다낭성난소증후군에서 심혈관계질환 위험요인 발생 예측을 위한 추적관찰 연구, 2011 = Follow-up Study of Patients with Polycystic Ovary Syndrome for Predicting Risk Factors of Cardiovascular Disease

국문 초록 (Abstract)

본 용역과제의 목표에 따른 주요사업내용은 다음과 같음.
1. 병원 기반의 전향적 PCOS 환자 registry 구축
- 심혈관질환(고혈압, 이상지혈증 등), 당뇨병 등 심혈관계질환 위험요인의 발생 위험도 산출
- 심혈관계질환의 발생위험과 관련한 병태생리 및 원인을 분석하고 임상적, 생화학적 지표 발굴을 통하여 PCOS 예방, 조기 진단 및 치료기술 개발에 기여
본 연구용역에서는 다낭성 난소증후군 여성의 전향적 추적 관리를 위해 2010.11월 구축, 개설된 웹기반 임상연구 데이터관리시스템(iCReaT) 프로그램의 입력항목으로 다낭성 난소증후군 여성의 심혈관 질환 위험요인의 유병률 및 임상양상, 부인과적 소견, 남성호르몬, 초음파 소견, 신체 검진 소견, 과거력 및 가 족력, 개인의 생활습관력 등 방대한 양의 자료를 서울시대 7개 임상연구네트워크 기관에서 환자진료와 동시에 입력할 수 있도록 안정적인 등록 관리 체계를 구축하였다. 이를 통해 어느 진료의 혹은 연구자 라도 향후 다낭성 난소증후군 환자의 등록 관리 시 이를 기초로 활용할 수 있도록 하였으며 특히 본 증후군을 가진 여성의 심혈관 질환 위험도 발생 추적 관찰 시 유용한 운용체계로 이용될 것으로 사료 된다. 아울러 다기관 연구 혹은 국가단위의 등록관리가 필요한 다른 질환에서도 유용하게 사용될 수 있을 것으로 판단된다. 본 연구자들이 구축한 병원 기반의 전향적 다낭성 난소증후군 환자 registry 및 웹기반 임상연구 데이터관리시스템(iCReaT) 프로그램은 향후 본 증후군을 대상으로 한 지속적인 추적 관찰연구에 유용하게 사용되어 국민 보건향상에 기여하기를 바라는 바이다.
2. 한국 다낭성 난소증후군 여성의 관련 임상자료 및 시료(혈액, DNA) 수집 : 다낭성 난소증후군 유전체 연구에서 가장 중요한 것은 전형적인 임상양상을 보이는 환자군을 대규모로 동원하는 것이며, 본 연구는 서울시내 7개 공동연구병원을 방문한 약 280명 가량의 다낭성 난소증후군 환자의 유전체 시료 및 혈청, 혈장을 질병관리본부에 입고할 예정이다. 이는 향후 한국인 다낭성 난소 증후군 유전체 연구 및 심혈관 질환 연구의 핵심 자료로 활용될 것으로 사료되며, 이를 통해 본 증후 군의 조기진단, 심혈관계 질환 위험도 세분화 및 관리지침 설정에 크게 기여할 것으로 사료된다.

Primary purposes of this project are as belows:
1. Hospital-based prospective registration of Korean patients with polycystic ovary syndrome (PCOS)
- Evaluation of cardiovascular disease (hypertension, dyslipidemia and diabetes) risk factor in Korean women with PCOS
- Investigation of pathophysiology and cause of cardiovascular disease risk factor and contribution to early recognition of cardiovascular disease risk factor in Korean PCOS wome.
In this research service, registration protocol of women with polycystic ovary syndrome (PCOS) was constructed using an Web-based Clinical Study Data Management System (iCReaT). Vast amounts of data about basic clinical aspects of women with polycystic ovary syndrome, such as prevalence of cardiovascular disease (hypertension, dyslipidemia and diabetes) risk factor, gynecological findings, hyperandrogenism, ultrasonography, physical examination, medical and family history, personal life, were entered in this program. At the same time, investigators from seven institutions have entered the patient care management system to build a stable registration. Through this, multi-center web-based registration protocol was constructed. This protocol can be used as a useful operating system in particular follow-up of cohort study of women with PCOS for any practitioners or researchers. The protocol of web-based registration also might be useful in other diseases, multicenter study or management system in the national level. Thus, out team note, with pride, that we have constructed hospital-based prospective registration of Korean patients with PCOS and built the first web-based clinical research data management system. We also confident that the our team's experience and knowhow can be used in continous follow-up study about PCOS, thus contribute to improving national health.
2. Collection of genetic and serum samples of patients with polycystic ovary syndrome for the detection of disease markers : Though the etiology of PCOS has not been elucidated, a number of studies have suggested that genetic factors play important roles in its etiology and pathogenesis. This research collaboration in 7 hospitals collected about 280 samples of patients with polycystic ovary syndrome, and will be delivered to Korea Centers for Disease Control and Prevention. This kind of material can be utilized as the core material for investigation of genetic components and serum marker of cardiovascular diseased in Koreans PCOS patients, which may be different from other ethnities.
