흉추 도수교정이 경추신경병증의 통증, 관절가동범위, 경부장애지수, 정중신경의 변화에 미치는 효과 = Effect of thoracic spine manipulation on pain, range of motion, neck disability index and change of median nerve in cervical radiculopathy

국문 초록 (Abstract)

본 연구의 목적은 경추신경병증 환자에게 적용되는 흉추 도수교정이 가짜 흉추 도수교정과 비교하여 통증과 기능장애에 어떠한 영향을 미치는지 확인하고자 하였다. 연구대상자는 연구 참여에 동의한 경추신경병증 진단을 받은 성인 남녀 30명을 대상으로 흉추 도수교정군과 대조군 각각 15명씩 배정하였다. 평가는 통증의 개선 정도를 확인하기 위해 숫자통증등급척도, 경추 능동관절가동범위, 한국어판 경부장애지수 설문지, 그리고 정중신경의 기계적 변화를 확인하기 위해 초음파 영상 측정을 진행하였다. 기초선 측정 이후 각 그룹에 흉추 도수교정과 가짜 흉추 도수교정을 적용하고 두 군에 동일하게 일반적인 물리치료(전기치료, 열치료)를 30분 적용하였다. 이후 변화를 알아보기 위해 위와 동일하게 재평가를 진행하였다. 통계분석은 윈도우용 SPSS version 22를 이용하였으며, 각 그룹의 일반적 특성을 분석하기 위해 기술 통계, 흉추 도수교정군과 가짜 흉추 도수교정군의 통증, 경추 능동관절가동범위, 경부장애지수, 초음파검사 전, 후, 추적평가 결과 비교를 위해 대응표본 t검정과 ANOVA를 사용하였다(유의수준 0.05). 연구 결과 통증은 중재 전에 비해 추적평가에서 유의미하게 개선되었으며, 관절가동범위는 중재 전, 후, 추적평가 모두에서 유의미하게 증가 되었다. 경부장애지수는 중재 전, 후, 추적평가에서 점수가 유의미하게 개선되었다. 결론적으로 경추 관련 통증을 가지고 있는 대상들에게 흉추 도수교정을 적용하는 것이 통증과 관절가동범위에 개선의 효과를 보였다. 특히 경부장애지수 점수 개선에 즉각적인 효과를 보이므로 적극적으로 사용할 필요성이 있다. 추후 연구에서는 더 많은 대상자를 확보하고 중재 기간을 확대하여 장시간 중재의 효과를 연구할 필요가 있을 것으로 생각된다.

다국어 초록 (Multilingual Abstract)

This study aimed to provide physical therapists with therapeutic knowledge by demonstrating how thoracic spine manipulation applied to patients with cervical radiculopathy affects, such as pain and dysfunction, when compared to sham thoracic spine manipulation.
 The study participants were divided into two groups: an experimental group and a control group, with 15 people in each group. The experimental group performed thoracic spine manipulation, and the control group performed sham thoracic spine manipulation.
 For the evaluation, a numeric pain-rating scale was used to confirm the degree of pain improvement. An iPhone 13 Pro measurement application was used to confirm the change in the cervical active range of motion. In addition, the Korean neck disability index questionnaire was used to assess improvement in daily life, and ultrasonography was performed to confirm mechanical changes in the median nerve. Statistical analysis was conducted using SPSS version 22 for Windows, and to analyze the general characteristics of each group, a sample t-test was performed to compare ANOVA, before, after, and after follow-up results for pain, range of motion, neck disability index, and ultrasonography.
 As a result of the study, the pain was significantly reduced in the follow-up evaluation compared to before and after the intervention, and the range of motion was significantly increased in both before and after the intervention, as well as in the a follow-up evaluation. The neck disability index was significantly reduced in the follow-up evaluation before, after, and after intervention, and the median nerve flatness ratio was improved after the intervention, indicating that the function was maintained at the same level as before intervention.
 The study’s result indicated that thoracic spine manipulation can be used as an effective intervention for pain, range of motion, neck disability index, and median nerve changes. In future studies, more participants should be included to measure the immediate effect at each intervention session and analyze the continuity of treatment by expanding the follow-up period. Research should be conducted to generalize the findings by modifying and supplementing the limitations of this study.
