Study Design: Case report.
Objectives: We report the case of a patient with C2 spondylotic radiculopathy who was treated by microscopic posterior foraminotomy.
Summary of Literature Review: C2 spondylotic radiculopathy is rare, but it can occur due to spondylosis, compression by a venous plexus or vertebral artery, or hypertrophy of the atlantoepistrophic ligament.
Materials and Methods: A 64-year-old woman was hospitalized with severe occipital pain radiating toward the left cervical area and posterior to the left ear. It started 3 years previously, and became aggravated 3 months previously. Foraminal stenosis of C1-2 was observed on magnetic resonance imaging (MRI) and degenerative changes of the facet joint of C1-2 and osteophytes originating from the left atlantoaxial joint were shown on computed tomography (CT). Dynamic rotational CT showed narrowing of the left C1-2 neural foramen when it was rotated to the left. Selective C2 root block was done, but the pain was aggravated. Thus, we decompressed the C2 nerve root by microscopic posterior laminotomy of the C1 vertebra. After surgery, the patient’s occipitocervical pain mostly resolved. By the 6-month follow up, pain had not recurred, and instability was not observed on plain radiographs.
Results: C2 Spondylotic radiculopathy was diagnosed by physical examination and imaging studies and it was treated by a surgical approach.
Conclusions: C2 spondylotic radiculopathy should be considered when a patient complains of occipitocervical pain triggered by cervical rotation and C1-2 foraminal stenosis is observed on MRI and CT.

국문 초록 (Abstract)

연구 계획: 증례 보고목적: 일측성 척추증을 동반한 제 2경추증성 신경근증을 이학적 검사 및 영상촬영을 통해 진단하고 현미경하 후방 감압술로 치료한 증례를 보고한다. 선행 연구문헌의 ...

연구 계획: 증례 보고목적: 일측성 척추증을 동반한 제 2경추증성 신경근증을 이학적 검사 및 영상촬영을 통해 진단하고 현미경하 후방 감압술로 치료한 증례를 보고한다.
선행 연구문헌의 요약: 제 2경추 신경근증은 발생빈도가 매우 낮지만 척추증, 정맥총의 압박, 척추동맥의 압박, atlantoepistrophic ligament의 비대 등에의해 발생할 수 있다.
대상 및 방법: 64세 여자환자가 3년 전부터 시작, 3달 전 악화된 좌측 경부 및 귀 뒤로 방사되는 심한 후두하 통증을 주소로 내원하였다. 자기공명영상상좌측 제 1-2경추간 신경공 협착증이 관찰되었고, 컴퓨터 단층촬영상 좌측 제 1-2경추간 후관절의 퇴행성 변화, 좌측 환축추에서 기시한 골극이 관찰되었다. 역동적 회전 컴퓨터 단층촬영상 좌측 회전시 좌측 제 1-2경추간 신경공의 좁아진 소견을 보였다. 선택적 제 2경추 신경근 차단술을 시행하였으나 통증은 점차 악화되었다. 이에 현미경하 제 1경추의 후궁 절개술 (laminotomy)을 시행하여 압박되어 있던 제 2경추 신경근을 감압하였다. 술후 환자의 경부및 후두하 통증은 거의 사라졌다. 6개월 추시시 통증은 재발하지 않았고, 단순 방사선 사진 상에서 불안정성은 관찰되지 않았다.
결과: 이학적 소견 및 영상촬영을 이용하여 일측성 척추증을 동반한 제 2경추증성 신경근증을 진단하였고 이를 수술적으로 치료하였다.
결론: 환자가 경부의 움직임에 의해 유발되는 경부 및 후두하 통증을 호소하고 자기공명영상 및 컴퓨터 단층촬영에서 제 1-2 경추간 신경공 협착증을 보일 때 척추증성 신경근증을 의심하여야 한다.
