소아에서의 수근관 증후군은 성인에 비해 드물게 발생하며 대지증(macrodactyly), 유선상과골증(melorheostosis) 같은 국소적 요인과 리소솜 축척증(lysosomal storage diseases)이나 점액다당류증(mucopolysaccharidosis) 등의 유전성 질환 및 특발성 요인이 발생 원인으로 알려져 있다. 면역글로불린(immunoglobulin)은 후천성면역에서 중요한 역할을 담당하며 면역글로불린의 감소는 다양한 형태의 감염, 특히 관절, 골 및 연부조직에 대한 감염의 위험성을 증가시킨다고 알려져 있다. 저감마글로불린혈증이 봉와직염, 관절염 및 골수염을 유발한다는 보고가 있었으나 이로 인한 수근관 증후군은 보고된 적이 없다. 이에 저자들은 저감마글로불린혈증으로 치료를 중단했던 소아에서 발생한 양측 수근관 증후군을 경험하였기에 문헌 고찰과 함께 보고하고자 한다.

Carpal tunnel syndrome occurs infrequently in children. Genetic disorders and systemic disease of carpal tunnel syndrome may be the predisposing factors for carpal tunnel syndrome in children. Immunoglobulins (lg) playa major role in adaptive immunity, and the loss of certain species of immunoglobulin is known to increase the risk of certain types of infection. Patients with hypogammaglobulinemia have an increased risk of cellulitis, arthritis and osteomyelitis. We describe the management of bilateral carpal tunnel syndrome in children with hypogammaglobulinemia.
