본 연구는 아급성기 뇌졸중 환자를 대상으로 점진적 이중과제를 활용한 트레드밀 훈련이 아급성기 뇌졸중 환자의 균형과 보행기능에 미치는 영향을 알아보기 위한 연구이다. 25명의 대상자들은 실험군(n=13)과 대조군(n=12)으로 무작위 프로그램을 통하여 배정하였다. 모든 대상자들은 트레드밀 훈련을 4주 동안 주3회, 1일 30분, 총 12회 수행하였다. 실험군은 트레드밀 훈련을 진행하며 동시에 주마다 단계별로 인지과제를 적용하여 이중 과제를 수행하였다. 연구 진행과정 중 실험 군에서 5명, 대조군에서 4명이 퇴원과 실험거부로 탈락하여 총 16명이 연구를 완료하였다. 중재에 따른 효과를 알아보기 위하여 10m 걷기 검사(10-m walk test, 10MWT), 6분 걷기 검사(6-minute walk test, 6MWT)를 통하여 보행 능력을 측정, 일어나 걸어가기 검사(Time up-and-go test; TUGT), 버그 균형 척도(Berg balance scale, BBS)를 통해 균형 능력을 측정하였고, 한국어판 활동 특이적 균형 자신감 척도(Korean version of activity-specific balance confidence scale)를 사용하여 균형자신감을 측정하였다. 모든 연구 대상자들은 사전검사 후 중재를 실시하였고, 모든 중재가 끝난 후 사후검사를 실시하였다. 연구 결과, 측정시기는 모든 항목에서 유의한 차이가 나타났고(p<.05), 군에 따른 주효과는 모두 유의한 차이가 나타나지 않았다(p>.05). 측정시기와 군 사이의 상호작용에 대한 분석에서는 TUG와 BBS 항목에서 유의한 차이가 나타났다(p<.05). 사후검정 결과 군간 비교에서도 TUG와 BBS 항목에서 유의한 차이가 나타났다(p<.05). 군 내 비교에서는 실험 군에서는 모든 항목에서 유의한 차이가 있었고(p<.05), 대조군에서는 10MWT, 6MWT, ABC-K 항목에서 유의한 차이가 있었다(p<.05). 본 연구를 통하여 점진적 이중과제를 활용한 트레드밀 훈련이 아급성 뇌졸중 환자의 균형, 보행능력에 긍정적으로 영향을 미치는 것을 알 수 있었다. 본 연구가 추후 아급성 뇌졸중 환자의 치료를 위한 기초자료로 활용될 수 있을 것으로 기대한다.

The purpose of this study was to explore the effects of treadmill training using progressive dual-task on the balance and gait function of subacute stroke patients.
A randomized study assigned 25 participants to an experimental group (n=13) and a control group (n=12). All participants performed treadmill training 30 min daily, three days a week for 4 weeks. The experimental group completed the by gradually applying the each week while running on the treadmill. Due to discharge and refusal, 5 people from the experimental group and 4 from the control group dropped out, leaving a total of 16 people to complete the study. Walking ability was measured using the 10-m walk test (10MWT) and 6-min walk test (6MWT), and balance ability using the time up-and-go test and Berg balance scale (BBS), and balance confidence was measured using the Korean version of the activity-specific balance confidence scale. After the pretest, all participants received the intervention, and the posttest was administered after all interventions were completed.
The study found a significant difference in the time factor in all items (p<.05), but no significant difference in any of the main effects by group (p>.05). In the analysis of the interaction between the time factors in the groups, there was a significant difference in the and BBS items (p<.05). According to the post hoc test, there was a significant difference between the groups in the TUG and BBS items (p<.05). In comparison, there were significant differences in all items in the experimental group (p<.05), and in 10MWT, 6MWT, and ABC-K items in the control group (p<.05).
This study found that treadmill training using the progressive dual task improved the balance and walking ability of patients with subacute stroke. This study is expected to provide basic data for the treatment of patients with subacute stroke in the future.
