This study was attempted to investigate the pharmacokinetics of theophylline in rabbits with folate induced renal failure. Plasma concentration and AUC of theopylline were increased in rabbits with folate induced renal failure compared with those of normal rabbits. Renal clearance and total body clearance were decreased in rabbits with folate induced renal failure compared with those of normal rabbits. Correlation of serum creatinine concentration and AUC, and correlation of serum creatinine clearance and renal clearance have linear relationship respectively. From the results of this experiment, it is desirable that dosage regimen of theophylline should be adjusted for effective and safe therapy in patient with renal failure in clinical pharmacy practice.

<P>Three rapidly growing mycobacterial strains, QIA-37 T, QIA-40 and QIA-41, were isolated from the lymph nodes of three separate Korean native cattle, Hanwoo (Bos taurus coreanae). These strains were previously shown to be phylogenetically distinct but closely related to Mycobacterium chelonae ATCC 35752 T by taxonomic approaches targeting three genes (16S rRNA, hsp6 and rpoB) and were further characterized using a polyphasic approach in this study. The 16S rRNA gene sequences of all three strains showed 99.7% sequence similarity with that of the M. chelonae type strain. A multilocus sequence typing analysis targeting 10 housekeeping genes, including hsp65 and rpoB, revealed a phylogenetic cluster of these strains with M. chelonae. DNA-DNA hybridization values of 78.2% between QIA-37 T and M. chelonae indicated that it belongs to M. chelonae but is a novel subspecies distinct from M. chelonae. Phylogenetic analysis based on whole-genome sequences revealed a 95.44 +/- 0.06% average nucleotide identity (ANI) value with M. chelonae, slightly higher than the 95.0% ANI criterion for determining a novel species. In addition, distinct phenotypic characteristics such as positive growth at 37 degrees C, at which temperature M. chelonae does not grow, further support the taxonomic status of these strains as representatives of a novel subspecies of M. chelonae. Therefore, we propose an emended description of Mycobacterium chelonae, and descriptions of M. chelonae subsp. chelonae subsp. nov. and M. chelonae subsp. bovis subsp. nov. are presented; strains ATCC 35752(T)(=CCUG 47445(T) = CIP 104535(T) = DSM 43804(T) = JCM 6388(T) = NCTC 946(T)) and QIA-37(T) (= KCTC 39630(T) = JCM 30986(T)) are the type strains of the two novel subspecies.</P>
Cutaneous squamous cell carcinoma arising in a patient with neurofibromatosis type 1: a case report

A 31-year-old man with a past history of neurofibromatosis type 1 presented with 2.5x1.5cm sized, exophytic tumor of back covered with crust and oozing. Histopathologic findings were consistent with poor differentiated squamous cell carcinoma. Neurofibromatosis type 1 (NF1) is an autosomal dominant inherited disease. It is known that the incidence of tumors in patients with NF1 is high compared with the normal population. And majority of the tumors are non-epithelial neoplasms, including neurofibromas, malignant peripheral nerve sheath tumors, gliomas and leukemia. Also, some studies have suggested that patients with NF1 also have a significantly higher risk of certain types of carcinomas, including breast cancer. However, the occurrence of cutaneous squamous cell carcinoma in a patient with NF1 is rarely reported and the association between NF1 and cutaneous squamous cell carcinoma is not established. Herein, we report a case of cutaneous squamous cell carcinoma occurring on the back of a 31-year-old man with neurofibromatosis type 1.
Linear sebaceous hyperplasia following Blaschko's line: Case report and review of published work

Sebaceous hyperplasia is a common and benign disorder of the sebaceous gland. It commonly develops in middle aged to older persons, and usually presents as yellowish, scattered, umbilicated papules on the forehead, nose, and cheeks. However, several unusual variants of sebaceous hyperplasia have been reported, including a premature form, familial form, sebaceous hyperplasia et plaque, and a linear form. Here, we report an unusual case of sebaceous hyperplasia in a 37-year old woman, presenting as linear arranged, yellowish papules on the right post-auricular area. We also reviewed the literature concerning linear sebaceous hyperplasia, and found that almost all cases show the common features of younger age at onset and linear pattern arranged along Blaschko`s lines.

Background/Aims: Data regarding the prognosis of early esophageal cancer are lacking. This study investigated the long-term outcomes and factors affecting the survival of patients with mucosal esophageal squamous cell carcinoma (T1aESCC). Methods: We analyzed the clinical and tumor-specific parameters of 263 patients who received surgical resection (SR; n=63) or endoscopic resection (ER; n=200) for T1aESCC. Underlying comorbidities were scored using the Charlson comorbidity index (CCI). Overall survival (OS) was the primary outcome, and multivariate regression analysis was performed to predict factors for OS. Results: Of the study patients (age, 64.5±8.0 years), the CCI was 1.0±1.4 in the ER group and 0.6±0.9 in the SR group (p=0.107). The 5-year OS rate during follow-up (54.4±20.4 months) was 85.7% (ER group, 86.8%; SR group, 82.4%; p=0.631). The cumulative 5-year incidence of esophageal cancer recurrence was 10.5% in the ER group (vs 0% in the SR group). The overall mortality rate was 12.9% (ER group, 12.0%; SR group, 15.9%; p=0.399). The most common cause of mortality was second primary cancers in the ER group (75%) and organ dysfunction or postoperative complications in the SR group (70%). According to multivariate analysis, only CCI was significantly associated with OS (p< 0.001). The 5-year OS rate in patients with a CCI >2 and in those with a CCI ≤2 was 60.2% and 88.2%, respectively (p<0.001). The treatment method (ER vs SR) was not a significant affecting factor (p=0.238). Conclusions: The long-term prognosis of patients with T1aESCC was significantly associated with underlying comorbidities. (Gut Liver 2021;15:705-712)
Hematoma beneath nail matrix after finger injury

Fingertip injuries are commonly seen, and many of the cases are simple to treat and do not need specialized treatment. However, there are certain conditions where early intervention is warranted for better functional and aesthetic outcomes. A 54-year old man presented with injured fingertip after crushing injury on left 2th finger. Inspection of the digit revealed bluish change of nail plate with erosion on proximal nail fold. Conventional radiograph showed multi-fragmentary fracture of the distal phalanx. After removal of nail plate, nail bed showed bloating. After incision on proximal nail fold, nail matrix showed bloody change with congestion. According to these findings, we diagnosed him as hematoma beneath nail matrix. Then, we incised the transverse line on nail matrix and remove hematoma beneath nail matrix. After this procedure, the patient didn`t complain of any pain. And nail plate showed normal growth without dystrophy. It is difficult to predict solitary nail matrix injury after fingertip injury. Nail matrix injury can cause persistent pain, disturb growth of normal nail plate, and increase the risk of infection. Therefore, evaluating nail matrix in fingertip injury is very important, especially accompanying significant injury to nail fold or digit. And injured nail matrix should be treated by appropriate method.

Blue toe syndrome (BTS) is often defined as the development of blue or violaceous discoloration in one or more digits. Initial cutaneous signs of BTS are usually non-specific, including intermittent pain, firm nodule localized to the lower leg, livedo reticularis, petechial, and purpura. A 82-year old woman presented with a 2-month-history of multiple purpuric patch on both leg with intermittent pain. In physical examination, both dorsalis pedis arteries were normally palpated. At first, the patient was diagnosed as senile purpura. After 2 weeks, she presented with increasing pain, ulceration, and black discoloration on multiple toes and heel. Radiologic examination revealed thinning of leg blood vessels with cholesterol thrombus on aorta. Based on clinical and radiologic findings, she was finally diagnosed as blue toe syndrome. And she underwent toe amputation for gangrenous change of toes. Here, we report a case of blue toe syndrome mistaken for senile purpura because this case highlights the importance of early diagnosis of blue toe syndrome

Some authors found that about 7% of basal cell carcinoma had a previous history of trauma. And basal cell carcinomas have been reported to develop within scars caused by surgery, laser ablation, and other injuries. A 65-year-old man presented with asymptomatic erythematous plaque on right mandible. Histopathologic findings revealed that buds and irregular proliferations of basaloid tumor cells attached to the undersurface of the epidermis. Four years earlier, he had presented with erythematous plaque on same site. Histologic examination of initial lesion was consistent with actinic keratosis, and this lesion was treated by cryosurgery and CO<sub>2</sub> laser ablation. Based on these findings, we diagnosed him as basal cell carcinoma arising from the scar of cryosurgery and laser ablation for a pre-existing actinic keratosis.
Investigation of the effect of SRSF9 overexpression on HIV-1 production

Serine-arginine-rich splicing factors (SRSFs) are members of RNAprocessing proteins in the serine-arginine-rich (SR) family thatcould regulate the alternative splicing of the human immunodeficiencyvirus-1 (HIV-1). Whether SRSF9 has any effect onHIV-1 regulation requires elucidation. Here, we report for thefirst time the effects and mechanisms of SRSF9 on HIV-1 regulation. The overexpression of SRSF9 inhibits viral productionand infectivity in both HEK293T and MT-4 cells. Deletion analysisof SRSF9 determined that the RNA regulation motif domainof SRSF9 is important for anti-HIV-1 effects. Furthermore, overexpressionof SRSF9 increases multiple spliced forms of viralmRNA, such as Vpr mRNA. These data suggest that SRSF9overexpression inhibits HIV-1 production by inducing the imbalancedHIV-1 mRNA splicing that could be exploited furtherfor a novel HIV-1 therapeutic molecule.
