      Purpose: To compare clinical and radiological results between closed reduction followed by percutaneous K-wire fixation and volar locking compression plate fixation in the treatment of AO type-C1, 2 fractures of the distal radius.
      Materials and Methods: The subjects of this study were 61 patients who had a AO type-C1,2 fracture of the distal radius treated from March 2006 to January 2010, and were followed up for over 12 months. Clinical evaluations included disabilities of arm, shoulder and hand score (DASH) score, patient-rated wrist evaluation questionnaires (PRWE) score, the Grip strength, and the range of joint motion at the last follow-up, and radiological assessment included the presence of bony union, the dorsal tilt angle of the radius, the inclination of the radius, and the radial shortening.
      Results: The range of joint motion, the grip strength, DASH and PRWE demonstrated no significant difference between the two groups. Radiological results revealed that the patients treated with percutaneous K-wire fixation showed greater radial shortening than those treated with locking compression plate fixation.
      Conclusion: Although clinical results are similar between volar locking plating and percutaneous pinning in the treatment of AO C1, 2 distal radius fractures, volar locking plating is more effective in preventing radial shortening during fracture healing.

      국문 초록 (Abstract)

      목적: AO C1, 2형 원위 요골 골절의 수술적 치료에서 도수 정복 및 K-강선 고정술과 수장부 잠김 압박 금속판 내골정술 간의 임상적, 방사선학적 결과를 비교하고자하였다. 대상 및 방법: 2006년 ...

      목적: AO C1, 2형 원위 요골 골절의 수술적 치료에서 도수 정복 및 K-강선 고정술과 수장부 잠김 압박 금속판 내골정술 간의 임상적, 방사선학적 결과를 비교하고자하였다.
      대상 및 방법: 2006년 03월부터 2010년 01월 까지 AO C1, 2형 원위 요골 골절로 치료 받고 12개월 이상 추시가 가능했던 61명을 대상으로 하였다. 임상적 결과는 마지막 추시 때 disabilities of arm, shoulder and hand score (DASH)와 patientrated wrist evaluation questionnaires 점수 파악력과 관절의 운동 범위를 조사하였고 방사선학적 결과는 골유합 여부, 요골의 배측 경사, 요골의 경사각 그리고 요골의 단축 정도를 측정 비교하였다.
      결과: 임상적 결과에서는 양 치료군 간의 유의한 차이를 보이지 않았으나 방사선학적결과에서는 도수 정복 및 K-강선 고정술을 받은 환자에서 잠김 압박 금속판 내고정술을 받은 환자보다 요골 단축 정도가 유의하게 심한 소견을 보였다.
      결론: 경피적 핀 고정술 또는 수장부 잠김 금속판 고정술로 치료한 AO C1, 2형 원위요골 골절 환자에서 임상적으로 유사한 결과를 보이나 잠김 금속판 고정술이 요골의단축을 방지하는 효과가 더 크다.
