Ascaris lumbricoides infection is usually asymptomatic, but when invasion of the ascaris to the human biliary tract is a prominent medical and surgical problems. Here we have two case reports of common bile duct ascariasis, which clinical manifestations were different each other, one is acute pancreatitis form and the other is acute cholangitis form. The management of biliary obstruction caused by ascaris lumbricoides has usually been surgical, but this report describes the endoscopic removal of the ascaris located in the common bile duct.
