Arcus Digma를 이용한 중심위 유도방법의 재현성 비교 = Comparison of the replicability of centric relation registration techniques using Arcus Digma

The purpose of this study was to compare the replicability of centric relation made by means of self-guided method, bimanual mandibular manipulation, chin point guidance, leaf gauge method and gothic arch tracing. The EPA test of Arcus Digma was employed for examining the reproducibility of centric relation. 10 adults with normal occlusion were participated in this study.
The following results were obtained;
1. The trend of condyle position was different depending on different centric relation taking method.
2. Left and right temporomandibular joints showed a little different degree of repeatability.
3. Bimanual mandibular manipulation used Lucia jig gave the highest replicability among all the centric relation recording methods used in this study. Chin point guidance provided the second most repeatable method, and gothic arch tracing showed the third, the leaf gauge method was the forth , the self-guided method was the least.

국문 초록 (Abstract)

중심위는 교합적, 교정적, 보존적 관점에서 진단과 치료에 중요한 요소이며, 중심위에 대한 정의와 채득방법에 대하여서는 학자들마다 다양하게 주장하고 있다. 중심위를 채득하는 방법에는 여러 가지가 있고 임상적으로 가장 중요한 부분 중 하나가 재현성을 확보하는 것이다. 이에 저자는 중심위 채득시 하악 과두의 kinematic axis를 인식하는 Arcus Digma의 EPA test를 이용하여 중심위 채득 방법에 따른 재현성을 비교하였다.
구강 악안면계의 기능이상이 없으며 교합이 정상이고 치아의 교모 및 마모가 심하지 않은 성인 10명을 대상으로 self-guided method, bimanual mandibular manipulation, chin point guidance, leaf gauge method, gothic arch tracing을 이용하여 각 방법 당 중심위를 채득한 결과 다음과 같은 결론을 얻었다.
1. 중심위 채득 방법에 따라 하악과두의 위치는 상이한 양상을 나타내었다.
2. 5개의 중심위 유도 방법에 따라, 좌우 측두 하악 관절에서 약간 다른 재현성의 양상을 나타내었다.
3. Bimanual mandibular manipulation이 가장 높은 재현성을 나타내었고 다음으로 chin point guidance, gothic arch tracing, leaf gauge를 이용한 방법 그리고 self-guided method 순이었다.
