상완골 근위부 골절에서의 관절 성형술

국문 초록 (Abstract)

상완골 근위부 삼분 또는 사분 골절과 같은 분쇄 골절, 골절 및 탈구, 상완골두 분리 골절 등은 치료가 어렵다고 알려져 있다. 특히 고령의 환자에서는 이러한 골절에 대한 치료로 상완골두 치환술이나 역행성 견관절 전치환술이 종종 사용되고 있으며, 그 빈도는 증가 추세에 있다. 상환골 근위부 골절에서의 관절 치환술은 많은 경험과 숙련된 기술을 요하는 고난이도 수술이며, 특히 삼각근의 기능 보존, 적절한 치환물의 선택 및 고정, 그리고 결절부의 적절한 고정 및 유합 등이 중요하다고 알려져 있다. 상완골 근위부 골절 치료로 시행된 관절 치환술 후 통증의 해소는 성공적이지만, 운동 범위와 근력의 회복은 환자의 회복에 대한 의지와 그 외 많은 인자에 의해 다양한 결과를 보일 수 있다. 수술의 결과와 관련되는 인자로는 환자의 연령, 수술의 시기, 결절부의 유합 및 적절한 재활 등을 들 수 있다.

다국어 초록 (Multilingual Abstract)

Comminuted proximal humeral fractures with displaced three- or four-part fragments, fracture-dislocations, and humeral headsplitting fractures are very difficult to treat. In older patients, hemiarthroplasty or reverse shoulder arthroplasty is often the indicated treatment. Arthroplasty in this patient cohort is a very technique-dependent procedure, and relies on preserving deltoid function, proper component placement and fixation, and tuberosity fixation. Although pain relief is predictable, it is often difficult to achieve functional improvement. Results depend on the patient"s age, timing of the surgery, tuberosity healing, and adequate rehabilitation.
