 years age was 37.7% of elementary school subjects. The history of a past measles infection increased as the age of students increased. Of the 18,139 students tested, 16,212 (89.4 %; 95% C.I.=88.9-89.8) were seropositive, and the seroprevalence was 85.4 % (95% C.I.=84.8-86.2) of elementary school subjects, 92.8% (95% C.I.=92.1-93.6) of middle school subjects, and 94.5 % (95% C.I.=93.8-95.2) of high school subjects for the measles IgG antibody. Of the 980 seronegative students, 952 (97.1 %) had been seroconverted after a revaccination with the MMR. Despite the nationwide outbreak of measles, only 28 % (54 / 188) of schools and 31.5 % (4,414 / 14,032) of students were estimated to be exposed to measles. In multiple logistic regression analysis, a secondary dose (odds ratio, 2.86; 95% C.I.=1.73-4.73), a primary dose (odds ratio, 2.56; 95% C.I.=1.16-5.65), the outbreak in the year 2000 (odds ratio, 1.94; 95% C.I.=1.27-2.96), and the grade in school (odds ratio, 1.69; 95% C.I.=1.04-2.76) were significant factors for the seropositivity. The single dose vaccination (odds ratio, 8.01; 95% C.I.=2.87-22.33), the two dose vaccination (odds ratio, 2.73; 95% C.I.=1.62-4.58), the outbreak in the year 2000 (odds ratio, 1.96; 95% C.I.=1.28-2.99), and the grade in school (odds ratio, 1.69; 95% C.I.=1.03-2.75) were significant predictors for the seropositivity as an alternative. CONCLUSIONS: The coverage rate of the MMR was not high enough to be able to protect against an outbreak of measles, particularly the secondary dose. Therefore, the maintenance of a high coverage rate of a two dose vaccination would be the decisive factor to prevent an outbreak of measles in the future and to eliminate measles in Korea. Furthermore, nationwide catch-up campaign for measles in elementary and middle school students is necessary.
