안과 진료 의뢰된 입원 환자에 대한 고찰 = Statistical Observation of the Inpatients Referred for Ophthalmologic Examination

Purpose: To study profiles of inpatients and eye problems evaluated and managed by the ophthalmology consultation service of a large university teaching hospital.
Methods: We evaluated one thousand fourteen patients from April 2002 through December 2002. We recorded demographic information, hospitalization information, and eye examination information, including reason for consultation, type of consultation, ophthalmologic diagnosis.
Results: Approximately half the patients were male. Patients in 7th decade were 199. Endocrinologic devision of internal medicine services accounted for 19.0% of all consultations. Twenty-two percent of all consultations was to rule out diabetic retinopathy. We recorded 64 primary ophthalmologic diagnoses and 40 secondary ophthalmologic diagnoses. The top five common primary ophthalmologic diagnoses were diabetic retinopathy, conjunctivitis, cataract, refractive error and dry eye. Cataract was the most common secondary ophthalmologic diagnosis. There were 266 primary hospital discharge diagnoses. The most common primary hospital discharge diagnosis was diabetes mellitus.
Conclusions:. Many people requiring examination for diabetic retinopathy had no history of previous ophthalmic examination. Therefore, education for patients with diabetes mellitus is needed. This study profiled the typical patients and eye problems seen by an ophthalmologist when inpatient consultation is requested. We hope that this report provides a basic source for planning and management of consultation services in residency training programs.

국문 초록 (Abstract)

목적 : 대학병원의 안과 진료 의뢰된 입원 환자의 안과적 문제에 대해 알아보고자 하였다.
대상과 방법 : 2002년 4월부터 12월까지 본원에 입원 중 안과 진료 의뢰된 1014명을 대상으로 성별, 연령, 의뢰과, 의뢰 이유, 안과 진단, 의뢰과 진단을 기록하였다.
결과 : 남자가 501명, 60대가 199명이었다. 가장 많이 의뢰한 과는 내분비내과였고 가장 많은 의뢰 이유는 당뇨망막병증에 대한 선별 검사였다. 64종류의 1차적 안과 진단과 40종류의 2차적 안과 진단이 기록되었다. 1차적 안과 진단은 당뇨망막병증, 결막염, 백내장, 굴절이상, 건성안 등이었다. 가장 흔한 2차적 안과 진단은 백내장이었다. 의뢰과의 1차적 진단은 266종류이고 가장 흔한 진단은 당뇨였다.
결론 : 당뇨망막병증 선별 검사를 의뢰받은 환자 중 입원 후 처음 안과 진료를 받은 환자들이 많았으며 당뇨망막병증 선별 검사의 중요성에 대한 홍보가 필요함을 알 수 있었다. 입원 환자의 안과 진료가 의뢰되었을 때 안과 의사가 접할 수 있는 전형적 환자 유형과 그들의 안과적 문제에 대한 자료는 안과 전공의 교육 계획 수립에 도움을 줄 수 있을 것으로 생각된다.
