안와에 생긴 에크린한선낭종 1예 = A Case of Orbital Eccrine Hidrocystoma

Purpose: To report the first domestic case of orbital eccrine hidrocystoma.
Case summary: A 40-year-old man visited our clinic after an orbital mass was discovered in the left eye. Orbital magnetic resonance imaging showed a well-defined 1 cm mass on the superomedial side of the orbit, slightly posterior to the globe. The mass was excised completely without rupture through an incision to the upper eyelid skin. The mass appeared more like a cyst than a hemangioma. The diagnosis of eccrine hidrocystoma was confirmed histopathologically. The patient recovered and there was no recurrence at the 6-month follow-up.
Conclusions: Although orbital eccrine hidrocystoma is very rare, it should be included in the differential diagnosis of orbital tumors.

국문 초록 (Abstract)

목적: 안와에 생긴 에크린한선낭종을 국내 최초로 보고하고자 한다.
증례요약: 40세 남자 환자가 건강검진에서 우연히 발견된 좌측 안와 내의 종괴로 내원하였다. 안와 자기공명영상검사에서 경계가 분명한 직경 약 1 cm 크기의 종괴가 좌측 안구의 약간 후방에서 안와의 내상방에 위치하고 있었다. 위눈꺼풀 피부 절개를 통한 종양제거술을 시행하였고, 피막 파열 없이 완전 적출하였다. 수술 당시 종괴는 혈관종의 양상이 아닌 낭종의 형태를 보였으며, 병리조직검사에서 에크린한선낭종으로 진단되었다. 환자는 잘 치유되었으며, 술 후 6개월 까지의 경과 관찰 동안 재발 소견은 없었다.
결론: 안와의 에크린한선낭종은 매우 드물지만, 안와에 생길 수 있는 종양의 하나로서 감별되어야 할 것으로 여겨진다.
