파킨슨병(Parkinson's disease, PD)은 독성물질, 약물, 대뇌손상, 유전적 변형 등에 의해서 발병하는 것으로 알려져 있지만 대부분 그 원인을 알 수 없는 특발성 파킨슨병(idiopathic PD, IPD)이다. 파킨슨병 환자의 말은 사지에서 발생하는 떨림, 느린 운동, 강직, 그리고 자세 불안정 문제에 비해서 상대적으로 그 중요성이 강조되고 있지는 않지만 파킨슨병 환자의 70~90%가 발병 후 말 또는 음성이 변화하였다는 보고를 감안한다면 삶의 질을 저하 시키는 주요 요인 중 일부라고 할 수 있다. 파킨슨병은 특히 발병 후 '말속도 빨라짐, 변화 없음, 말속도 느려짐'으로 일치하지 않는 말 변화가 보고되고 있다.
이에 본 연구는 선행 연구들에 근거하여 파킨슨병 환자군 내에 존재하는 서로 다른 말 특징이 파킨슨병을 유발하는 기저핵의 이상뿐만 아니라 소뇌의 비기능과도 긴밀하게 관련된다는 사실을 증명하고자 한다. 말산출에서 피질하 구조의 역할을 보다 명확하게 구분하기 위해 파킨슨병 환자의 운동저하형 마비말장애를 조음교대운동속도와 문맥발화 과제를 사용하여 파킨슨병 집단과 정상 집단 간의 말속도 수행력을 확인하며, 이 차이를 휴식상태 뇌 기능검사(resting-state fMRI)로 증명하고자 한다.
본 연구는 만 55세 이상의 정상 성인 10명(남:여=4:6)과 서울 소재 종합병원 신경과에서 IPD로 확진된 뇌심부 자극술(deep brain stimulation, DBS)을 받지 않은 PD 환자 20명(남:여=9:11)을 대상으로 하였다. 일차검사를 통해 말산출에 영향을 미칠 수 있는 인지, 언어, 우울을 통제하였으며, 환자군의 경우는 약물이 효과적으로 작용하는 ‘ON’ 상태에서 말 및 뇌영상 자료를 수집하였다.

It has been estimated that about 70~90% of patients with PD undergo a disruption of speech motor control which is known as hypokinetic dysarthia. Change of speech rate (i.e. fast speech rate, unchanged speech rate, slow speech rate) is highly prevalent in PD patients which has a negative impact on their day-to-day communication. A major focus of this study is to obtain more comprehensive data of speech rate using diadochokinetic and contextual speech tasks and support their speech dysfunction with resting-state fMRI.
The first objective of this study is to identify the speech rate using diadochokinetic and three different contextual speech tasks. The second objective is to investigate cerebellar dysfunction existing in PD patients. It was hypothesized that heterogeneous speech rates in PD are associated with loss of dopamine and/or the degeneration of nondopamingeric systems that affected the function of cerebellum. Abnormal basal ganglia signaling might disrupt the cerebellar function and could be related to disturbances of speech performance in PD. We used a resting-state fMRI technique to analyze the intrinsic connectivity in patients with PD within a speech rate network defined by a previous meta analysis study.
