뇌운동피질자극술(MCS, Motor Cortex Stimulation)은 다양한 종류의 만성통증 및 운동장애를 치료하는데 이용되어 왔지만 아직도 그 치료의 메커니즘은 정확히 밝혀져 있지 않다. 지금까지MCS의 효과가 대뇌피질 수준에서만 작용한다는 것과 더 나아가 원심성 경로를 통해 척수 수준에까지 미친다는 서로 대립되는 결과들이 있으며, 제 일차 운동피질이 제 일차 몸감각피질, 시상 뒤배쪽핵, 척수시상로 등을 억제한다는 연구들이 제시되어 왔다. MCS가 통증경감 효과와 소동물에서의 행동변화를 가져온다는 관찰을 토대로 한 이런 실험들이 존재하지만 실제로 MCS가 뇌의 신경활성에 미치는 영향을 다룬 연구 결과들은 거의 보고되지 않고 있다. 최근 고해상도의 micro PET을 이용한 소동물에서의 기능적 뇌영상이 가능해짐으로써 이 논문에서는 [F-18]Fluorodeoxyglucose (FDG)를 이용하여 마취 등 인위적인 제약을 가하지 않은 평상시 상태의 쥐에서의 MCS가 미치는 뇌활동에 미치는 영향을 영상화하였다. 이 실험을 통해MCS가 가해지는 동안 운동피질과 소뇌가 활성화 되었으며 다양한 감각인지관련 영역들의 활성이 억제 되는 것을 관찰 할 수 있었다.

Motor cortex stimulation (MCS) has been seen as a promising and potential surgical option to treat chronic pain in clinical practice. However, the exact mechanism of action of MCS remains to be elucidated. There is debate as to whether it acts at the cortical level or at the level of pyramidal neurons and their efferents. This debate most likely stems from the fact that the primary motor cortex inhibits the primary somatosensory cortex, ventral posterior thalamus, and spinothalamic tract. Although several studies have demonstrated pain reduction on behavioral effects in rodents, few studies have dealt with the neural activity of the brain induced by MCS. This may be because it is difficult to measure functional changes in a conscious rat brain in vivo. Recently, a high-resolution functional neuroimaging technique has become available, the micro positron emission tomography (PET), for visualizing brain activities in small animals. Here, we used the [F-18] fluorodeoxyglucose (FDG) micro PET neuroimaging technology to examine the effects of MCS on brain activity in conscious freely-moving rats. We found that the motor cortex and cerebellum were activated and a diverse of sensory-related areas in the rat brain were deactivated when treated with MCS.
