대퇴 과간 절흔의 형성 부전과 전신적 인대 이완성을 동반한 소아의 습관성 슬개골 탈구에서 “Four-in One” 술식 = 예비 보고

국문 초록 (Abstract)

목적: 하지의 부정정렬이나 슬개골 고위증이 없고 대되 과간 절흔의 형성 부전과 전신적 인대 이완성이 동반된 소아에서의 습관성 슬개골 탈구에 대해 “Four-in-One" 술식을 시행한 후 치료 결과를 알아보고자 하였다.
대상 및 방법: 4명의 환아,5예의 슬관절을 대상으로 하였으며 수술 시 평균 연령은 5.9세였고 술 후 평균 추시 기간은 41.6개월이었다. 술 후 신체검사와 방사선학적 검사 빛 Kujala가 고안한 평가법에 의거하여 임상적 결과를 평가하였다.
결과: 추시 기간 동안 전 예에서 탈구가 재발한 경우는 없었으며 슬관절 동통이나 관절운동의 제한 및 보행 장애도 없었다. Kujala 점수는 평균 96.8점이었으며,2예에서 창상 치유와 관련된 합병증이 있었으나 모두 치유 되었다.
결론: 외측지대 유리술, 내측 강화술, 반건양근 건고정술 및 슬개건 이전술을 복합적으로 적용한 소위 “Four-in-One" 술식은 부정정렬이나 슬개골 고위증이 없고 전신적 인대 이완성과 대퇴 과간 절흔의 형성 부전이 동반된 소아에서 적용될 수 있는 만족할 만한 술식이라 사료된다.

다국어 초록 (Multilingual Abstract)

Purpose: To evaluate the clinical results of a “Four-in-One” procedure for a habitual dislocation of the patella in children with generalized ligamentous laxity and formation failure of the femoral trochlea.
Materials and Methods: Five knees in 4 patients were included in this study. The average age of the patients at the time of surgery was 5.9 years and the subjects were followed up for an average of 41.6 months postoperatively. The clinical results were evaluated using the criteria of the Kujala's scoring system as well as a physical examination and radiological findings.
Results: During the follow-up period, there were no recurrent dislocations, knee joint pain, limitations of motion or gait disturbances in any of the cases. The mean Kujala score was 96.8. Two cases had complications related to wound healing but they healed eventually.
Conclusion: The "Four-in-One" procedure, which include the lateral retinacular release, medial vector augmentation, semitendinosus tenodesis, and patellar tendon transfer is recommended for a habitual dislocation of the patella in children with generalized ligament laxity and formation failure of the femoral trochlea.
